Phenotypic and Genotypic Characterization and Treatment of a Cohort With Familial Tumoral Calcinosis/Hyperostosis-Hyperphosphatemia Syndrome.

Familial tumoral calcinosis (FTC)/hyperostosis-hyperphosphatemia syndrome (HHS) is a rare disorder caused by mutations in the genes encoding fibroblast growth factor-23 (FGF23), N-acetylgalactosaminyltransferase 3 (GALNT3), or KLOTHO. The result is functional deficiency of, or resistance to, intact FGF23 (iFGF23), causing hyperphosphatemia, increased renal tubular reabsorption of phosphorus (TRP), elevated or inappropriately normal 1,25-dihydroxyvitamin D3 (1,25D), ectopic calcifications, and/or diaphyseal hyperostosis. Eight subjects with FTC/HHS were studied and treated. Clinical manifestati…

Hyperostosis-hyperphosphatemia syndrome (HHS): report of two cases with a recurrent mutation and review of the literature.

Hyperostosis-hyperphosphatemia syndrome (HHS) is a rare autosomal recessive metabolic disorder caused by mutations in the GALNT3 and FGF23 genes. The main features of this disorder include painful swelling of long bones, increased renal reabsorption of phosphate but normal renal function and vitamin D and parathormone levels. Previously, we reported a novel missense mutation in the FGF23 gene in a patient suffering from HHS. In the present report, we demonstrated the same mutation (c.471C>A) in two other cases of HHS with similar clinical manifestations. As this nucleotide change has not been …

A case of synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome presenting with hypertrophic pachymeningitis.

A 43-year-old woman with a 3-year history of headache, fever, and swelling of the forehead, presented to our hospital. A general examination … n biopsy indicated palmoplantar pustulosis. From these results, SAPHO (synovitis, acne, pustulosis, hyperostosis and osteitis) syndrome with associated hypertrophic pachymeningitis was diagnosed. After corticosteroid therapy and tonsillectomy, the clinical symptoms and radiological abnormalities were improved. Clinicians should be aware of SAPHO as a potential …

Radiographic and MRI characteristics of lumbar disseminated idiopathic spinal hyperostosis and spondylosis deformans in dogs.

OBJECTIVES: To evaluate clinical signs, describe lesions and differences in the magnetic resonance imaging appearance of spinal new bone formations classified as disseminated idiopathic spinal hyperostosis and/or spondylosis deformans on radiographs and compare degeneration status of the intervertebral discs using the Pfirrmann scale. METHODS: Retrospective analysis of 18 dogs presented with spinal disorders using information from radiographic and magnetic resonance imaging examinations. RESULTS: All dogs were found to be affected with both disseminated idiopathic spinal hyperostosis and spo…

Diffuse idiopathic skeletal hyperostosis and familial hypocalciuric hypercalcemia: a unique association in a young female.

Familial hypocalciuric hypercalcemia (FHH) is a cause of lifelong hypercalcemia resulting from inactivated mutations in the calcium sensing receptor gene. Diffuse idiopathic skeletal hyperostosis (DISH) is an under diagnosed condition that leads to ossification of ligaments and entheses of the spine and peripheral skeleton. We describe a 45-year-old diabetic woman with hypercalcemia secondary to FHH who developed dysphagia because of external esophageal compression from DISH. The patient had no additional identifiable risk factors for DISH except for diabetes mellitus. An association between F…

Synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO) syndrome with review of the relevant published work.

Synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO) syndrome is characterized by various dermatological manifestations and osteoarthropathy frequently localized to the anterior chest wall. Dermatologists should be familiar with this syndrome. Its early diagnosis is important to avoid prolonged antibiotic treatments and unnecessary invasive procedures. We report a new case of this syndrome with review of the relevant published work.

Genetic analysis and effect of triiodothyronine and prednisone trial on bone turnover in a patient with craniotubular hyperostosis.

Craniotubular hyperostosis are a group of high bone mass disorders related to mutations in the LRP5 and SOST genes, although other causative genes remain to be identified. Little is known about the bone turnover and the response to T3 or glucocorticoids in these patients. We describe a patient with craniotubular hyperostosis, including mutation analyses of the LRP5, SOST, DKK1 and KRM1 genes. We also studied bone turnover and bone mineral density (BMD), before and after a trial with T3 (75 microg/d for 28 weeks) and T3 and prednisone (T3 100 microg/d for 2 weeks, followed by 10 weeks on predni…

Hyperextension fracture-dislocation of the thoracic spine with paraplegia in a patient with diffuse idiopathic skeletal hyperostosis.

