Long-term prognosis of congenital heart defects: a systematic review.

BACKGROUND: In the large and expanding population of adults with congenital heart disease, little is known about their long-term outcome. By means of a systematic literature search, we aimed to assess the quantity and quality of data on long-term survival and morbidity in adults with common congenital heart defects. METHODS: All literature on MEDLINE from January 1980 to January 2007 was searched, using a broad range of keywords for atrial septal defect, ventricular septal defect, pulmonary stenosis, tetralogy of Fallot, aortic coarctation, and transposition of the great arteries. After study

CT imaging features of atrioventricular shunts: what the radiologist must know.

UNLABELLED: In the last decade, cardiac computed tomography (CT) has gained mainstream acceptance for the noninvasive exclusion of significant coronary disease in a selected population. Improvements in electrocardiogram (ECG)-triggered imaging techniques also allow, by extension, a proper evaluation of the complete heart anatomy. Given the increasing worldwide clinical implementation of cardiac CT for coronary artery evaluation, radiologists can, incidentally, be confronted with unfamiliar and previously unsuspected non-coronary cardiac pathologies, including congenital morphological defects.

[Right posterolateral thoracotomy in the treatment of of simple and isolated lesions of the heart].

The right posterolateral thoracotomy approach was used in 13 young girls aged from 1 to 8 years for open heart repair of atrial septal defect (ostium secundum and primum) and ventricular septal defect, without morbidity nor mortality. Neither the thoracotomy scar nor impaired growth of the mammary gland were visible. This approach is considered as safe as median sternotomy for open heart repair of isolated and simple cardiac defects, in prepuberty young females.

Minimally invasive congenital cardiac surgery through right anterior minithoracotomy approach.

BACKGROUND: Median sternotomy has been the conventional approach for correction of congenital cardiac defects despite poor cosmetic results at times. Right anterior minithoracotomy was, therefore, assessed as an alternative procedure with a better cosmetic outcome. METHODS: From October 2002 through February 2007, 75 patients underwent correction of congenital cardiac malformations with the use of cardiopulmonary bypass through right anterior minithoracotomy involving a short incision through the fifth intercostal space and the minimally invasive cannulation. Of them, 18 patients were infants

Heart murmurs in pediatric patients: when do you refer?

Many normal children have heart murmurs, but most children do not have heart disease. An appropriate history and a properly conducted physical examination can identify children at increased risk for significant heart disease. Pathologic causes of systolic murmurs include atrial and ventricular septal defects, pulmonary or aortic outflow tract abnormalities, and patent ductus arteriosus. An atrial septal defect is often confused with a functional murmur, but the conditions can usually be differentiated based on specific physical findings. Characteristics of pathologic murmurs include a sound le

Residual cor triatriatum sinistrum after atrial septal defect repair in an adult.

Cor triatriatum sinistrum is an uncommon congenital heart disease. The surgical repair outcomes are often satisfactory without reintervention. We report a case of residual cor triatriatum sinistrum after atrial septal defect repair in an adult. He was hospitalized to the emergency because of persistent palpitation. In classic cor triatriatum, all pulmonary veins return to proximal chamber (accessory left atrium), but this case we reported with left upper pulmonary vein returned to distal chamber (true left atrium). The transthoracic echocardiography and reconstructed computed tomography image

Partial anomalous pulmonary venous connection to the right side of the heart.

Partial anomalous pulmonary venous connection to the right side of the heart often complicates surgery for atrial septal defects. Between 1964 and 1987, 39 patients, ranging from 2 to 52 years old, underwent repair of partial anomalous pulmonary venous connection. At least one anomalous pulmonary vein arose from the right upper lobe in 38 patients and right middle lobe in 30 patients and connected to the superior vena cava in 28 patients and the right atrium only in 11 patients. An atrial septal defect was present in 32 patients (82%). Patients who had partial anomalous pulmonary venous connec

Evaluating variation in pre-operative evaluation and planning for children undergoing atrial or ventricular septal defect repair.

BACKGROUND: CHD care is resource-intensive. Unwarranted variation in care may increase cost and result in poorer health outcomes. We hypothesise that process variation exists within the pre-operative evaluation and planning process for children undergoing repair of atrial septal defect or ventricular septal defect and that substantial variation occurs in a small number of care points. METHODS: From interviews with staff of an integrated congenital heart centre, an initial process map was constructed. A retrospective chart review of patients with isolated surgical atrial septal defect and vent

Diastolic right-to-left shunting in a patient with atrial septal defect and pericardial tamponade.

