Dynamic hip screw and fibular strut graft for fixation of fresh femoral neck fracture with posterior comminution.

INTRODUCTION: Posterior comminution of the femoral neck fracture is a major cause of delayed and non-union owing to the loss of the buttressing effect against the posterior rotation. When a femoral neck fracture with posterior comminution is anatomically reduced, only the anterior portions of the femoral neck fracture surfaces are brought into contact leaving a posterior defect. The purpose of this study was to evaluate the use of fibular strut grafting and dynamic hip screw (DHS) for fresh femoral neck fractures with posterior comminution in young patient less than 50 years. The Effects of Dorsal Cortical Comminution on Radiographic Results following Percutaneous Pinning for Extra-Articular Colles' Fracture.

A number of studies have demonstrated that dorsal cortical comminution (DCC) can predict redisplacement after nonoperative treatment of Colles' fractures; however, the effects of a DCC defect on radiographic outcomes following percutaneous pinning for dorsally displaced extraarticular Colles' fractures are unclear. We, therefore, performed a retrospective study on 85 patients who sustained such fractures treated with percutaneous pinning within 2006-2009. The main outcome measures included four radiographic parameters, including radial height, radial inclination, radial tilt, and ulnar varianc

Distal radius fracture metaphyseal comminution: a new radiographic parameter for quantifying, the metaphyseal collapse ratio (MCR).

INTRODUCTION: Metaphyseal comminution is widely considered as a key radiographic parameter that predicts fracture instability for distal radius fractures. However, no quantitative parameter is available to measure this degree of comminution. To quantify metaphyseal comminution objectively, the authors devised and validated the metaphyseal collapse ratio, a new radiographic index. MATERIALS AND METHODS: Seventy-four cases of distal radius fracture in the elderly were included in this analysis. After closed reduction, a strictly lateral plain radiograph was obtained and digitally adjusted. The

Calcar comminution as prognostic factor of clinical outcome after locking plate fixation of proximal humeral fractures.

OBJECTIVE: In the treatment of proximal humeral fractures, the decision between open fixation and arthroplasty is often difficult. Applicable radiographic prognostic factors would be useful. The purpose of the present study was to investigate the influence of calcar comminution on the clinical and radiologic outcome after locking plate fixation of these fractures. METHODS: In patients with proximal humeral fractures that were treated by locking plate fixation, fracture morphology and the presence of comminution of the calcar were documented on preoperative radiographs. Follow-up for at least

Radial column and volar plating (RCVP) for distal radius fractures with a radial styloid component or severe comminution.

Individual fracture patterns demand specific and adequate fixation. Locked volar plating has become popular in the operative fixation of distal radius fractures. However, in cases in which there is a radial styloid fragment or in cases of severe comminution, the amount of fixation from volar plating alone can be inadequate and may lead to loss of reduction. The use of locked radial column plates or Kirschner (K) wires provides additional radial column fixation and allows the surgeon to tailor the amount of fixation to the individual fracture pattern. Outlined here is the technique of combining

Median sternotomy and ventral stabilisation using pins and polymethylmethacrylate for a comminuted T5 vertebral fracture in a Miniature Schnauzer.

A 2.9 kg Miniature Schnauzer was referred to our clinic, the Emergency & Critical Care Medicine Service at the Michigan State University Veterinary Teaching Hospital, following a dog fight. Physical examination findings upon admission included multiple thoracic wounds, absence of hindlimb deep pain, and marked Schiff-Sherrington syndrome. Computed tomography imaging revealed thoracic wall penetration and a comminuted T5 vertebral fracture. Thoracic exploration and thoracic wall repair were performed through a median sternotomy. The vertebral fracture was exposed and stabilised intra-thoracical

Incidence of post-traumatic osteoarthritis in olecranon fractures and the role of instability and comminution in its development: a systematic review.

