Transfer of remote ischemic preconditioning plasma from heart transplant patients into isolated perfused rat hearts prior ischemia/reperfusion injury-a translational study of cardioprotection.

Remote ischemic preconditioning (RIPC) has been shown in several experimental studies as an organ protective procedure against ischemic injury, but the implementation of RIPC into routine clinical practice has so far failed due to contradictory study results. However, in order to identify patient groups that could benefit from RIPC, numerous clinical trials have been initiated, but only one study with patients undergoing heart transplantation (HTX). In HTX patients, RIPC appears to be cardioprotective when used immediately before surgery, while it has not been investigated whether the cardiopr

Kidney Injury Molecule 1 (KIM-1) as an Early Predictor for Acute Kidney Injury in Post-Cardiopulmonary Bypass (CPB) in Open Heart Surgery Patients.

INTRODUCTION: Postoperative acute kidney injury is associated with a higher mortality, a more complicated hospital course with longer hospital stay. Urinary kidney injury molecule 1 may play an important role as an early predictor of acute kidney injury post-cardiopulmonary in open heart surgery. METHODS: We evaluated 45 patients who underwent open heart surgery from January 2016 to June 2016. Both urinary kidney injury molecule 1 and serum creatinine were evaluated before operation and 3hs and 24hs after operation. Acute kidney injury was diagnosed according to Kidney Disease: Improving Glob

Associations of the aggregate index of systemic inflammation (AISI) with myocardial injury markers in patients with coronary heart disease.

BACKGROUND: The Aggregate Index of Systemic Inflammation (AISI) is a potential cardiovascular risk indicator. Its association with contemporaneous myocardial injury in coronary heart disease (CHD) patients remains unclear. OBJECTIVE: To explore the association between AISI and elevated myocardial injury markers and evaluate its discriminatory performance. METHODS: In this single-centre retrospective study of consecutive CHD patients, AISI was calculated at admission. The patients were assessed at admission and before coronary angiography. Elevated cTnT was defined as >0.014 ng/mL and elevate

Identification of Risk Factors and Prediction Models for Acute Kidney Injury in Hospitalized Elderly Patients with Heart Failure.

BACKGROUND: Elderly heart failure (HF) patients have an increased risk of acute kidney injury (AKI) during hospitalization. Identifying risk factors and developing a prediction model are clinically important. STUDY METHOD: We retrospectively enrolled 7076 elderly HF patients hospitalized from January 2019 to December 2023. All predictors were restricted to data available within 24 hours of admission and prior to AKI occurrence. Candidate predictors were first screened by univariate analysis (P<0.05) and then further refined by LASSO regression, and the selected variables were ultimately enter

Individual and combined effects of wheel running and ischemic preconditioning in protecting the rat heart from reperfusion injury.

It is well known that exercise training (TRN) and ischemic preconditioning (IPC) reduce cardiomyocyte (CM) death following ischemia and reperfusion (IR), but it is unknown if protection can be elicited by moderate exercise TRN or if the effects of TRN and IPC summate to generate greater protection than either protocol alone. To address these questions, we used wheel running, a moderate-intensity paradigm, to TRN male and female rats, and two IPC protocols. Hearts were studied ex vivo with regional ischemia (RI) produced by occluding the left anterior descending coronary artery for 30 min follo

Birth day of the week is associated with surgical timing and postoperative brain injury in neonates with congenital heart disease.

OBJECTIVES: White matter injury (WMI) is a common neurologic complication in neonates with critical congenital heart disease (CHD) and is associated with adverse neurodevelopmental outcomes. Although intraoperative and perioperative risk factors have been extensively studied, emerging evidence suggests that preoperative factors, including time from birth to surgery, may play a critical role in neurologic injury. We hypothesize that birth day of the week is associated with time to surgery and thus neurologic injury. METHODS: We performed a retrospective analysis of 192 neonates born at term wi

A triad of hypertension, heart failure, and glomerular injury in subacute Kawasaki disease: a case report and literature review.

Kawasaki disease (KD) is the leading cause of acquired pediatric heart disease, characterized by acute systemic vasculitis. We report a 7-year-6-month-old boy with typical KD [meeting the 2017 American Heart Association (AHA) diagnostic criteria] who responded completely to initial intravenous immunoglobulin (IVIG) therapy (2 g/kg), with rapid defervescence and normalized inflammatory markers. Despite this favorable initial response and no coronary abnormalities, he uniquely developed a triad of acute heart failure with cardiomegaly [New York Heart Association (NYHA) class II], severe hyperten

Systolic blood pressure strata and acute kidney injury risk in heart failure: a retrospective cohort study.

BACKGROUND AND OBJECTIVE: In patients with acute heart failure (HF), both hypotension and hypertension may compromise renal perfusion, yet the association between systolic blood pressure (SBP) and acute kidney injury (AKI) risk is controversial and poorly quantified. This study aimed to define this relationship. METHODS: In this retrospective cohort analysis of 19,604 HF patients from the MIMIC-IV database, individuals were categorized into three SBP strata based on their mean SBP during the first 24 hours of intensive care unit admission: strict (<110 mmHg), standard (110-139 mmHg), and leni

The effects of paced breathing on psychological distress vulnerability and heart rate variability in adults sustaining traumatic injury.

