Osteopetrosis is a rare disease characterised by generalised sclerosis of the bone. Surgical treatment for fractures in osteopetrotic bones is difficult due to their hardness. We report successful surgical treatment of humeral and clavicular fractures in a 30-year-old osteopetrotic patient with severe multiple trauma. Two years after surgery, the patient had a full range of movement at the shoulder and elbow, with good bone union and alignment.

We reported a 24-year-old man with right radial nerve palsy. He had suffered from a right humeral shaft fracture approximately three years before. The fracture was treated with intramedullary nailing. Our radiograph of the right upper limb showed callus around the fracture site. A nerve conduction study revealed conduction block of the radial nerve at the lateral side of the callus. Tinel's sign was present in the median nerve at the medial side of the callus. Surgical exploration revealed that the radial nerve trunk was compressed at the callus site. The median nerve trunk was close to the ca

Twenty children with fracture-separation of the entire distal humeral epiphysis have been reported. The key to the radiographic diagnosis is the relationship between the radius and capitellum. A line extending from the shaft of the radius will always pass through the capitellum regardless of the position in which the radiograph is obtained. Posteromedial displacement of the humeral fracture fragment together with the ulna and the radius most commonly occurs. The injury must be distinguished from dislocation of the elbow, fracture of the lateral condyle, and supracondylar fracture of the distal

BACKGROUND: Osteonecrosis of the humeral head (ONHH) is a severe complication after the internal fixation of proximal humeral fractures (IFPHF). The risk factors remain controversial though many studies have reported. In this research, meta-analysis was used to evaluate which surgeon-level factors can be modified to lower the risk and we hope to provide evidence-based support for preventing ONHH.
METHODS: Literature was retrieved from PubMed, Cochrane Library, Embase, Web of Science, and Scopus for eligible studies published up to January 2023. The pooled odds ratios (ORs) were calculated wit

INTRODUCTION: Osteoporosis can affect the surgical outcomes of proximal humeral fractures in older people. Recently, the cortical bone thickness of the proximal humerus on plain radiograph has been proposed to reflect local osteoporosis of the proximal humerus; however, its effect on the surgical outcome of proximal humeral fractures remains unclear. The purpose of this study is to investigate the influence of cortical bone thickness on postoperative radiographic outcomes after osteosynthesis for proximal humeral fractures.
MATERIALS AND METHODS: We retrospectively identified 190 patients (≥5

PURPOSE: The aim of our study was to evaluate the effect of the S3 (spatial subchondral support) humeral plate and to compare the clinical outcomes and complications after ORIF (open reduction and internal fixation) of proximal humeral fractures using either a S3 plate or a PHLP (proximal humeral locking plate).
METHODS: A total of 144 patients with displaced unstable proximal humeral fractures were treated with either a S3 plate or a PHLP. Each patient had a follow-up at least for one year. We retrospectively collected the data and compared the shoulder functional outcome as well as complica

BACKGROUND: The aim of this study was to describe for the first time the medium to long-term outcome after distal humeral hemiarthroplasty (DHH).
METHODS: Twenty-six patients (mean age, 62; range, 29-92 years) treated with DHH for intra-articular distal humeral fractures and its sequelae were studied retrospectively.
RESULTS: Four patients had died and 4 had been revised to total elbow arthroplasty: 2 for periprosthetic fractures and 2 for primary component loosening (all in prostheses without an anterior flange). Six other complications had occurred: ulnar neuritis, 4; stiffness, 1; and wou

BACKGROUND: Severe proximal humerus comminuted fractures are often accompanied by medial calcar comminuted fractures and loss of medial support, which are important factors that lead to internal fixation failure. The appropriate treatment for proximal humerus comminuted fractures has not been established. Therefore, this study assessed the outcomes of using a fibular autograft with locking plates to treat severe proximal humerus comminuted fractures.
AIM: To investigate the outcomes of using a fibular autograft with locking plates to treat severe proximal humerus comminuted fractures.

