OBJECTIVE: To review prevalence, causes, types, and appropriate therapy to treat urinary incontinence (UI) in geriatric patients.

Comprehensive geriatric assessment (CGA) consists of medical history taking, medication review, physical examination with special attention to sensory and neurological evaluation, and use of assessment instruments to measure cognitive, affective, functional, social, and economic status. CGA is most often beneficial when applied to frail elders at risk of functional decline. Beneficial outcomes may include decreased risks of falls, cognitive decline, and nursing home placement--in addition to improved sense of well-being and satisfaction with care.

Fractures in the elderly patient require specialized management encompassing a complex care-providing concept that involves particular expertise, specific instruments, interdisciplinary cooperation and comprehensive aftercare. This latter aspect needs to be extended to include the provision of care in the patient's home. Surgical correction of a fracture alone can no longer be considered sufficient management.

OBJECTIVE: To examine the utility of a short depression inventory (SDI) for use with elderly medical inpatients.
METHOD: Subjects were 132 consecutive admissions to the inpatient geriatric rehabilitation unit at MetroHealth Medical Center, a general medical teaching hospital in Cleveland, Ohio. Sixty of the original sample qualified for inclusion in the final sample. Within the first five days of admission, all subjects were administered the Mini-Mental Status Examination (MMSE). Geriatric patients develop infections, but many have a different appearance from what usually is expected. The difference depends on the age and immune status of the patient and the virulence of the organism. Differences may make recognition more difficult. Therapy may require different doses. The older population, the group of the very old in particular, is increasing and thus the risk group for the two most important psychiatric disorders, dementia and depression. Many individuals become mentally ill for the first time late in life, mainly because of losses, loneliness and somatic diseases. Therefore, beside physical examination, screening for depression and cognitive testing are very important for the general practitioner. Relatives and caregivers often play a keyrole. They have to be involved in evaluation and therapy and their needs for support and advice should be considered.

There remains some controversy and confusion regarding arthroscopic surgery of the knee in the elderly population, particularly for patients with osteoarthritis. A retrospective review was undertaken of 105 knees in patients 60 years or older who had arthroscopy during a 4-year period. One hundred and fifty-nine patients attending a geriatric medical outpatient clinic were screened for depression using the geriatric depression scale (GDS) and brief assessment schedule depression cards (BASDEC).
