To stop nasal bleeding we use a manually operated combined bipolar suction and coagulation instrument under microscopic or endoscopic control, managed by one hand. As a result of the simultaneous suctioning of blood, the bleeding source can be coagulated even during the most effussive bleeding. Since the instrument is a combination of suction tube and bipolar coagulation sound, it can perform three functions at the same time: inspection of the source of bleeding, sucking off the blood, and coagulation. This does away with the need of nasal tamponade which most patients find highly disagreable.

Splenectomy performed in a patient with polycythaemia vera and incipient osteomyelofibrosis was complicated by severe postoperative bleeding. A transient decrease of factor VIII/von Willebrand factor (VIIIR:WF) was detected as a possible cause of the bleeding tendency. As a result of this observation the VIIIR:WF system was investigated in all patients with myeloproliferative disorders who are presently followed by this laboratory. Subnormal VIIIR:WF activity was found in 5 of 9 prospectively investigated patients with polycythaemia vera and in one of 12 patients with chronic myeloid leukemia.

Twenty-three patients with severe hemophilia were given single or multiple doses of diphenhydramine hydrochloride, chlorpromazine hydrochloride, or guaifenesin to test their effects on hemostasis. In contrast with earlier studies with aspirin, bleeding times were not prolonged after ingestion of these three drugs. No patient had hemorrhage because of the drugs, and none recalled excessive bleeding following drug ingestion. Rabbits were exposed to carbon disulphide vapours in the toxicological chamber. Fibrinogen level, antiplasmin activity and soluble fibrin monomer complexes in the blood were increased, fibrinolysis time was prolonged. These disturbances in hemostasis depend on the concentration of carbon disulphide in the inhaled air and on the intoxication time. The authors suggest that these findings depend on the changed lipid metabolism and on the direct antagonistic influence of CS2 on coagulation and fibrynolysis agents.

METHODS: In 32 patients with hypofibrinogenemia < 150 mg/dL during either thoracic or thoracoabdominal aortic surgery, blood coagulation ability was observed using ROTEM Sigma® and the 3 min bleeding amount was measured during surgery.

Viscoelastic testing is an old but evolving technology that is commonly used in the management of trauma patients. Unlike conventional coagulation studies that provide information on specific parts of the clotting process, viscoelastic testing gives a more global view of clotting. The two currently used forms of viscoelastic testing are rotational thromboelastometry (ROTEM) and thromboelastography (TEG). This review will outline the history, technical aspects, and clinical data for ROTEM and TEG in the context of managing traumatic injury.

A 71-year-old gentleman with prior bioprosthetic aortic valve replacement was admitted with aortic valve dehiscence and an aortic root abscess. He underwent reoperative sternotomy, aortic root, mitral valve, and hemiarch replacement.