We present the case of a patient with an atrial septal defect who presented with hypotension, hypoxia, and cyanosis as a result of pericardial tamponade. The classic findings of pulsus paradoxus and low measured right heart output were not present. Echocardiography demonstrated the atrial septal defect, pericardial effusion, and diastolic chamber collapse. Furthermore, Doppler study showed not only a large left-to-right shunt but also a diastolic right-to-left shunt, which explained the patient's hypoxia.

Cor-Triatriatum Dexter with Associated Cyanosis in a 3-Month-Old Girl.

Cor-triatriatum is uncommon and cor-triatriatum dexter is rarer, and patients may remain asymptomatic in isolated cases especially if the partitioning is mild and nonobstructing. There may be other cardiac defects associated with it which are mostly right-sided cardiac defects such as atrial septal defect, Ebstein anomaly, and pulmonary stenosis. However, left-sided heart anomaly such as aortic regurgitation has been associated with it, but its association with transposition of the great vessels has not been documented before now. Therefore, the case of a 3-month-old girl with cor-triatriatum

Acquired atrial septal defects secondary to rupture of the atrial septum in dogs with degenerative mitral valve disease.

Acquired atrial septal defects (ASDs) secondary to atrial septal rupture are uncommon in dogs with degenerative mitral valve disease. Echocardiographic diagnosis of these defects has not previously been described. The echocardiographic features in two cases identified antemortem are presented in this report. ASDs can be difficult to identify on two-dimensional echocardiogram when relatively small in size and may be underreported. Acquired ASDs may alter the physiology, treatment plan and prognostic implications for the dogs in which they occur and their presence should be considered in all cas

Congenital heart defects in the recurrent 2q13 deletion syndrome.

The recurrent 2q13 deletion syndrome is a rare genetic disorder associated with developmental delay, cardiac and urogenital malformations, and minor facial anomalies. Congenital heart defects (CHDs) are the most frequent malformations associated with del2q13. Experimental studies in zebrafish suggest that two genes mapping within the 2q13 critical region (FBLN7 and TMEM87B) could confer susceptibility to congenital heart defects in affected individuals. We reviewed the cardiac characteristics in four patients with 2q13 deletion admitted to our hospitals, and in published patients. Two of our p

[Comparative study of right anterolateral minithoracotomy and median sternotomy in the repair of atrial septal defects].

OBJECTIVE: To compare the effect of right anterolateral minithoractomy and median sternotomy for atrial septal defect closure. METHODS: In this study, 82 patients, whose age ranged from 4 to 46 years with a mean of 15.7+/-8.6 years, underwent surgical repair of the atrial septal defects on the beating heart through right anterolateral minithoractomy. Ten of these cases were complicated by moderate to severe pulmonary hypertension, and 3 by partial anomalous pulmonary venous connection. Another 67 patients with an age range of 3 to 49 years (mean 17.0+/-12.5 years) underwent surgical repair of

[Congenitally corrected transposition of the great arteries in patient with atrial septal defect and acute inferior segment myocardial infarction].

Congenitally corrected transposition of the great arteries is a rare abnormality accounting for approximately 1% of clinically apparent congenital heart disease. Age at time of diagnosis and survival rate vary and depend on associated anomalies, including pulmonary stenosis, ventricular septal defect, atrial septal defect, atrioventricular block, and atrioventricular valve regurgitation. Reported cases of corrected transposition of the great arteries with single coronary ostium anomaly and atrial septal defect are very rare. Described in the present report is the case of a 55-year-old male who

The structure of the common atrioventricular valve in hearts having isomeric atrial appendages and double inlet ventricle.

BACKGROUND AND AIMS OF THE STUDY: It is now well recognized that regurgitation through a common atrioventricular valve can compromise the clinical course both before and after surgical interventions in patients with visceral heterotaxy. This may reflect the anatomic structure of the valve. This study aimed to determine whether the structure of the common atrioventricular valve found in the setting of hearts with isomeric atrial appendages and double inlet ventricle differs from that of the valve guarding a common junction in hearts with biventricular atrioventricular connections. METHODS: Six
