Gingival involvement in oral paracoccidioidomycosis.

BACKGROUND: Paracoccidioidomycosis, a deep mycosis endemic in parts of Latin America, often presents with oral lesions involving the gingiva. Nevertheless, the periodontal literature is devoid of references to oral paracoccidioidomycosis. The purpose of this study was to characterize the gingival involvement in oral paracoccidioidomycosis and to contrast clinical and histopathologic diagnosis of the disease. Differential diagnosis and management of oral paracoccidioidomycosis were reviewed. METHODS: From January 1995 to October 2006, the files of the Oral Pathology Laboratory, School of Denti

Phenobarbital-induced gingival hyperplasia.

AIM: The aim of this article is to present a case of a phenobarbital-induced gingival hyperplasia (GH), discuss possible etiological mechanisms of drug-induced GH, and to present a concise review of the literature. BACKGROUND: GH is a well-known complication associated with anticonvulsant phenytoin, antihypertensive calcium channel blockers, and immunosuppressant cyclosporine therapy. Sodium valproate and primidone has very rarely been found to cause GH. REPORT: An extremely rare case of phenobarbital-induced GH in a 28-year-old male patient who had received this drug for three years is pres

The etiology and prevalence of gingival recession.

BACKGROUND: Gingival recession in its localized or generalized form is an undesirable condition resulting in root exposure. The result often is not esthetic and may lead to sensitivity and root caries. Exposed root surfaces also are prone to abrasion. The purpose of this article is to describe the prevalence, etiology and factors associated with gingival recession. TYPES OF STUDIES REVIEWED: The authors reviewed cross-sectional epidemiologic studies of gingival recession and found that they correlated the prevalence of recession to trauma, sex, malpositioned teeth, inflammation and tobacco co

Repositioning of the gingival margin by extrusion.

In this case report, orthodontic intervention was used to move the gingival margin of a maxillary canine incisally by almost 9 mm to mimic a lateral incisor. Increasing the thickness of the labial plate of bone of the canine and subsequently increasing the thickness of the attached gingiva before extrusion prevented gingival recession at a later stage. In many situations, orthodontic treatment can achieve results that could not be attained by restorations and other means of cosmetic dentistry, especially when dealing with gingival margins and gingival height. A step-by-step approach to achievi

Drug-induced gingival enlargements.

There are many types of gingival enlargements which vary according to the etiologic factors and pathologic processes that produce them. The most common drug-induced gingival enlargement is associated with Dilantin (phenytoin). Cyclosporin and calcium channel blockers, which are increasingly used for some cardiac diseases, immunosuppression and autoimmune disorders, also cause gingival enlargement. The term "gingival hyperplasia" is an inappropriate term because enlargement is not the result of an increase in the number of cells, but rather an increase in extracellular tissue volume. Perception of gingival bleeding by Nigerians.

Two hundred and seventy-five (275) subjects were seen to assess their perception of gingival bleeding and relate it to their periodontal status, sex, and age. Only about a quarter of the subjects claimed to bleed from the gingivae during routine oral prophylaxis. Unfortunately, about the same number admitted to having gum disease, whereas during clinical oral examination it was discovered that all these subjects had gingival bleeding. Also this study showed that more of the subjects who claimed to bleed from the gums thought it was normal compared to those who claimed not to bleed from the gum

Gingival enlargement induced by drugs.

Gingival enlargement, an abnormal growth of the periodontal tissue, is mainly associated with dental plaque-related inflammation and drug therapy. Its true incidence in the general population is unknown. Gingival enlargement produces aesthetic changes, pain, gingival bleeding and periodontal disorders. Although gingival overgrowth has been traditionally recognised as an adverse effect of phenytoin therapy, it has recently been reported in association with the use of cyclosporin and calcium antagonists. These 3 classes of drugs produce important changes in fibroblast function, which induce an i

The ultrastructural examination of gingival fibromatosis.

