In a randomized controlled trial 50 patients with duodenal ulcer treated by proximal gastric vagotomy (P.G.V.) without drainage were compared with 50 who underwent selective vagotomy and gastrojejunostomy. The clinical results were assessed in 99 patients one to four years after operation. Patients who had undergone P.G.V. had significantly less dumping, nausea, and bile vomiting and fared better in their overall clinical grading. MATERIAL AND METHODS: There were 52 patients who underwent redo gastric surgery after previous resections, drainage and antireflux procedures in 2011-2017 at the Vishnevsky National Research Surgical Center. BACKGROUND/PURPOSE: The aim of this study was to investigate the efficacy of pylorus-preserving pancreatoduodenectomy (PPPD) versus standard pancreatoduodenectomy (PD).
METHOD: A cohort of 80 patients were studied for 5 years using prospectively gathered data. PPPD was performed in 48 (60%) patients, and the other 32 (40%) underwent a standard Whipple's operation with partial distal gastrectomy (PD). In all cases, the gastric or duodenal stump was anastomosed with the first loop of jejunum as a Billroth I-type reconstruction.

We report herein the case of a 59-year-old man in whom blind pouch syndrome was successfully treated by laparoscopic surgery. The patient had undergone distal gastrectomy and Roux-en Y gastrojejunostomy for a peptic ulcer 35 years previously, and had been suffering from watery diarrhea, anemia, weight loss, and pain in the left upper quadrant of his abdomen for several years. The results of duodenal ulcer treatment have been analyzed for 240 patients. Selective proximal vagotomy (SPV) was performed in 198 patients with uncomplicated disease, and SPV with gastroduodenoanastomosis (SPV + GDA) was conducted in 42 patients with different ulcer complications. All the patients were examined by X-ray before the operation and throughout 10-14 days, 2-3 months, 1 year and so on each year within 10-15 years. Cancers of the head of the pancreas are only resectable in 10 to 25% of cases. The majority of patients are therefore candidates for a palliative treatment jaundice, pain or upper gastro-intestinal obstruction. A clinical series of 208 patients who had elective operations for gastroduodenal ulcer in Surgical Department II, Oslo City Hospital, has been reviewed. The study concentrates on postoperative gastric retention, comparing the frequency of this complication following antrectomy and gastroduodenal anastomosis with and without vagotomy. The patients in the nonvagotomy group had no retention problem.
