[Fixed and functional decentering of the head of the humerus in patients with omarthrosis].

AIM: To determine whether in patients with specific types of osteoarthritis of the shoulder not only a fixed but also a functional decentering of the humeral head exist. METHOD: The shoulder joints of 10 healthy volunteers and of 16 patients with osteoarthritis of the shoulder were examined in various arm-positions, using an open MR scanner. After segmentation, 3D reconstruction of the scapula and humerus were performed and the position of the midpoint of the humeral head calculated relative to the center of mass of the glenoid cavity. RESULTS: At 30 degrees of abduction, 4 of 16 patients de…

Bilateral Glenohumeral-Acromioclavicular Composite Articulations Identified During Routine Cadaveric Dissection: A Case Report.

The acromion and lateral clavicle normally lie outside the glenohumeral joint capsule. During routine dissection of an 80-year-old female donor, bilateral glenohumeral-acromioclavicular composite articulations were identified. In each shoulder, the surface opposing the humeral head extended superiorly to include a variant acromion and lateral clavicle; the humeral heads appeared morphologically typical. In the more extensively dissected right shoulder, the lateral end of the clavicle was broad, flattened, and polished where it opposed the humeral head, with a gross appearance consistent with c…

Glenohumeral relationship in maximum elevation.

PURPOSE: The purpose of this study was to clarify rotational relationships between the anatomical landmarks of the glenohumeral joint in max … parallel to the equator (within 2°). Each glenoid center located on antero-superior portion of the humeral head, and the direction from the top of the head to its location was the same as that of the shaft tilting, indicating the glenoid only translated without rotation after reaching the top of the head on the equator. CONCLUSIONS: Before reaching maximum e …

The reverse shoulder prosthesis for primary and secondary treatment of proximal humeral fractures: a case report.

Humeral head fractures are very common among elderly people, often requiring shoulder arthroplasty. One requirement for good function after shoulder arthroplasty is an intact or at least reparable rotator cuff. In patients with multifractured and or osteoporotic tuberosities, refixation leads to the potential risk of redislocation and resorption of the tuberosities or coexisting irreparable rotator cuff tears lead to a high failure rate after implantation of traditional fracture prosthesis, whereas the reverse shoulder arthroplasty may provide better outcome. Here we report of a 79-year-old wo…

Shoulder arthroplasty and its effect on strain in the subscapularis muscle.

BACKGROUND: Increasing the thickness of the prosthetic humeral head on subscapularis strain in patients undergoing total shoulder arthroplasty has not been elucidated. The optimal postoperative rehabilitation for total shoulder arthroplasty that does not place excessive strain on the subscapularis is not known. We hypothesize that the use of expanded non-anatomic humeral heads during shoulder replacement will cause increased tension in the repaired subscapularis. We identified a recommended passive range of motion program without invoking an increase in tension in the repaired subscapularis, a…

Chondral Damage After Arthroscopic Repair Techniques for Acute Bony Bankart Lesions: A Biomechanical Study.

BACKGROUND: Bony Bankart lesions can be encountered during treatment of shoulder instability. Current arthroscopic bony Bankart repair techniques involve intra-articular suture placement, but the effect of these repair techniques on the integrity of the humeral head articular surface warrants further investigation. PURPOSE: To quantify the degree of humeral head articular cartilage damage secondary to current arthroscopic bony Bankart repair techniques in a cadaveric model. STUDY DESIGN: Controlled laboratory study. METHODS: Testing was performed in 13 matched pairs of cadaveric glenoids wi…

[Hemi-arthroplasty--primary or secondary measure for 3- and 4-fragment fractures of the proximal humerus in the elderly?].

Successful treatment of three- and four-part proximal humerus fractures is a therapeutic challenge to the surgeon, particularly in the case of elderly patients. Open reduction and internal fixation have been advocated, but have not consistently produced acceptable results. The results of humeral head replacement as a salvage procedure after non-union or failed open reduction and internal fixation are less predictable. The outcome of hemiarthroplasty (Neer II) performed for three- and four-part proximal humerus fractures in elderly patients was studied. The average patient follow-up was 42.9 mo…

MR arthrographic examination of the association with glenohumeral instability of posterior labroligamentous i̇nsertion types and posterior labrum anatomical variants.