BACKGROUND: Proximal fracture of the humeral head is the third most frequent fracture in humans. Most (70%) of those affected are over 60 years old. It is hoped that advanced locking medullary screws or plates will reduce the risk of secondary dislocation of screws or fracture segments when the bone of the humeral head is osteoporotic.
METHODS: From January 2002 to August 2005, 225 displaced humeral head fractures in 223 patients aged on average 66+/-15 years were treated with a new locking proximal humeral plate.
RESULTS: In 176 patients in whom follow-up was possible, the average Constant

BACKGROUND: Humeral shaft fractures constitute around 1% of adult fractures in the UK, with an annual incidence of approximately 13 per 100,000 population. Historically, these injuries have been primarily managed non-operatively, with operative fixation reserved for specific indications. Although some recent retrospective studies have suggested there are potential benefits of operative fixation over humeral bracing, there is a deficiency in level 1 evidence to support operative management as the primary treatment for humeral shaft fractures.
METHODS/DESIGN: This single-centre prospective rand

Every year over 330,000 people are hospitalized for hip fractures in the US. Of those patients, approximately 4-5% of patients with hip fractures present with a concomitant upper extremity fracture. Upper extremity (UE) fractures account for an estimated 2 million fractures a year. The number of hip fractures is projected to double by 2050. There is evidence that the presence of a concomitant upper extremity fracture results in an increased hospital length of stay (LOS), lower functional capacity upon discharge and higher mortality rates than seen with hip fractures in isolation. Additionally,

BACKGROUND: There are no clear criteria for decision-making after proximal humeral fractures, and the choice of surgical strategy and, in particular, timing is controversial. Vascular disruption can lead to delayed avascular necrosis of the humeral head, which may result in failure of the fixation procedure and the need for revision surgery. MRI pulse sequences with zero echo time (ZTE) provide "CT-like" images of the bone, while dynamic contrast-enhanced MRI (DCE MRI) allows for visualization and quantification of vascular permeability, blood flow, and clearance rates within the bone. We hypo

HYPOTHESIS AND BACKGROUND: Preoperative computed tomography (CT) scans can be used to measure the thickness of the center of the humeral head to identify patients at a higher risk of screw cutout after open reduction-internal fixation.
METHODS: At an academic medical center, we performed a retrospective review of all patients aged ≥ 18 years who had sustained a proximal humeral fracture that was treated with open reduction-internal fixation between January 1, 2005, and December 31, 2014, and who underwent preoperative shoulder CT. Ninety-four patients were included. Patient charts were review

BACKGROUD: Peripheral nerve injuries are a common complication of pediatric supracondylar humeral fractures. While most resolve spontaneously, the timeline for recovery remains unpredictable. This study investigated whether earlier correction of fracture alignment-regardless of timing of definitive surgical fixation-shortens the duration of neurologic recovery.
METHODS: We retrospectively reviewed pediatric patients (< 18 years) treated surgically for supracondylar humeral fractures between October 2008 and February 2025. Patients presenting with preoperative neurologic deficits were included

BACKGROUND: Most fractures in children are fractures of the upper extremity. Proximal and diaphyseal humeral fractures account for a minority of these fractures. To our knowledge, few previous reports address these fractures. This study aimed to describe the epidemiology and current treatment of proximal and diaphyseal humeral fractures by using the Swedish Fracture Register (SFR).
METHODS: In this nationwide observational study from the SFR we analysed data on patient characteristics, injury mechanism, fracture classification and treatment. We included patients aged < 16 years at time of inj

BACKGROUND: The exact incidence of postoperative periprosthetic humeral fractures (PPHF) months or years after fracture-related implantation of a hemiprosthesis is unknown. The currently available literature is predominantly concerned with operative treatment approaches. As a rule, these involved older patients and severe fracture conditions so that severe complications and unsatisfactory healing results were described. This article presents an alternative conservative treatment approach which is discussed based on the results of treatment.
PATIENTS AND METHODS: Between 2011 and 2016 a conser

We retrospectively reviewed supracondylar distal humeral fractures that had been treated with fracture-specific plating and definitive fixation through a lateral approach with a medial triceps mobilization technique. We determined the incidences of preoperative and postoperative radial nerve palsies (RNP) to evaluate the impact of the plating and fixation technique on RNP. Seventy-three patients treated at our institution from 2006 through 2009 were included in the study. The patients were assigned to 2 groups: 57 to the blunt injury group (motor vehicle collisions, falls, struck pedestrians,
