Eculizumab is a C5 inhibitor approved for the treatment of paroxysmal nocturnal hemoglobinuria (PNH), atypical hemolytic uremic syndrome (aHUS), and anti-acetylcholine receptor antibody-positive generalized myasthenia gravis (AChR + gMG) in Japan. We report integrated safety data from post-marketing surveillance in these three indications, focusing on commonly occurring adverse events (AEs) and infection-related AEs. Of 1219 patients registered, 1055 (PNH: 780; aHUS: 192; AChR + gMG: 83) had available safety data. Total eculizumab exposure was 3977.361 patient-years. AEs were reported in 74.03…

Paroxysmal nocturnal hemoglobinuria is a chronic, multi-systemic, progressive and life-threatening disease characterized by intravascular hemolysis, thrombotic events, serious infections and bone marrow failure. Paroxysmal nocturnal hemoglobinuria results from the expansion of a clone of hematopoietic cells that due to an inactivating mutation of the X-linked gene PIG-A are deficient in glycosylphosphatidylinositol-linked proteins. Early diagnosis, using flow cytometry performed on peripheral blood, the gold standard test to confirm the diagnosis of paroxysmal nocturnal hemoglobinuria, is esse…

A 25-year-old man presented with a history of fever, chills and vomiting for three days. The parasite count was 207 ring-forms of P. falciparum per 1000 red cells. He developed hemoglobinuria and excreted hemoglobin in the urine 0.20-0.30 g/dl for 14 days during admission. Many blood transfusions were administered for correcting anemia. Although the malarial parasites disappeared one week after anti-malarial therapy, however, the fever and hemoglobinuria persisted. The Weil-Felix reaction OXK was positive with a titre of 1:40 on admission and increased to 1:160 on the second week. Chlorampheni…

Aquired immune hemolytic anemias are classified in autoimmune hemolytic anemia (AIHA) of warm type, cold agglutinine disease, paroxysmal cold hemoglobinuria, drug-induced immune hemolytic anemia, and paroxysmal nocturnal hemoglobinuria. The autoantibodies in AIHA of warm type react most strongly at 37 degrees C (warm autoantibodies). They are of the IgG, less commonly of the IgM and IgA classes. The cause of autoimmunization remains obscure in 50% of the patients (idiopathic AIHA). In the remaining cases, the AIHA is associated with other diseases. Corticosteroids are the mainstay of therapy, …

Deep vein thrombosis (DVT) is a common venous thrombotic disorder. Current treatment strategies focus on anticoagulation and pharmacomechanical thrombectomy technologies; however, associated complications must be considered. Acute kidney injury (AKI) secondary to hemoglobinuria requiring renal replacement therapy is a rare yet serious complication of mechanical thrombectomy with the AngioJet® rheolytic thrombectomy system. This report describes the case of a 47-year-old male patient who developed AKI, hemoglobinuria, and metabolic acidosis within hours of the procedure. Despite fluid administr…

BACKGROUND: During major ABO-mismatched bone marrow transplant (BMT), the infusion of incompatible red blood cells (RBCs) that are present in the bone marrow graft can cause adverse events from hemolysis. RBC depletion of the bone marrow graft can decrease this risk, but the optimal method to prevent hemolysis is unclear.
PROCEDURE: We conducted a retrospective cohort study of patients who underwent major ABO-mismatched BMT at a pediatric center and had RBC depletion with either hydroxyethyl starch (HES) sedimentation or Ficoll density gradient separation. Postinfusion hemoglobinuria and crea…

During two cases of lumbar spine surgery with instrumentation, we used intraoperative autologous transfusion (IAT), resulting in hemolysis during collection and hemoglobinuria and coagulation abnormalities after transfusion. Hemolysis during IAT collection can lead to hemoglobinuria and binding of nitric oxide, leading to vasoconstriction. The literature suggests that stroma from damaged cells and contact of the blood with the IAT device can lead to coagulation abnormalities and other morbidities, including adult respiratory distress syndrome.
