Twelve infants with heart failure and one with perinatal oedema were treated with intramuscular frusemide (1·0-1·25 mg/kg). Frusemide was found to be a safe effective diuretic in infants with heart failure, resulting in clinical improvement, marked excretion of water, sodium, and chloride, with a lesser and more variable effect on potassium excretion. Complications were uncommon with this dose. The drug is most effective in acute pulmonary oedema or acute worsening of heart failure, but day-to-day control of persistent heart failure is more easily obtained by using repeated doses of oral or in…

In the last decades, outcomes significantly improved for both heart transplantation and LVAD. Heart transplantation remains the gold standard for the treatment of end stage heart failure and will remain for many years to come. The most relevant limitations are the lack of grafts and the effects of long-term immunosuppressive therapy that involve infectious, cancerous and metabolic complications despite advances in immunosuppression management. Mechanical circulatory support has an irreplaceable role in the treatment of end-staged heart failure, as bridge to transplant or as definitive implanta…

IMPORTANCE: The Pooled Cohort Equations to Prevent Heart Failure (PCP-HF) estimate the 10-year risk for symptomatic heart failure (HF) from routine clinical data. The PCP-HF score should detect asymptomatic individuals with cardiac maladaptation preceding HF symptoms for it to be a useful HF prediction tool in primary prevention.
OBJECTIVE: To assess the concordance between PCP-HF risk scoring and the presence of subclinical cardiac maladaptation in the community.
DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional analysis included participants enrolled in the Flemish Study on Environme…

BACKGROUND: Heart failure (HF) is a complex clinical syndrome with diverse etiologies and pathophysiological mechanisms. While chronic HF has gained substantial understanding, acute HF remains less elucidated. This study aimed to analyze the data from the Jordanian Heart Failure Registry (JoHFR) to assess the clinical characteristics, management approaches, and outcomes of patients with HF.
METHODS: The study included data from 21 healthcare centers from private sector clinics and hospitals and public secondary and tertiary university hospitals across Jordan from July 1, 2021, to February 28,…

INTRODUCTION: Heart failure (HF) affects more than 6 million adults in the United States, contributing to substantial morbidity, mortality, and health care costs. Despite advances in medical care, many medications can exacerbate HF, yet their prevalence of use remains unknown. This study examined the national use of prescription medications that could exacerbate HF in adults with self-reported HF.
METHODS: We analyzed data from US adults with self-reported HF in the National Health and Nutrition Examination Survey (NHANES) from 2011 to March 2020. Medications known to exacerbate HF, identifie…

AIMS: Iron deficiency (ID) is prevalent in chronic heart failure (HF) but lacks a consensus definition. This study evaluates the prevalence and the prognostic impact of ID using different criteria on all-cause and cardiovascular mortality, as well as first hospitalization for HF in patients with new-onset chronic HF.
METHODS: In this nationwide registry-based cohort, we explored four definitions of ID: the current European Society of Cardiology (ESC) guidelines [ferritin <100 ng/mL or ferritin 100-299 ng/mL and transferrin saturation (TSAT) <20%], ferritin level <100 ng/mL, TSAT < 20% and ser…

AIMS: Recurrent heart failure hospitalization (HFH) is an important feature of the progression of heart failure (HF). In the Treatment of Preserved Cardiac Function Heart Failure with an Aldosterone Antagonist (TOPCAT) trial, we analysed risk factors for recurrent HFH events in HF patients with preserved ejection fraction (HFpEF) and developed a risk prediction model for recurrent HFH.
METHODS AND RESULTS: This analysis focused on the subset of TOPCAT participants enrolled in the Americas (n = 1767). Recurrent HFH was defined as two or more hospitalizations for HF during the follow-up period.…

