[Scientific controversy to push the progress of glaucoma practice--inspiration by the debate on the classification of primary angle closure glaucoma].

The highlight in the scope of primary angle closure glaucoma is the new classification and criteria of early diagnosis of the condition. It is the different point of view to the same academic problem that raise the scientific controversy mentioned above. This short paper might offer an insight into the argument background, focus view, and some possible benefit to push the progress in the diagnosis and management of primary angle closure glaucoma.

Cortical gradient compression links cognition and transcriptomic signatures in primary angle-closure glaucoma.

The cerebral cortex is organized along hierarchical gradients that integrate sensory and cognitive functions, yet whether this organization is disrupted in primary angle-closure glaucoma and contributes to cognitive impairment remains unclear. Here we examine cortical functional gradients in individuals with primary angle-closure glaucoma using resting-state functional magnetic resonance imaging and transcriptomic analyses. We show that patients exhibit compression and reorganization of cortical gradients, particularly within visual and default mode networks, and that these alterations are ass

Research progress of corneal characteristics and changes in primary angle‑closure glaucoma (Review).

The cornea plays a crucial role in the refractive system of the eyeball, and its changes can significantly impact the visual quality of patients with glaucoma. In patients with primary angle-closure glaucoma, several corneal alterations occur, including changes in corneal thickness and curvature, modification in corneal epithelial cells and nerves, activation of inflammatory cells, and variations in the number and morphology of stromal and endothelial cells. These morphological and structural changes in the cornea are mainly influenced by acute or chronic elevation of intraocular pressure and

Aqueous Misdirection After Trabeculectomy in a Down Syndrome Patient With Angle-closure Glaucoma.

Down syndrome is a genetic disease caused by trisomy of chromosome 21 that is characterized by numerous systemic abnormalities including intellectual disability, stereotypical facies, and congenital heart malformations. Ocular abnormalities are commonly seen with Down syndrome including corneal disease (keratoconus), refractive error, and atypical irides (Brushfield spots). We report the first case of aqueous misdirection in a patient with Down syndrome after trabeculectomy. Patients with Down syndrome often have small, hyperopic eyes with narrow iridocorneal angles and may be at increased ris

Aqueous humor proteomic analysis of acute angle-closure glaucoma with visual field loss.

BACKGROUND: Acute angle-closure glaucoma (AACG) is an ophthalmic emergency that occurs over the course of hours or days and may cause irreversible blindness if not treated immediately. In most cases, optic nerve damage is the cause of visual field (VF) loss in AACG. There has been no reliable biomarker found to evaluate optic nerve damage to date. Aqueous humor (AH) proteome analysis might reveal the proteomic alterations in AACG and provide helpful clues in the search for an AH biomarker of optic nerve damage and VF loss. METHODS: In this study, we used the AH proteome to explore the functio

Non-contact tests for identifying people at risk of primary angle closure glaucoma.

BACKGROUND: Primary angle closure glaucoma (PACG) accounts for 50% of glaucoma blindness worldwide. More than three-quarters of individuals with PACG reside in Asia. In these populations, PACG often develops insidiously leading to chronically raised intraocular pressure and optic nerve damage, which is often asymptomatic. Non-contact tests to identify people at risk of angle closure are relatively quick and can be carried out by appropriately trained healthcare professionals or technicians as a triage test. If the test is positive, the person will be referred for further specialist assessment.

Altered functional connectivity density in primary angle-closure glaucoma patients at resting-state.

BACKGROUND: Primary angle-closure glaucoma (PACG) is a neurodegenerative disease. Previous structural and functional studies of functional magnetic resonance imaging (fMRI) have demonstrated widespread dysfunction of spontaneous activity in the PACG brain. In this study, we applied a data-driven graph theory approach of functional connectivity density (FCD) mapping to investigate the altered local and global functional connectivity (FC) of the cortex in PACG. METHODS: Forty-five PACG patients (53.28±10.79 years, 17 males/28 females) and 46 well-matched healthy controls (HCs) (52.67±11.01 year
