Everyday clinical practice frequently leads us to suspect a close relationship between the lumbar spine and the hip-joints. Sagittal balance fundamentally expresses a postural strategy mobilizing the dynamic structure of the lumbar-pelvic-femoral complex in an authentic balance by which obligatory coupled movements transmit stresses in a single structure, the spine, to the two-part structure of the lower limbs, and vice-versa. Flexion contracture is a frequent hip pathology, but congenital dislocation and ankylosis of the hip have the greatest impact on the spine, due to excessive mechanical s…

Hip injuries are common in athletes, and there is an extensive differential diagnosis of potential causes. This article reviews the anatomy of the hip, and discusses the imaging findings of hip pathology in athletes including skeletal, intraarticular, and extra-articular abnormalities. The role of radiography, computed tomography (CT), magnetic resonance (MR) imaging, MR arthrography, CT arthrography, and sonography in evaluating each condition is discussed.

From 1999 to 2008, according to a prospective and sequential compilation of data, the 12,590 hip operations (11,059 in adults and 1,531 in children) performed in our hospital showed a postoperative complication rate of 4.96% (5.2% in adults and 1.96% in children). The most frequent complications were 110 thromboses (0.87%), which were clinically apparent and proven by Doppler ultrasonography or phlebography; 191 superficial and deep disorders of wound healing (1.5%), including 57 (0.45%) infections; 88 peripheral nerve lesions (0.7%); and 53 hematoma revisions (0.42%). Operation-specific stati…

Hip fractures account for <1% of all pediatric fractures. Most are caused by high-energy mechanisms, but pathologic hip fractures also occur, usually from low-energy trauma. Complications occur at a high rate because the vascular and osseous anatomy of the child's proximal femur is vulnerable to injury. Surgical options vary based on the child's age, Delbet classification type, and degree of displacement. Anatomic reduction and surgical stabilization are indicated for most displaced hip fractures. Other options include smooth-wire or screw fixation, often supplemented by spica cast immobilizat…

Coxarthrosis is the most frequently occurring hip disease, is the most common type of osteoarthritis in general and is the second most frequent cause of chronic invalidity. Causes are inherited hip diseases (e.g. dysplasia), growth diseases (e. g. Perthes' disease, slipped capital femoral epiphysis), injuries (fractured femur, fractures of the femoral head or acetabulum, necrosis of the femoral head), and inflammatory, rheumatic or metabolic diseases. Hip deformities such as hip dysplasia or protrusio acetabuli lead to increased mechanical deterioration as a result of the pathological anatomy …

Injuries of the hip and pelvis in pediatric athletes are receiving increased attention. The majority of injuries are soft tissue injuries or apophyseal injuries that heal with nonoperative supportive treatment. Unique injury patterns can be seen in patients who have underlying pediatric hip disorders such as slipped capital femoral epiphysis, and Legg-Perthes disease. With the advent of hip arthroscopy and the development of more advanced imaging of the hip through MR arthrography, internal derangements of the hip such as labral tears, loose bodies, and chondral injuries are being diagnosed an…

Early diagnosis of developmental dysplasia of the hip (DDH) is important to institute appropriate treatment and reduce the incidence of long-term complications. Risk factors for DDH include genetic, developmental, mechanical, and physiologic factors. Physical examination using Ortolani and Barlow maneuvers is standard for early detection. Plain film radiography is limited for the diagnosis of DDH even when applying several classic landmarks, lines, and measurements, due to the lack of visualization of the cartilaginous components of the infant's hip. Ultrasound arose as a tool for evaluation o…

It has been stated in the literature that a bipolar hemiarthroplasty has a lower risk of dislocation compared to a unipolar hemiarthroplasty. As this statement has not been substantiated we undertook a systematic review of the literature of published articles from the last 40 years. In addition we used our own database of hip fractures. One hundred and thirty-three published articles were included in the review to give a total of 23,107 cases. The overall dislocation rate for all types of hemiarthroplasty was 791/23,107 (3.4%). An increased risk of dislocation was associated with a posterior s…

Complications associated with hip arthroscopy occur between 1.6% and 5%. Fortunately, with the greater understanding of the causes and advancements in techniques and equipment, the incidence is declining. Most of the complications were transient neuropraxias and fluid extravasations resulting in no permanent damage. Severe scuffing of two femoral heads and one case of avascular necrosis were considered serious and permanent, thereby resulting in a 0.5% rate in our series for significant complications.

Instability after total hip arthroplasty is a major source of patient morbidity, second only to aseptic loosening. Certain patient groups have been identified as having a greater risk of instability, including patients undergoing revision arthroplasty as early or late treatment for proximal femoral fractures.

BACKGROUND: Our aim was to elucidate the dislocation rate after total hip arthroplasty at Haugesund County Hospital, Norway.
MATERIAL AND METHODS: Information was obtained from the Norwegian Arthroplasty Register and from hospital patient files. All 1,076 patients (781 female) operated at the hospital 1987-2000 were included. The ITH prosthesis was used in 545 patients and the Charnley prosthesis with a smaller head in 531 patients.
RESULTS: 21 patients (2.0%) experienced dislocation after total hip arthroplasty, 17 during activities of daily living and three after a fall. 17 dislocations ha…

Hip pain in athletes involves a wide differential diagnosis. Adolescents and young adults are at particular risk for various apophyseal and epiphyseal injuries due to lack of ossification of these cartilaginous growth plates. Older athletes are more likely to present with tendinitis in these areas because their growth plates have closed. Several bursae in the hip area are prone to inflammation. The trochanteric bursa is the most commonly injured, and the lesion is easily identified by palpation of the area. Iliotibial band syndrome presents with similar lateral hip pain and may be identified b…

When walking, each hip alternately carries the body mass minus the supporting leg. This mass exerts a force K acting on the hip with a lever arm h'; it is counter-balanced by a force M exerted by the abductor muscles, which acts on the hip with a lever arm h. The hip joint transmits the resultant R of forces K and M. Force R evokes compressive stresses in the joint. In a normal hip, force R is exerted at the center of the force transmitting surface of the joint, and the stresses are evenly distributed over this surface. This is reflected by the narrow ribbon of subchondral sclerosis in the roo…

Reports in the literature suggest that there is an association between two childhood disorders: torticollis, an easily recognized clinical deformity, and developmental dislocation or dysplasia of the hip, an occult disorder. The identification of the obvious disease, torticollis, may prompt a search for the occult disease, developmental dislocation of the hip. If the association of these two disorders is common, it may be justified to expend resources to diagnose the occult disorder in all cases in which the more obviously noticed disorder is recognized. The reported association varies between…

INTRODUCTION: Hip growth continues on till adolescence with the fusion of the different ossification centers. Does this growth exist in hip dysplasia? What is the role of surgery at adolescence? Is an additional varus osteotomy indicated with a pelvic osteotomy?
MATERIAL AND METHODS: Clinical and radiological criteria of adolescent hip dysplasia were studied in a series of 18 patients (mean age 12 years). The 28 hips were divided into 4 groups depending on the treatment: non operated hips (group 1, N = 8), hips operated on only by pelvic osteotomy (group 2, N = 8), hips operated on by combine…