BACKGROUND: Heart failure (HF) is associated with high mortality, morbidity, and frequent hospitalizations due to acute HF (AHF) and requires immediate diagnosis and individualized therapy. Some differences between acutely decompensated chronic heart failure (ADCHF) and de novo HF (dnHF) patients in terms of clinical profile, comorbidities, and outcomes have been previously identified, but the hemodynamics related to both of these clinical states are still not well recognized.
PURPOSE: To compare patients hospitalized with ADCHF to those with dnHF, with a special emphasis on hemodynamic profi…

OBJECTIVES: The authors sought to evaluate the association of heart failure hospitalization (HFH) with guideline-directed medical therapy (GDMT) prescribing patterns among patients with heart failure with reduced ejection fraction (HFrEF).
BACKGROUND: HFH represents an important opportunity to titrate GDMT among patients with HFrEF.
METHODS: The CHAMP-HF (Change the Management of Patients With Heart Failure) registry is a prospective registry of adults with HFrEF (ejection fraction ≤40%). Using data from the CHAMP-HF registry (N = 4,365), adjusted time-to-event models were created to study t…

We propose that stage D advanced heart failure be defined as the presence of progressive and/or persistent severe signs and symptoms of heart failure despite optimized medical, surgical, and device therapy. Importantly, the progressive decline should be primarily driven by the heart failure syndrome. Formally defining advanced heart failure and specifying when medical and device therapies have failed is challenging, but signs and symptoms, hemodynamics, exercise testing, biomarkers, and risk prediction models are useful in this process. Identification of patients in stage D is a clinically imp…

INTRODUCTION AND OBJECTIVES: To estimate the percentage of heart failure patients in Spain that received the European Society of Cardiology recommended treatments, and in those that did not, to determine the reasons why.
METHODS: The study included 2834 consecutive ambulatory patients with heart failure from 27 Spanish hospitals. We recorded general information, the treatment indicated, and the reasons why it was not prescribed in some cases. In patients who met the criteria to receive a certain drug, true undertreatment was defined as the percentage of patients who, without justification, di…

Congestive heart failure is a disease state distinguished by the regular presence of both renal and hepatic abnormalities in drug handling. One such abnormality involves flaws in the process of drug absorption. In most instances, congestive heart failure-related abnormalities in drug absorption are of inconsequential significance. However, this is not the case with loop diuretics. Loop diuretic action ordinarily tracks the rate and extent of absorption if a sufficient amount of diuretic has been given to exceed the threshold for diuretic effect. In congestive heart failure, both the rate and a…

Central sleep apnoea, especially Cheyne-Stokes respiration, is found in 45 to 66% of patients with congestive heart failure (CHF) in functional classes NYHA II to IV. Cheyne-Stokes breathing cycles are characterised by central apnoeas, followed by a crescendo--like increase of tidal volume into hyperventilation and a subsequent decline of tidal volume, ending in another central apnoea. Cheyne-Stokes respiration has been shown to be a poor prognostic factor for patients with CHF. Apnoeas and hypopnoeas cause marked oxygen desaturation and rises of carbon dioxide concentrations in the blood. The…

This clinical consensus statement revisits the role of left ventricular ejection fraction (LVEF) as a measurement of cardiac function, a prognostic marker and a major criterion to classify patients with heart failure, and gives new advice for clinical practice. Heart failure is traditionally classified on the basis of LVEF thresholds and this has major implications for treatment recommendations. However, the reproducibility of LVEF measurement is poor and its prognostic and diagnostic value lessens when it is above 45%, with no relationship with the severity of either cardiac dysfunction or ou…

BACKGROUND: Sparce data suggest higher mortality in heart failure (HF) with left ventricular ejection fraction (EF) >65% to 70%. We characterized EF distribution, characteristics, and outcomes in patients with HF and EF ≥50%.
METHODS AND RESULTS: There were 5576 patients enrolled in the Swedish HF registry between 2017 and 2021 and included in the study; 21% had EF ≥60%, 5% EF ≥65%, and 1.5% EF ≥70%. Patient characteristics independently associated with EF ≥60% were assessed by multivariable logistic regression and were identified as being a diagnosis of hypertrophic cardiomyopathy, worse New…