Hereditary gingival fibromatosis (GF) is a special type of fibrous overgrowth classified as non-inflammatory gingival enlargement. Microscopically, the connective tissue consists of coarse collagen bundles and fibroblasts. The ultrastructural examination of fibrous gingival hyperplasia reveals that fibroblasts phagocyte the mast cell granules and mast cells stimulate collagen synthesis which results in hyperplasia. In the ultrastructural examination of phenytoin-induced hyperplasia, fibroblasts, phagocytosing mast cell granules were also found. Based on these findings, the purpose of this stud

Effectiveness of bromochlorophene on gingival health.

The effect of a commercial toothpaste containing 0.2% bromochlorophene (BCP) on established plaque and gingivitis was compared with placebo. The two toothpastes were similar in packaging, taste, consistency, and color. Thirty-two patients with chronic gingivitis were enrolled in this randomized, double-blind comparison and were asked to brush twice a day with the given toothpaste without changing their oral hygiene habits. Plaque index, gingival index, gingival crevicular fluid, and papillary bleeding index were recorded at baseline and after 3 weeks. Significant decreases in gingival and papi…

Focal gingival hyperplasia in a horse.

A diagnosis of gingival hyperplasia in a 22-year-old Quarter Horse gelding was confirmed by histologic examination. Clinical signs included difficulty eating hay, and a large, intraoral soft tissue mass measuring 13 x 8 x 4.5 cm. The mass was located in the mucosa on the lingual aspect of the caudal left portion of the mandible, ventral to the base of the tongue, and covered the second and third lower molars, extending rostrally along the buccal mucosa to the premolars. The left maxillary second and third molars were overgrown with sharp edges. Lateral radiography of the mandible revealed abse

Gingival lesions in lipoid proteinosis.

Lipoid proteinosis is a rare heritable disease of the skin and mucous membranes characterized by subepithelial deposits of hyaline material. The morbid factors include disfiguring papulo-nodular lesions particularly of exposed skin, hoarseness of voice due to vocal cord infiltration, nodular deformation of the eyelids, and board-like rigidity of the tongue. Infiltration of the gingiva is usually unreported, and when mentioned has been equivocally related to the overall disease process. This patient demonstrates striking ulceration and hypertrophy with histologic evidence of heavy deposition of

[Periodontology and esthetics: the gingival recession].

Gingival recessions are regarded by many people as an esthetical problem. Successively the etiology, the significance and the indications for therapy are discussed. Different kinds of therapy such as oral hygiene instruction, the free gingival graft and various pedicle grafts are explained. Finally it is advocated that surgical kinds of therapy have to be applied with reservedness.

[Langerhans cells in the gingival epithelium].

Langerhans cells (LCs) were specifically demonstrated by monoclonal antibody OKT 6. In healthy gingival tissue LCs were mainly seen in stratum spinosum of the surface epithelium. They had small nuclei and with long cell processes. The LCs of oral epithelium in marginal gingivitis and adult periodontitis tissue were more in numbers than in healthy tissue. In juvenile periodontitis tissue LCs numbers seemed to be increased obviously in comparison to the healthy tissue. The LCs were round and often located in both deep spinous and basal layers. These results demonstrate that LCs play an important…

[Gingival recession: what to do?].

The qualitative and quantitative character of the marginal gingiva is not the only cause of root exposure, as mechanical factors can favorize and/or aggravate gingival recession. Suggested therapy is dependent on patient motivation and the evolution of the lesions. Even though there are multiple possibilities for the treatment of gingival recession, one must remain prudent when restoring areas of root exposure, as no protocol exists today which can assure constant success.

Occurrence of gingival recession in adults.

The occurrence of gingival recession was investigated in adults by age and gender and in relation to their dental status and frequency of toothbrushing. A total of 258 dentate subjects were clinically examined. Their mean age was 46 years and they had an average of 19.4 natural teeth. Gingival recession was recorded as present if any root surface was clearly visible without retraction of the gingival tissue. Recession was found on at least one tooth surface in 68% of subjects. Mean number of surfaces with recession was 7.2 for women and 10.4 for men. Subjects with gingival recession had fewer
