Immunotherapy for invasive Candida infections: Translating host-directed strategies into clinical practice.

BACKGROUND: Candida infections are a leading cause of invasive fungal disease (IFD), especially among critically ill and immunocompromised patients. Despite available pathogen-directed antifungal therapies, outcomes remain unsatisfactory with high associated morbidity and mortality rates. Increasing recognition of immune dysregulation of the host, including immunoparalysis and impaired antifungal effector responses, has driven interest in immunotherapeutic strategies as adjuncts to conventional treatment. OBJECTIVES: To provide a comprehensive overview of emerging immunotherapeutic approaches

Phenothiazines enhance antibacterial activity of macrophage by inducing ROS and autophagy.

Bacterial infections pose a major global public health threat. While antibiotics have historically served as the primary treatment, the rapid rise of antibiotic resistance has led to an urgent need for new therapeutic strategies. Host-directed therapies (HDTs), which activate defense mechanisms of host cells, are emerging as a promising alternative. Host-acting compounds (HACs) have no direct effect on bacteria and therefore do not induce drug resistance or alter intestinal microbiota composition. In the present study, we demonstrated that phenothiazines significantly enhance the antibacterial

Mycophenolate mofetil impairs transduction of single-stranded adeno-associated viral vectors.

Adeno-associated virus (AAV) liver-directed gene therapy seems a feasible treatment for Crigler-Najjar syndrome type I, an inherited liver disorder characterized by severe unconjugated hyperbilirubinemia. Transient immunosuppression coupled with vector administration seems needed to overcome host immune responses that prevent long-term expression in patients. The immunosuppressive mycophenolate mofetil (MMF), which inhibits de novo synthesis of purines, is a promising candidate. To investigate the potential use of MMF in patients with Crigler-Najjar syndrome, we studied its effect on single-st

Host genetic variants in the pathogenesis of hepatitis C.

Direct-acting antiviral drugs (DAAs) are currently replacing antiviral therapy for Hepatitis C infection. Treatment related side effects are even worse and the emergence of resistant viruses must be avoided because of the direct-antiviral action. Altogether it remains a challenge to take treatment decisions in a clinical setting with cost restrictions. Genetic host factors are hereby essential to implement an individualized treatment concept. In recent years results on different genetic variants have been published with a strong association with therapy response, fibrosis and treatment-related

cJun-assisted loading of RelA DNA-binding promotes synergistic lung epithelial antimicrobial responses.

Lower respiratory tract infections remain a leading cause of death worldwide, highlighting the need for host-directed therapies. We previously identified an inhaled immunostimulatory combination of Pam2CSK4 (Pam2) and CpG oligodeoxynucleotide ODN M362 (ODN) that synergistically protects against bacterial, viral, and fungal pneumonias by reprogramming lung epithelial defenses. Using RNA-seq, dual transcription factor ChIP-seq, and reverse phase protein array, we define the transcriptional basis of this synergy. Cooperative regulation by the AP-1 component cJun and the NF-κB subunit RelA is cent

Sigma-1 Receptor-Targeting Compounds Show Antiviral Activity Across Multiple Coronavirus Models.

Over the past two decades, coronaviruses have caused three major public health crises, including the epidemics of SARS-CoV, MERS-CoV, and SARS-CoV-2. Following the outbreak of SARS-CoV-2, several antiviral drugs have been approved for the treatment of COVID-19. However, the high mutation rate of SARS-CoV-2 has led to the emergence of multiple variants, some of which can escape vaccine-induced immunity and develop resistance to virus-targeted drugs. Viruses rely on host cell mechanisms to complete replication and transmission. Host-directed therapy is an emerging strategy that inhibits infectio…

Disease-Causing Mechanisms and Therapeutic Targets in Infectious Diseases: Implications for Clinical Management and Public Health.

Infectious diseases remain a major cause of mortality and disability worldwide. This burden is driven, in part, by antimicrobial resistance (AMR) and the re-emergence of epidemic and pandemic threats, underscoring the need for translational research to address knowledge gaps exposed by recent pandemics. Despite significant advances enabled by antibiotics and antivirals, their effectiveness is increasingly constrained by resistance development, limited pathogen spectra, and prolonged development timelines that fail to keep pace with rapidly shifting epidemiology. Diagnostic limitations impede t

Pharmacogenetics of hepatitis C.

Recent discoveries have highlighted the influence of host genomics on hepatitis C virus (HCV) infection outcomes. As a result, our views on hepatitis C pathogenesis and therapeutic approaches have been transformed. The recognition of the impact of single-nucleotide polymorphisms (SNPs) of the genes interleukin 28B (IL28B), inosine triphosphatase (ITPA) and low-density lipoprotein cholesterol receptor (LDLR) may lead to refinements in the pharmacogenomic prediction of antiviral response and drug-related toxicities and favour the discovery of new therapeutic targets for hepatitis C. Although the

Phase I study of MVE-2 therapy in human cancer.