BACKGROUND: Olecranon fractures are common fractures of the elbow and may lead to symptomatic post-traumatic osteoarthritis (OA). The incidence and risk factors for ulnohumeral OA after an olecranon fracture remain uncertain. Therefore, this review aimed to: (1) determine the incidence of OA following isolated olecranon fractures, (2) assess the role of instability and comminution in the development of OA, and (3) assess the impact of OA on patient-reported outcome measures. METHODS: Multiple medical databases were searched for studies containing the terms "olecranon", "osteoarthritis", and "

Analysing plate fixation of a comminuted fracture of the proximal ulna in relation to the elbow joint: a finite element study.

This study investigated the biomechanical behavior of four different screw configurations used to fix comminuted proximal ulna fractures with a locking compression plate (LCP), via a detailed finite element model based on realistic anatomical geometry. The model incorporated realistic anatomical geometry including both cortical and cancellous bone, soft tissue constraints, and loading conditions representing the physiological self-weight of the forearm, with the humerus fixed at its proximal end. The stress distribution on the plate, strain intensity within the bone tissue, and interfragmentar

Intramedullary Fibular Nail Fixation in an Elderly Patient with a Comminuted Weber C Ankle Fracture: A Case Report.

INTRODUCTION: Ankle fractures are among the most common lower extremity injuries, accounting for approximately 9% of all fractures. Various methods exist for the internal fixation of fibular fractures, with open reduction and internal fixation using plates and screws currently regarded as the gold standard. However, plate and screw fixation is highly invasive and may lead to significant post-operative complications. Intramedullary (IM) nails, offering a less invasive alternative, have conventionally been indicated for non-comminuted Weber B and C fractures. To our knowledge, this is one of the

Treatment of elderly comminuted proximal humeral fracture using endosteal anatomical support system: A case report.

INTRODUCTION: Intramedullary anatomical medial strut with allograft bone (IAMSAB), which accommodates the shape of the proximal humeral cavity and provides rotational stability and direct support to the medial column, was successfully introduced to augment Lateral locking plate (LLP) in the treatment of elderly comminuted proximal humeral fractures. Based on the LLP-IAMSAB construct, a newly titanium endosteal anatomical support system (EASS) was developed. CASE PRESENTATION: Reported here is a single case of a highly comminuted proximal humeral fracture. The fractures were treated with EASS.

Finite element analysis of coronoid prostheses with different fixation methods in the treatment of comminuted coronoid process fracture.

BACKGROUND: Comminuted fractures of the coronoid process significantly compromise the stability and function of the elbow joint. Reconstruction of the coronoid process with a prosthesis has been suggested as an alternative to restore the architecture. The purpose of this study was to investigate the strength and stability of various methods for the fixation of a coronoid prosthesis by finite element analysis. MATERIALS AND METHODS: A coronoid prosthesis was designed based on the morphological information from computed tomography images acquired from 64 subjects in whom the top 40% of the coro

Single volar locking plating for the intra- and extra-articular distal radius fractures with dorsal metaphyseal comminution.

Источник пометил публикацию как отозванную

BACKGROUND: Volar locking plating remains a popular method for the surgical management of distal radius fractures. Dorsal metaphyseal comminution (DMC) is a common fracture pattern which weakens the stability during fracture fixation. In this study, we aimed to compare the radiographic and functional outcome of the intra- and extra-articular distal radius fractures with DMC following single volar locking plate fixation. MATERIALS AND METHODS: Patients suffered from a distal radius fracture with DMC were reviewed in the clinical database of the authors' institution between Jan 2016 and Jan 202

Fixation of Small Osteochondral Fragments in a Comminuted Distal Humerus Fracture with Magnesium Bioabsorbable Screws: A Case Report.

The treatment of comminuted distal humeral fractures with free osteochondral fragments is challenging. Osteochondral fragments should be retained whenever possible and secured with implants buried beneath the articular surface to obtain a uniform articular surface. Headless compression screws and K wires are commonly used for this purpose. However, certain complications have been reported with these fixation implants in case of the non-union and osteolysis of the fragments such as migration and cartilage damage. Fixation of osteochondral fractures in distal humeral fractures using bioabsorbabl