BACKGROUND: Traumatic physical injuries can lead to psychological distress and increased risk of psychiatric disorders, often reflected in dysregulated autonomic responses measurable through heart rate variability (HRV). Slow-paced breathing has shown potential in enhancing HRV, but its effectiveness in injured survivors remains unexplored. This study investigates the effect of slow-paced breathing on HRV among injured survivors compared to non-injured individuals and explores the influence of psychological distress and spontaneous respiratory rate on this effect. METHODS: The study involved

Propionic acid/FBP1 is involved in polystyrene nanoplastic-induced cardiac injury via the gut-heart axis.

BACKGROUND: Micro-/nanoplastics (MNPLs) are widely found in the environment and have toxic effects on various organs and systems. However, the role of the gut-cardiac axis in cardiotoxicity induced by MNPLs has not yet been elucidated through research. RESULTS: In this study, we examined the effects of 80 nm polystyrene nanoplastics (PS-NPs) on the heart and human cardiomyocytes (AC16) cells. Histopathological examination showed that NPs caused impaired cardiac function and increased myocardial collagen deposition. In view of the potential influence of gut microbiota and its metabolites on ca

From smog to scarred hearts: unmasking the detrimental impact of air pollution on myocardial ischemia-reperfusion injury.

Air pollution is a global environmental health hazard associated with elevated cardiovascular morbidity and mortality. Emerging evidence suggests that exposure to various air pollutants, specifically particulate matter (PM), ultrafine particulate matter (UFPM), and diesel exhaust particles, may exacerbate myocardial ischemia-reperfusion (I/R) injury. PM exposure can directly impair cardiomyocyte survival under ischemic conditions by inducing inflammation, oxidative stress, apoptosis, and dysregulation of non-coding RNAs. Moreover, exposure to PM, UFPM, and diesel exhaust particles can increase

Protective effects of neutrophil serine protease inhibition against ischemia-reperfusion injury in lung or heart transplantation.

Transplanted organs are inevitably exposed to ischemia-reperfusion (IR) injury, which is known to cause graft dysfunction. Functional and structural changes that follow IR tissue injury are mediated by neutrophils through the production of oxygen-derived free radicals, as well as from degranulation which entails the release of proteases and other pro-inflammatory mediators. Neutrophil serine proteases (NSPs) are believed to be the principal triggers of post-ischemic reperfusion damage. Extended preservation times for the transplanted donor organ correlate with heightened occurrences of vascula

Exploring the mechanism of Qiling Jiaogulan Powder in protecting heart injury of heat-stressed broilers based on transcriptome.

The heart serves as the center of blood circulation, transporting oxygen and nutrients to all tissues and organs of the body. Cardiac function is closely associated with the growth performance, immune function, and meat quality of broilers. In this study, a broiler heat stress (HS) model was established by artificially controlling environmental temperature, and transcriptomic analysis was used to investigate the effect and underlying mechanisms of Qiling Jiaogulan Powder (QLJP) on heat stress-induced heart injury in broilers. The results showed that compared with the HS group, both the HS+QLJP

Environmental Exposures Influence Fetal Brain Growth and Risk of Neonatal Brain Injury in Congenital Heart Disease.

BACKGROUND: Neurodevelopmental impairments are common in congenital heart disease (CHD) and fetal brain volume is an important predictor of outcomes. Social determinants of health (SDOH) and environmental factors influence brain growth in other populations and likely play a neurodevelopmental role in CHD. This study evaluated the influence of SDOH and environmental factors on fetal and neonatal brain volume, growth, and risk of brain injury in CHD. METHODS: This prospective single-center longitudinal cohort study enrolled fetuses with severe CHD to undergo third-trimester fetal and preoperati

Injury-induced Clusterin+ cardiomyocytes suppress inflammation and promote regeneration in neonatal and adult hearts by reprogramming macrophages.

Adult hearts scar after injury, while neonatal hearts regenerate. The mechanisms underlying this dichotomy remain unclear. Through comparative spatiotemporal single-cell analyses and dual recombinase-mediated lineage tracing, we uncovered an injury-induced Clusterin+ cardiomyocyte (Clu+ CM) population that coordinates reparative, anti-inflammatory macrophage activity. Following injury, Clu+ CMs emerge in the border zone of regenerative hearts but are scarce in non-regenerative contexts. These CMs secrete CLU, which binds to macrophage Toll-like receptor 4 (TLR4), attenuating inflammation and p

Heart rate variability in adults with mild traumatic brain injury: a population-based cross-sectional study.

Autonomic nervous system disruptions following mild traumatic brain injury (MTBI) may play a role in elevated mortality risk. However, the relationship between MTBI-related autonomic dysfunction and mental health in the general population remains understudied. This study examined autonomic dysfunction and mental health symptoms among individuals reporting previous MTBI within a community sample. This population-based cross-sectional study was implemented among adults residing in Ulaanbaatar, Mongolia. Heart rate variability (HRV) analysis served as a non-invasive method for evaluating autonomi

Sex-Specific Vulnerabilities to Subclinical Vascular Brain Injury in Early Late-Life: The Framingham Heart Study.

OBJECTIVE: Subclinical vascular brain injury is an increasingly recognized risk factor for stroke and dementia. Despite well-established sex differences in vascular risk and disease prevalence, the impact of sex on drivers of subclinical vascular brain injury remains unclear, presenting a barrier to developing sex-specific prevention guidelines. We aimed to establish the extent to which sex moderates associations between vascular risk factors and magnetic resonance imaging (MRI) measures of subclinical brain injury in stroke-free older adults. METHODS: We leveraged cross-sectional data from 1
