Clinical significance of adverse outcomes of placental abruption developing at home.

We reviewed the obstetric records of 40 singleton deliveries complicated by placental abruption which developed at patients' homes after 24 weeks' gestation. Of the 40 cases complicated by placental abruption which developed at home, 13 cases (33%) were defined as showing adverse outcomes (5 cases of IUFD and 8 cases of UA pH < 7.0). The rate of patients who complained of abdominal pain without bleeding was higher in the adverse outcome group than in the control group (p = 0.02). The average time interval between the onset and hospital (or clinic) visit in the adverse outcome group was longer

Promoting physical activity and physical functioning among residents in nursing homes - lessons learnt by the PROGRESS project.

INTRODUCTION: Nursing home residents show high sedentary behavior and limited physical functioning, while structural constraints hinder sustainable prevention. This study examined how group-based physical exercise and environmental interventions, such as activity-promoting materials and interior design, can be integrated into everyday care to promote a physical activity-promoting culture in nursing homes and identified key organizational and individual determinants of sustainable implementation. METHODS: PROGRESS was a participatory, cluster-randomized cross-over trial conducted in seven nurs

National randomized controlled trial of virtual house calls for Parkinson disease.

OBJECTIVE: To determine whether providing remote neurologic care into the homes of people with Parkinson disease (PD) is feasible, beneficial, and valuable. METHODS: In a 1-year randomized controlled trial, we compared usual care to usual care supplemented by 4 virtual visits via video conferencing from a remote specialist into patients' homes. Primary outcome measures were feasibility, as measured by the proportion who completed at least one virtual visit and the proportion of virtual visits completed on time; and efficacy, as measured by the change in the Parkinson's Disease Questionnaire-3

A prospective study of the consequences of marital disruption for adolescents: predisruption family dynamics and postdisruption adolescent adjustment.

This study employed data from the National Longitudinal Study of Adolescent Health to examine the consequences of marital disruption for youth prospectively. Using a diverse sample of 6,416 youth, we examined pre- and postdisruption group differences among youth in subsequently disrupting and continuously intact homes. We also examined the extent to which predisruption family dynamics (e.g., marital discord) accounted for both pre- and postdisruption group differences in youth functioning and examined the interaction between level of marital discord and marital disruption. Group differences in

Effectiveness of a structured pharmaceutical review of pharmacotherapeutic plans to reduce drug-related problems in nursing homes: a study protocol of a pragmatic quasi-experimental trial.

BACKGROUND: Patients living in nursing homes have increased aging, comorbidity, spending on health resources and mortality. High drug use is associated with an increased risk of falls, disability, and death. It is estimated that 20%-50% of inappropriate medications are consumed in the elderly. OBJECTIVE: This abstract outline the study protocol for evaluating the effectiveness of a pharmaceutical intervention designed to optimize medication use among nursing home residents, with a particular focus on reducing drug-related problems (DRPs) and minimizing polypharmacy. HYPOTHESIS: The pharmaceu

Enduring Challenge of Latent Tuberculosis in Older Nursing Home Residents: A Brief Review.

Tuberculosis (TB) kills almost 4,000 people a day and is competing with human immunodeficiency virus/acquired immune deficiency syndrome (HIV/AIDS) as the most deadly infectious disease in the world. The gold standards of detection and management of latent tuberculosis infection (LTBI) have not been successful in complete eradication of the disease. Current screening modalities of TB include tuberculin skin testing (TST) and/or interferon-γ release assay (IGRA). However, these screening tests have been heavily studied in healthy populations but not in the elderly who are more likely to have mu

Comparison of the effectiveness of different home-based exercise programs in donors after liver transplantation: a randomized controlled trial.

BACKGROUND: The home exercise programme is a treatment method that ensures the continuity of rehabilitation after discharge. Although it is known that home exercise programmes improve recovery in patients compared to those who do not exercise at all in different diseases, it is not known which home exercise programme is more effective in donors after organ transplantation. METHODS: A total of 117 donors were included in the study. The participants were randomly divided into three groups: control, home exercise, and video-based exercise groups. While no intervention was made to the control gro

Outcomes of Cancer Patients Who Seek Outpatient Visits Despite Post-chemotherapy Decline.

INTRODUCTION: The medical needs of cancer patients are complex and increase with the disease's progression. However, Japan's aging population and increased medical needs have produced challenges like a shortage of hospital beds. Therefore, patients who have completed chemotherapy are recommended home-based care or referred to local palliative care facilities, especially when cancer treatment is no longer a viable option and outpatient visits become unfeasible. However, some patients still strive to continue outpatient visits, which could result in increased pain and possible urgent hospitaliza

The importance of atmospheric odours for the homing performance of pigeons in the sonoran desert of the southwestern united states.

The importance of atmospheric odours for homing pigeon navigation in a desert environment was tested using birds from two lofts located in the Sonoran desert near Tucson, Arizona, USA. When released from a familiar training site, experienced control pigeons and pigeons given intranasal injections of zinc sulphate to produce anosmia both displayed good homeward orientation and homed rapidly. When released from two unfamiliar locations, in contrast, the controls continued to display good homing performance while the zinc-sulphate-treated pigeons homed poorly. Significant differences in vanishing

"I thought he had longer than that": family caregivers' experiences of grief, loss, and bereavement in residential aged care.

BACKGROUND: Residential aged care is increasingly becoming the final place of care for many older adults in high-income countries. For many families, grief begins well before the death of their loved one, emerging as early as the time of entry into residential care. This grief often continues as they witness ongoing decline and deterioration and extends into bereavement following the resident's death. While caregiver grief is well recognised, there is limited understanding of how these experiences unfold across this full journey and the specific challenges families face at each stage. This stu
