This case of intoxication of two aviators by inhalation of JP-5 fuel vapors emphasizes a dangerous safety hazard. One or both aviators experienced burning eyes, nausea, fatigue, impairment of eye-hand coordination, euphoria, and memory defects when their cockpit became overwhelmed with the odor of JP-5 fuel. Physical and laboratory examinations were normal except for their ill appearance, conjunctivitis, and mild hypertension, which resolved without sequelae. Exposure to JP-5 fuel vapor occurs frequently, particularly after acrobatic flight in some aircraft. The neurologic effects and insidiou

We report the first tetrodotoxin (TTX) poisoning cases from Van, Eastern Turkey, a city located more than 1000 km inland. Three male patients presented in December 2020 after eating Lagocephalus sceleratus, which a local vendor mistook for an edible fish. One patient, a 41-year-old man who consumed the fish liver, rapidly developed severe signs of poisoning (adverse event grade 4) with complete paralysis and respiratory failure. Arterial blood gas analysis showed severe acidosis (pH 7.179) and high lactate levels (5.3 mmol/L). Despite intensive care, including mechanical ventilation and high-d

Anaerobic bacteria of the genus Clostridia are a major threat to human and animal health, being responsible for pathologies ranging from food poisoning to gas gangrene. In each of these, the production of sophisticated exotoxins is the main cause of disease. The most powerful clostridial toxins are tetanus and botulinum neurotoxins, the causative agents of tetanus and botulism. They are structurally organized into three domains endowed with distinct functions: high affinity binding to neurons, membrane translocation and specific cleavage of proteins controlling neuroexocytosis. The increasing popularity of scuba diving has added a new category to the differential diagnosis of headache. Headache in divers, while uncommon and generally benign, can occasionally signify serious consequences of hyperbaric exposure such as arterial gas embolism, decompression sickness, and otic or paranasal sinus barotrauma. Inadequate ventilation of compressed gases can lead to carbon dioxide accumulation, cerebral vasodilatation, and headache. Other types of headache encountered in divers include exertional headache, cold stimulus headache, migraine, tension-type headache, acute traumati

At the Institute for Legal Medicine in Hamburg (investigation period 1983 to 1993) post mortem examinations were carried out in 45 elder people ( > 60 years) who died from major burns. This retrospective epidemiologic and descriptive study evaluates the factors associated with accidents and suicides in relation to the burn surface area, burn depth, location, the cause of death in the burn victims and carbon monoxide or cyanide gas poisoning. Severe burn injuries were the most common cause of death (41%), and in the majority of our cases the traumatic shock led to death during the first two hou…

Hydrogen sulfide (H2S) is one of the most serious toxic gases encountered in forensic practice. Aside from being a by-product of many industrial processes, this gas is naturally produced during the putrefaction of organic substances. We report six autopsy cases of fatal H2S poisonings from inhalation of H2S gas after an occupational accident. These six men died during the unblocking of a wastewater cistern. The first worker died shortly after clearing the obstruction, the other five died, one by one, as they attempted to help their colleagues. The macroscopic and histological findings are disc

The general health of a German shepherd dog had deteriorated slightly when it was found after being loose for one hour. After 10 hours of observation, the dog showed signs of pain for the first time and signs of poisoning, such as tenseness of muscles, slight opisthotonus, regurgitation, salivation, mydriasis, dyspnoea and cyanosis, were observed; it died 15 minutes after showing the first clinical signs but it had no seizures or tetanic spasms at any time. A postmortem examination did not reveal any pathological changes. A screening test for alkaloids was positive for strychnine (strychnidin-

BACKGROUND: Acute poisoning is a common reason for emergency department (ED) visits and intensive care unit (ICU) admission in sub-Saharan Africa. This study aimed to describe the clinical and toxicological characteristics of acute poisonings admitted to the ICU and to identify clinical and circumstantial factors associated with ICU mortality in Côte d'Ivoire.
METHODS: This is a retrospective, observational, single-center study conducted from January 2020 to December 2022. All patients admitted to the ICU for acute poisoning were included. Sociodemographic, clinical, toxicological, and emerge

In the period 1995-1999 there were 388 car exhaust-gas suicides in Denmark. Of these 343 (88.4%) were men and 45 (11.6%) were women, the average age being 47 years. The car exhaust-gas suicides made up 9.3% of all suicides in Denmark in the period. The corresponding rate was 11.7% for men and 3.7% for women. In rural areas a larger part of all suicides were committed with car exhaust-gas compared to the more densely populated areas. Mental disease was diagnosed in 124 (32.0%) cases. A suicide note was found in 165 (42.5%) cases. A hose was fitted to the exhaust pipe in 334 (86.1%) cases. Of th

