OBJECTIVE: To report on a patient with clear gelatinoid deposits on the iris and in the anterior chamber (AC).
DESIGN: Observational case report.
METHODS: Examination of AC aspirate and iris and conjunctival biopsies by light and transmission electron microscopy.
MAIN OUTCOME MEASURES: Clinical and histopathological findings.
RESULTS: Analysis of the aqueous humor and iris ultrastructure disclosed the presence of urate deposits.
CONCLUSIONS: Urate deposition in the iris and anterior chamber rarely occurs.

Clinical identification of tapioca melanoma of the iris is important because its medical treatment may differ from that of other malignant iris melanomas. The characteristic iris nodules must be differentiated from granulomatous uveitis, metastases, and Lisch nodules (neurofibromatosis). We will discuss the anterior segment findings, secondary glaucoma, and fluorescein iris angiographic and histopathologic data from two patients, one with a single nodular type and one with a seeding type of tapioca melanoma of the iris.

Ultrasound Biomicroscopy is a newly developed high resolution imaging method that uses high frequency ultrasound (50-100 MHz). Tissue penetration is about 4 mm. This method allows detailed observation of anterior and posterior chamber anatomy in the living eye, and is thus a useful tool in both clinical assessment of glaucoma, and research into causes of various glaucoma types.

Blood may be refluxed into Schlemm's canal in the normal eye by occlusion of recipient veins in a sector of the conjunctiva and episclera. The blood makes the anterior margin of the canal distinct near the midportion of the trabecular mesh at the same place in which it is seen after hypotony. Circumferential flow in segments of the canal is possible when a pressure head exists to promote such flow. Evidence was found that the canal is not partially collapsed in the normal eye.

The contrast is described between two different types of coloboma of the lens: congenital and traumatic origin. The former is combined with epicanthus, unilateral coloboma of iris, retina and choroid. There seems to be a close relation between coloboma of the lens and a defect of the zonule at least during growth of lens.

This case report aims to present the findings of in vivo confocal microscopy (IVCM) and anterior segment optical coherence tomography (AS-OCT) in three patients with iridocorneal endothelial (ICE) syndrome.