OBJECTIVES: The objective of this study is to evaluate the relationship between posterior labroligamentous morphological variants and abnormal position of the humeral head in the glenoid cavity using MR arthrography (MRA). Additionally, the study seeks to investigate whether these variations act as biomechanical risk factors for subclinical instability. METHODS: The study's sample population comprised 190 patients who did not exhibit posterior labral tears or clinical instability. The posterior labrum morphology and capsulolabral insertion types were assessed using MRA, and the posterior subl…

Scapular orientation influences glenohumeral translation after anterior and posterior labral tears: a cadaveric study.

BACKGROUND: Scapular dyskinesis is frequently observed with various types of shoulder instability, but whether scapular dyskinesis could contribute to shoulder instability is still unclear. The purpose of this study was to determine the effects of scapular orientation on anterior and posterior glenohumeral translation using a cadaveric model of anterior and posterior labral tears. METHODS: Twenty fresh-frozen cadaveric shoulders were divided into 2 groups: the anterior lesion (n = 10) and posterior lesion (n = 10) groups. The humeral head was translated anteriorly or posteriorly with a consta…

Influence of chondrodystrophy and brachycephaly on geometry of the humerus in dogs.

OBJECTIVE: To assess the geometry of canine humeri as seen on radiographs in chondrodystrophic dogs (CD) and brachycephalic dogs (BD) compared to non-chondrodystrophic dogs (NCD). METHODS: Mediolateral (ML) and craniocaudal (CC) radiographs of skeletally mature humeri were used (CD [n = 5], BD [n = 9], NCD [n = 48]) to evaluate general dimensions (length, width, canal flare, cortical thickness), curvature (shaft, humeral head, and glenoid), and angulation (humeral head and condyle). Measurements from CD, BD, and NCD were compared. RESULTS: Mean humeral length was shorter in CD (108 mm) compa…

Joint Preserving Treatment of Chronic Locked Posterior Shoulder Dislocation by Means of Combined Humeral Allograft Reconstruction and Posterior Glenoid Autograft Augmentation.

Chronic locked posterior shoulder dislocations are challenging to treat and often warrant total shoulder arthroplasty. While joint preserving treatment is preferable in young patients, surgical techniques to treat this pathology have rarely been described in the literature. This technical note presents the treatment of a 30-year-old male patient with a chronic locked posterior shoulder dislocation by means of combined humeral allograft reconstruction and posterior glenoid autograft augmentation. Restoration of the spheric humeral head surface was obtained using a fresh-frozen femoral allograft…

Immunologic Contributions Following Rotator Cuff Injury and Development of Cuff Tear Arthropathy.

»: Rotator cuff tear arthropathy (RCTA) describes a pattern of glenohumeral degenerative changes following chronic rotator cuff tears that is characterized by superior humeral head migration, erosion of the greater tuberosity of the humeral head, contouring of the coracoacromial arch to create a socket for the humeral head, and eventual glenohumeral arthritis. »: Acute and chronic inflammatory changes following rotator cuff tears are thought to contribute to cartilage damage, muscle fibrosis, and fatty infiltration in the glenohumeral joint. »: In vitro animal studies targeting various infla…

Proximal humeral osteotomy to correct the anatomy in patients with recurrent shoulder dislocations.

Surgical treatment of patients with traumatic shoulder dislocations is usually successful, but soft-tissue surgery in patients with nontraumatic shoulder dislocation often fails to stabilize the joint. Previous studies have shown that decreased humeral head retroversion might be one cause of anterior shoulder instability. Eleven patients with anterior recurrent dislocations, five traumatic and six nontraumatic, and all with a small humeral head retroversion angle, have been operated on using a proximal humerus osteotomy to correct the abnormal anatomy. After surgery, the humeral head retrovers…

[Diagnosis and therapy of traumatic posterior shoulder dislocations].