One of the basic aims of forensic toxicology is the identification of previously unknown drugs and poisons. This is frequently achieved using the combination of gas chromatography and benchtop quadrupole or ion trap mass spectrometers. The influence of matrix loading on the mass spectral quality was tested, and it was found that a realistic amount of matrix changed the pattern of the spectra obtained by the ion trap mass spectrometer. Disturbed mass spectra led to unsuitable suggestions from the library search and thus rendered the identification of the "general unknown" more difficult. On the

Specimens of blood and other body fluids were obtained at autopsy from all deaths in Sweden classified as suicide covering a 10-year period (N=11,441 cases). The mean age (+/-standard deviation, SD) of the victims was 51.3+/-18.8 years with a clear predominance of males 71% (mean age 51.3+/-18.8 years) compared with 29% females (mean age 51.4+/-18.9 years). The concentration of ethanol in blood samples was determined in duplicate by headspace gas chromatography and a mean blood-alcohol concentration (BAC) of 0.1g/L (10mg/100mL) was the cut-off used to identify ethanol positive cases. Hydrogen cyanide gas formation and subsequent contribution to inhalation injuries caused by residential and industrial fires is an increasing concern in public health care. Currently used cyanide antidotes are limited in their routes of administration and mechanisms of action. A potential cyanide antidote based on the [Mo2O2(μ-S)2]2+ core was assessed for its pharmacokinetic behavior and its efficacy in combating acute cyanide poisoning by inhalation in a mouse model. Pharmacokinetic studies revealed rapid absorption of the molybdenum compound into the bloodstream after intraperitoneal injecti

We report a novel and as yet undescribed clinical scenario in a young girl with liver failure, in whom, the liver histopathology was suggestive of alcoholic hepatitis in the background of hepatoportal sclerosis and incomplete septal cirrhosis. An extensive clinical and investigational evaluation revealed chronic consumption of multiple Ayurvedic herbal medications for seizure disease. Six months after stopping herbal medicines, the repeat liver biopsy demonstrated resolution of alcohol-related changes but persistence of classical features of non-cirrhotic portal hypertension. Sulfur mustard and related vesicants are cytotoxic alkylating agents that cause severe damage to the respiratory tract. Injury is progressive leading, over time, to asthma, bronchitis, bronchiectasis, airway stenosis, and pulmonary fibrosis. As there are no specific therapeutics available for victims of mustard gas poisoning, current clinical treatments mostly provide only symptomatic relief. In this article, the long-term effects of mustards on the respiratory tract are described in humans and experimental animal models in an effort to define cellular and molecular mechanisms contributing to …

Most mucocutaneous surfaces of humans harbor a rich indigenous microbial flora with predominance of anaerobes. Anaerobic infections are usually endogenous indicating that they originate from the host's own flora. Important exceptions are botulism, tetanus, food poisoning by Clostridium perfringens, some cases of gas gangrene and cases of hospital-acquired C. difficile-induced diarrhea. Endogenous anaerobic infections often occur in adjacent to the mucosal surfaces. Other organs are infected by penetration or hematogenous spread. A predisposing condition to anaerobic infections is a low redox p

Although the use of cyanides as warfare agents has not been documented since the Iran-Iraq war in the 1980s, there are rising fears of cyanide being used by terrorists. An Al-Qaeda terror plot to use cyanide gas in the London Underground was foiled in 2002. The threat of similar events becomes more imminent in light of the terror attacks in our country and worldwide, accompanied by statements and threats by fundamentalist leaders to employ chemical weapons. Therefore, mass-intoxication with cyanides is not merely a hypothetical scenario. The treatment of cyanide poisoning is under constant eva

A 6.08 kg female stray Arctic fox (Vulpes lagopus) of unknown age was presented with tail gland inflammation. Initial conventional therapy and subsequent tail amputation at a primary veterinary facility resulted in limited improvement. Subsequently, a topical medication red mercuric oxide (Hydrargyri Oxydum Rubrum) was applied for 4 weeks. Although the local infection showed signs of improvement, the fox subsequently developed progressive systemic signs, including anorexia, dark urine, and weight loss, prompting referral. Clinical examination revealed a large amount of cherry-red medication co
