Adult-onset Still's disease: an Italian multicentre retrospective observational study of manifestations and treatments in 245 patients.

Adult-onset Still's disease (AOSD) is a systemic inflammatory condition of unknown aetiology characterized by typical episodes of spiking fever, evanescent rash, arthralgia, leukocytosis and hyperferritinemia. Given the lack of data in Italian series, we promote a multicentric data collection to characterize the clinical phenotype of Italian patients with AOSD. Data from 245 subjects diagnosed with AOSD were collected by 15 centres between March and May 2013. The diagnosis was made following Yamaguchi's criteria. Data regarding clinical manifestations, laboratory features, disease course and t…

Long-term follow-up of 124 patients with polymyositis and dermatomyositis: Statistical analysis of prognostic factors.

OBJECTIVES: The aims of this study were to clarify the long-term outcome of patients with polymyositis and dermatomyositis (PM/DM) and to el … al rates were significantly lower in patients with higher age of onset, with malignancies, and with hyperferritinemia in univariate analysis; however, multivariate analysis identified age of onset and serum ferritin as the most significant prognostic factors. CONCLUSIONS: Our study indicates that when age of onset and serum ferritin are used in combination, we can …

Macrophage activating syndrome is associated with lobular hepatitis and severe bile duct injury with cholestasis.

Macrophage activating syndrome (MAS) is a rare hematological disorder associated with uncontrolled systemic T-cell activation. Persistent fever, fatigue and hepatosplenomegaly are frequent clinical manifestations, whereas hyperferritinemia, elevated serum lactate dehydrogenase levels and cytopenia are key criteria for the diagnosis of MAS. The nature of liver pathology in MAS has been partially elucidated but destructive biliary lesions have been rarely described. This report illustrates four cases of MAS developing marked cholestasis, leading to one case of biliary cirrhosis necessitating liv…

Multiorgan Toxicity Following Rifapentine and Isoniazid Therapy for Latent Tuberculosis Infection: A Case Report.

The once-weekly combination of rifapentine and isoniazid (3HP regimen) is a preferred treatment for latent tuberculosis infection (LTBI). Although generally well tolerated, rare but severe systemic adverse reactions may occur. We report the case of a 57-year-old woman receiving once-weekly rifapentine, isoniazid, and pyridoxine for LTBI who developed recurrent chest pain, generalized weakness, nausea, and vomiting within a short time after taking each weekly dose. The evaluation demonstrated multiorgan involvement, including acute liver injury, hyperbilirubinemia, hyperferritinemia, markedly e…

Acute Respiratory Failure due to Pulmonary Toxoplasmosis in an Immunocompetent Patient: A Case Report.

Pulmonary toxoplasmosis is a rare but potentially life-threatening manifestation of Toxoplasma gondii infection in immunocompetent individuals. Clinical presentation is often nonspecific and may mimic viral or atypical pneumonia, leading to delayed diagnosis. We report the case of a previously healthy 32-year-old man who presented with a mononucleosis-like syndrome complicated by rapidly progressive hypoxemic respiratory failure requiring invasive mechanical ventilation. Initial evaluation revealed marked systemic inflammation, cytopenias, hyperferritinemia, and mixed-pattern hepatitis. Chest …

Adult-Onset Still's Disease-A Complex Disease, a Challenging Treatment.

Adult-onset Still's disease (AOSD) is a systemic inflammatory disorder with an unknown cause characterized by high-spiking fever, lymphadenopathy, hepatosplenomegaly, hyperferritinemia, and leukocytosis. The clinical course can be divided into three significant patterns, each with a different prognosis: Self-limited or monophasic, intermittent or polycyclic systemic, and chronic articular. Two criteria sets have been validated. The Yamaguchi criteria are the most generally used, although the Fautrel criteria offer the benefit of adding ferritin and glycosylated ferritin values. AOSD's pathogen…

Multicentric Castleman disease (MCD) is a rare lymphoproliferative disorder that mainly affects middle-aged patients with human immunodefici … lymphadenopathy. Laboratory findings showed anemia and increased markers of inflammation including hyperferritinemia and polyclonal hyperglobulinemia. Infectious causes including HIV were ruled out. Successful treatment of refractory acute lupus haemophagocytic syndrome using rituximab: a case report.

Systemic lupus erythematosus (SLE)-associated haemophagocytic lymphohistiocytosis (HLH) is called acute lupus haemophagocytic syndrome (ALHS), which is relatively rare but life-threatening. We present the case of a 43-year-old woman diagnosed with SLE with panniculitis, pleuritis, and autoimmune hepatitis. She was treated with high-dose glucocorticoids. Although disease activity temporarily improved, she developed fever, elevation of liver enzymes, hyperferritinemia, severe inflammatory response, and thrombocytopenia a month after starting glucocorticoids. Bone marrow biopsy was performed and …

Dynamic hematologic recovery patterns to guide clinical differentiation along the dengue-hemophagocytic lymphohistiocytosis hyperinflammatory spectrum: A case report.

Dengue infection may trigger a hyperinflammatory response that closely resembles secondary hemophagocytic lymphohistiocytosis (HLH), making therapeutic decision-making particularly challenging in patients who fulfill multiple HLH-associated diagnostic criteria. Distinguishing self-limited dengue-associated immune activation from progressive HLH requiring immunomodulatory therapy remains an important clinical dilemma. We report a 19-year-old woman presenting with prolonged fever, pancytopenia, hepatosplenomegaly, marked hyperferritinemia, hypofibrinogenemia, hypertriglyceridemia, elevated solub…