In cases of dislocation of the shoulder the humeral head almost always dislocates anteriorly. Only rarely does the dislocation result in a posterior position of all or part of the humeral head. The diagnosis of posterior dislocation is not always as easy as the diagnosis of anterior dislocation and for this reason the injury is often missed. Cisternino et al. found that over 50% of patients with posterior dislocation of the shoulder were missed on initial examination. The longer the time between accident and treatment, the more difficult that treatment becomes and the harder it is to achieve a…

Arthroscopically assisted core decompression of the proximal humerus for avascular necrosis.

Abstract Core decompression has been described as an effective treatment for early stage avascular necrosis of the humeral head. This article describes the technique for arthroscopically assisted core decompression of the humeral head using a transtibial anterior cruciate ligament guide. This technique provides an advantage over the open procedure by avoiding soft-tissue stripping or damaging the biceps tendon or ascending branch of the anterior humeral circumflex artery, as well as providing accurate and safe placement of the core biopsy tract and the ability to address other intra-articular …

Stemless arthroplasty for avascular necrosis of the humerus: implant survival and outcomes.

BACKGROUND: Avascular necrosis (AVN) of the humeral head often affects younger patients and may lead to progressive pain and functional deficits. While stemmed hemiarthroplasty and total shoulder arthroplasty (TSA) are established treatments for advanced-stage AVN, stemless arthroplasty offers potential advantages including bone preservation and facilitation of future revision. However, the impact of bone necrosis on stemless humerus fixation is yet to be elucidated. Therefore, the purpose of this study was to evaluate the outcomes of stemless hemiarthroplasty and TSA in treating humeral head …

Results of transosseous suture fixation in four-part valgus impacted fractures of the proximal humerus.

BACKGROUND: The osteonecrosis ratio in valgus impacted fractures of the proximal humerus is low (<10 %), giving osteosynthesis all its meaning. However, the optimal fixation technique remains controversial. After recentering the humeral head, osteosuture is stable enough to allow bone healing in an adequate position as well as a good long-term shoulder functionality. METHODS: Our cohort included 22 patients with a mean age of 65 (28-83). Patients were placed in a beach-chair position. Surgical exposure was done through an anterolateral, transdeltoid approach. Stay sutures were placed on both …

[Static posterior humeral subluxation without osteoarthritis (posterior shoulder instability type C1) : Where does it come from and is there a solution?].

BACKGROUND: Type C1 posterior shoulder instability represents a pre-arthritic condition characterized by chronic, static posterior subluxation of the humeral head. Its multifactorial etiology includes increased glenoid retroversion, altered posterior acromial morphology, reduced humeral retrotorsion, abnormal scapulothoracic alignment, and muscular imbalances. DIAGNOSTICS: Clinically, the condition predominantly manifests as atraumatic shoulder pain, typically without overt instability symptoms. Diagnosis is based on a comprehensive clinical history, targeted physical examination, and the use…

Failed arthroscopic repair of a large reverse Hill-Sachs lesion using bone allograft and cannulated screws: a case report.

Anterior impression fractures of the humeral head (reverse Hill-Sachs lesions) are typical concomitant bony injuries of posterior shoulder dislocations. When more than 20% of the humeral articulating surface is affected, surgical treatment is required, typically necessitating open surgery. Recently, cases of successful arthroscopic treatment of small reverse Hill-Sachs lesions involving less than 30% of the articulating surface have been reported. This article presents a case of a large reverse Hill-Sachs lesion affecting over 40% of the articulating surface that was treated arthroscopically b…

Luxatio erecta humeri. A case report.

Luxatio erecta humeri is a rare type of glenohumeral dislocation. The pathomechanics of this injury involve either direct axial loading on a fully abducted extremity or leverage of the humeral head across the acromion by a hyperabduction force. The clinical presentation of this type of shoulder dislocation is unique, with the affected extremity held rigidly above the head in abduction. Reduction is accomplished by a form of traction-countertraction under intravenous sedation and analgesia. A variety of neurologic and vascular injuries may be associated with luxatio erecta humeri, involving the…
