Monitoring the course of platelet function in HELLP (haemolysis, elevated liver-enzymes and low platelets) syndrome is important for clinical decision-making. We present a primigravid woman developing HELLP syndrome at 29 weeks and 6 days. Platelet function was monitored by multiple electrode aggregometry (MEA), platelet function analyzer (PFA-100®), platelet count and mean platelet volume (MPV) over an 11-day period. MPV and PFA-100® seem better predictors for platelet function than platelet levels.

The following is a case report of a primiparous woman who developed fulminant liver failure in the setting of HELLP syndrome complicated by hepatic rupture. It is unique in that a timely ABO compatible liver donor was unavailable, necessitating the transplantation of an ABO incompatible organ. Despite aggressive therapy, severe reperfusion injury and humoral rejection dictated retransplantation with an ABO compatible organ on postoperative day 15, resulting in rapid clinical recovery.

BACKGROUND: The occurrence of hemolysis, elevated liver enzymes, low platelets (HELLP) syndrome before 20 weeks of gestation is extremely ra … tric conditions. Within 11 days of admission her condition deteriorated and the diagnosis of severe HELLP syndrome was supported by the findings of hemolysis, elevated liver enzymes, and severe thrombocytopenia, all of which resolved after termination of the pregnancy.
CONCLUSION: Clinicians should consider the diagnosis of HELLP syndrome in women presenting wi …

Spontaneous intrahepatic hemorrhage with or without subcapsular extension is a rare and grave complication of pregnancy. We present a 22-year-old lady in whom liver rupture was noted on emergency caesarian section and later confirmed on contrast-enhanced CT scan. She gradually recovered following selective hepatic angiography and embolization.

Peripartum cardiomyopathy (PC) and preeclampsia with HELLP syndrome are serious complications of pregnancy, but the coincidence of both in one pregnancy is extremely rare. Here, we report a case of 32-year-old primipara who in 35th Hbd presented for the first time in her life symptoms of severe heart failure (HF) in NYHA class III/IV. In 37th Hbd the diagnosis of PC was established based on clinical status and echocardiographic examination, which demonstrated a dilatation of heart chambers and impaired left ventricular systolic function with decreased ejection fraction (EF) 17%. In 37th Hbd sh…

BACKGROUND: Each year, around 50,000 women die from preeclampsia-eclampsia worldwide. Thus, hypertensive disorders during pregnancy are public health problems in both developed and developing countries.
OBJECTIVE: To identify prognosis factors associated with HELLP syndrome in patients with severe preeclampsia.
MATERIAL AND METHODS: A retrospective, observational, cross-sectional, and analytical study was carried out. It included patients that suffered from severe preeclampsia, with and without HELLP syndrome. They were hospitalized at the Division of Obstetrics Dr. Manuel Gea González Gener…

OBJECTIVE: Our purpose was to find out and compare perinatal outcomes in pregnancies complicated by severe preeclampsia-eclampsia with and without HELLP syndrome.
METHODS: Clinical and laboratory findings, and perinatal-neonatal outcomes of all pregnants with severe preeclampsia, eclampsia and HELLP have been prospectively recorded. Results were compared by means of Student's t test, chi2 analysis and Fisher's exact test as appropriate.
RESULTS: Among 367 consecutive severe preeclampsia, 106 (29%) had HELLP syndrome, 261 (71%) had severe preeclampsia and eclampsia. Mean gestational age and b…

This case report describes a pregnant woman with acute renal failure, caused by aggravation of HELLP syndrome, who developed coma induced by overdose of continuous magnesium sulfate administration for eclampsia. Before examination of serum concentration of magnesium, coma was suspected to be the result of brain vascular problem or brain infarction. Patient was treated in ICU and hemodialysis was started, and she recovered fully from abnormal neurological symptoms. This case illustrate that hypermagnesemia should be considered in the pregnant woman with coma at ER and ICU. Furthermore, the accu…

The aim of this retrospective study was to evaluate the relationship between HELLP (hemolysis, elevated liver enzymes, low platelets) syndro … e aminotransferase, aspartate aminotransferase, bilirubin, uric acid, and proteinuria. Incidence of HELLP syndrome was 24% in the overall study population whereas 48% of the patients with the blood group O Rh-negative had HELLP syndrome associated with an increase in risk by a factor of 3.1. To our knowledge this is the first report of such an association.

A case of severe HELLP sd. Was associated with cerebral oedema, anuria, retinal ablation, ascites and hydrothorax. The mother survived after intensive care and therapy, including 10 times haemodialysis. The acute fatty liver of pregnancy (M. Sheehan II) with a much poorer prognosis should be excluded. A diagnostical corticosteroids and prompt delivery is pointed out.

HELLP syndrome belongs to the group of pathological states known as pregnancy-induced hypertension or EPH gestosis. The basic criteria for establishing the diagnosis are as follows: H for hemolysis, EL for elevated liver enzymes and LP for low platelets. A pregnant woman, 38 years of age, multipara (V pregnancy, third delivery) has been admitted to the Clinic of Gynecology and Obstetrics in Novi Sad in 36-37 week gestation complaining of nausea, vomiting, epigastric pain, general weakness, exhaustion as well as symptom of previously diagnosed preeclampsia. Due to signs of fetal distress, the p…

A newborn boy developed annular erythematous lesions on his entire body. Histopathological examination showed typical features of lupus erythematosus. His mother was positive for anti-Ro/SSa and anti-La/SSb antibodies. Neonatal lupus erythematosus was diagnosed. During pregnancy the mother had suffered from HELLP syndrome. The reported case points out the necessity to differentiate HELLP syndrome from first manifestation of lupus erythematosus during pregnancy. A direct causal relationship between neonatal lupus erythematosus and HELLP syndrome of the mother seems to be unlikely.

The acronym HELLP describes a syndrome of (H) hemolysis, (EL) elevated liver enzymes, and (LP) low platelets, which symptomatically defines a unique group of preeclamptic/eclamptic women. Pregnancies complicated by this syndrome are associated with poor maternal and fetal outcome. This poses a considerable challenge to the anesthesiologists as many of these patients require emergent Cesarean section. We reported a 30-year-old woman with gestation of 32 wk who sustained Cesarean section under the impression of pregnancy with HELLP syndrome. General anesthesia was performed for this patient. The…

We show by two case reports, that the HELLP-syndrome is the most severe form of pre-eclampsia and associated with a relative high maternal and perinatal mortality because of the sudden onset of the complications. In the first case report we describe the development of this syndrome after two eclamptical convulsions on the 6th day of puerperal period. A similar case has not been published yet in all literature. In the second report a mother died by rupture of the capsule of liver because of the HELLP-syndrome 72 hours after Section caesarea. We want to show, that caesarean section cannot avoid

OBJECTIVE: For clinical management and counseling purposes, we determined the neonatal salvage by weeks' gestation and birth weight of pregnancies complicated by HELLP syndrome (hemolysis, elevated liver enzymes, and low platelets).
METHODS: All patients who delivered with a diagnosis of HELLP syndrome between January 1, 1980 and December 31, 1991 at a single tertiary care medical center were evaluated for neonatal outcome and survival. The syndrome was diagnosed in the presence of severe preeclampsia/eclampsia accompanied by laboratory evidence of hemolysis, hepatic dysfunction, and thromboc…