MVE-2, a polymer of maleic anhydride and divinyl-ether (molecular weight, 15,500), was given to 57 patients in a phase I study. The agent was selected for study because it was a potent macrophage activator, interferon inducer, and immunotherapeutic agent in animal tumor models. The drug was administered by iv infusion over a 1-hour period using three schedules of administration: (a) weekly at doses of 25-650 mg/m2, (b) every other week at doses of 500-1200 mg/m2, and (c) every 3 weeks at doses of 1200-1500 mg/m2. No cardiac, pulmonary, hematologic, or hepatic toxicity was observed. There were

High-Throughput Screening for Novel Inhibitors of Intracellular Pathogens, Including Chlamydia trachomatis.

High-throughput drug screening (HTS) is a powerful tool that can be used rapidly to identify new potential bacterial inhibitors and/or compounds which enhance host cell control of pathogens, which can then go on to be developed as novel therapeutics. Typically screening is commonly done in artificial culture medium; however, obligate intracellular pathogens, such as Chlamydia trachomatis, cannot be tested this way. Intracellular screening methods allow for such pathogens to undergo HTS, while still giving reliable and consistent data. Plus, as well as identifying new potential bacterial inhibi

Prospects for leveraging knowledge on ISG and NF-kB effector functions into anti-HBV therapies.

Chronic hepatitis B virus (HBV) infection affects nearly 300 million people worldwide and remains a leading cause of liver cirrhosis and hepatocellular carcinoma. Although current therapies, including interferon-α and nucleos(t)ide analogs, can suppress viral replication, they rarely achieve a functional or sterilizing cure, largely due to the persistence of covalently closed circular DNA (cccDNA) and profound immune dysfunction. Accumulating evidence indicates that noncytopathic immune mechanisms, particularly those mediated by interferons (IFNs) and inflammatory cytokines, are important to v

Mechanistic and molecular insights into emerging anti-virulence therapeutics.

Antimicrobial resistance (AMR) poses a global health crisis and necessitates novel therapeutic strategies beyond traditional antibiotic, driving interest in anti-virulence (AV) strategies that disable pathogenicity rather than bacterial viability. This review provides an integrated overview of emerging AV approaches targeting quorum sensing, type III secretion systems, biofilm development, adhesion, toxin activity, iron acquisition, and host-pathogen interactions. We highlight representative phytochemicals, repurposed drugs, engineered inhibitors, nanomaterial-enabled formulations, and antibio

Anti-microbial Antibodies, Host Immunity, and Autoimmune Disease.

Autoimmune diseases are a spectrum of clinical inflammatory syndromes with circulating autoantibodies. Autoimmune diseases affect millions of patients worldwide with enormous costs to patients and society. The diagnosis of autoimmune diseases relies on the presence of autoantibodies and the treatment strategy is to suppress the immune system using specific or non-specific immunosuppressive agents. The discovery of anti-microbial antibodies in the blood of patients with Crohn's disease and Sjogren's syndrome and cross-reactivity of anti-microbial antibodies to human tissue suggests a new molecu

Transferrin Enhances Antifungal Therapy and Improves Survival in Experimental Mucormycosis.

Mucormycosis is a highly lethal invasive fungal infection caused by fungi of the order Mucorales, with mortality rates exceeding 50% despite current antifungal therapies with polyenes or azoles. Because iron acquisition is essential for Mucorales growth and virulence, we investigated whether transferrin, the primary physiological iron-sequestering protein in plasma, could serve as a host-directed therapeutic strategy against mucormycosis. Transferrin inhibited the growth of clinically relevant Mucorales species, including Rhizopus delemar (R. delemar), Rhizomucor, and Lichtheimia corymbifera (

Sexual dimorphism of COVID-19 inspires drug repositioning and host-targeting immunotherapy for viral pneumonia.

Sexual hormones play an important role in modulating disease outcome of COVID-19. The interplay between viral replication, host immune responses, pathology process and sexual hormone levels are complicated, and the underlying mechanisms remain exclusive. Here, we reveal the dose-dependent manner and multi-faceted role of the male hormone testosterone in hamster model of COVID-19 by evaluations of manifestations including survival rate, body weight loss, viral load, immune responses and lung injury. Both low and high doses of testosterone treatment cause more severe illness in male hamsters. Lo

Targeted therapy and immunotherapy for orbital and periorbital tumors: a major review.

Traditionally, for patients who are poor candidates for surgery and/or radiotherapy, palliative chemotherapy is often offered but with significant toxic side effects. However, recent advancements in our understanding of tumor biology and molecular genetics have brought new understanding to the molecular pathways of certain tumors and cancers. This has ushered in a new era of precision medicine specific to a tumor or cancer treatment pathway (targeted therapy) or directed to host-tumor responses (immunotherapy). This article will focus on recent updates in the application of available targeted