The case is described of a hyperextension thoracic spine fracture in a patient with diffuse idiopathic skeletal hyperostosis (DISH). A 70-year-old woman awoke paraplegic after retroperitoneal surgery after having been positioned in the hyperextended axially rotated position. Imaging demonstrated a T9-10 fracture-dislocation with severe pressure on the spinal cord, as well as findings typical of DISH. This case emphasizes the danger inherent in manipulating the ankylosed hyperostotic spine. It is proposed that patients undergoing surgery in this position should be evaluated for DISH as an addit…

Acquired hyperostosis syndrome: spectrum of manifestations at the sternocostoclavicular region. Radiologic evaluation of 34 cases.

Thirty-four patients with chest wall hyperostosis, a condition which has been designated by various terms in the literature were evaluated radiologically. We prefer the name acquired hyperostosis syndrome (AHS), which we categorize into the complete, incomplete and possible form. In complete AHS, sternocostoclavicular hyperostosis is associated with axial and/or peripheral (endosteal, periosteal, enthesopathic, metaplastic) hyperostosis and with psoriasiform or acneform dermatosis. In addition, these three manifestations are accompanied by erosive or non-erosive peripheral and/or axial arthrit…

[The elbow exposed to vibrating instruments and idiopathic diffuse skeletal hyperostosis: association or pure coincidence?].

A group of 11 patients with chronic enthesopathy of the elbow caused by vibrating instruments was systematically examined for spinal signs of diffuse idiopathic skeletal hyperostosis (DISH). Nine patients revealed spinal involvement, the two exceptions probably relating to the patients' youth. The findings support the hypothesis that DISH is not a disease but rather a tendency to excessive ossification of ligamentous structures in response to stimuli that generally produce only modest new bone formation. Those suffering from such ossifying diathesis should be directed towards work unlikely to …

The etiology and porotic hyperostosis among the prehistoric and historic Anasazi Indians of Southwestern United States.

Porotic hyperostosis was studied in 539 crania from maize-growing prehistoric and historic groups who occupied two dissimilar ecological zones of the Plateau country of Arizona and New Mexico--canyon bottoms and sage plain. Defined as abnormal localized sieve-like structural changes involving the hematopoietic areas of the cranium, it was found in 185 (34.3%) of these skulls. More frequent in children than in adults, it shows significant frequency differences between both children and adults of the two ecological zones. The two ecological zones differ in the availability of iron in the diet; t…

Hyperostosis Cranialis Interna Presenting With Cerebrospinal Fluid Leak: A Case Report and Review of Literature.

This report is of a case of hyperostosis cranialis internal (HCI), a rare disease characterized by the abnormal thickening of cranial bones, usually in the frontal and parietal regions. It is an uncommon disease that has clinical significance because of its possible symptoms: neurologic, aesthetic concerns, or asymptomatic, which may incidentally be found on imaging. Detailed attention has been given to the case of a 32-year-old female patient who presented with a two-year history of right clear, salty nasal discharge, anosmia, and band-like headache. Computed tomography and magnetic resonance…

Right Forearm Arteriovenous Malformation with Dual Osseous Remodeling: Hyperostosis and Erosion of Ulna and Radius.

Arteriovenous malformations (AVMs) are rare vascular anomalies, characterized by direct communications between arteries and veins without an … g predominantly into the basilic vein. Associated osseous changes in the ulna and radius, including hyperostosis, cortical thickening, periosteal reaction, and focal erosions, were also noted. Congenital cortical hyperostosis: a rare cause of inconsolable crying in a baby. Clinical case report.

Here we describe the case of a 2-month-old infant who consulted several times due to excessive crying, initially interpreted as having a gastrointestinal cause. Since the symptom persisted, a fracture was suspected due to its association with mobilization of the limbs and palpation of a mass on the anterior aspect of the right tibia. X-rays showed diaphyseal polyostotic involvement and lesions compatible with cortical involvement of long bones. Caffey-De Toni-Silverman syndrome was diagnosed and treatment with nonsteroidal anti-inflammatory drugs was initiated, resulting in symptom remission.

Patterns of hyperostosis during the ontogenic development of Atlantic spadefish (Broussonet, 1782) in Brazilian coast.

Marine teleost species of commercial interest are often reported with hyperostosis, an osteological condition that results in bone thickening. Various specimens of Atlantic Spadefish Chaetodipterus faber (n = 86) obtained from artisanal fishermen in Rio de Janeiro, Brazil, were radiographed and assessed to detect the occurrence of hyperostosis across four different size classes. Of the examined individuals, 58.62% displayed signs of hyperostosis, which manifested in eight skeletal regions, notably in the supraoccipital crest, cleithrum and supraneural areas. In the vertebral column, hyperostos…

Otolaryngologic manifestations of diffuse idiopathic skeletal hyperostosis (Forestier's disease): A systematic review of the literature.

OBJECTIVES: Forestier's disease or diffuse idiopathic skeletal hyperostosis (DISH) is a common, yet underreported, disease affecting the elderly population. From an otolaryngologic perspective, DISH may manifest with dysphagia, dysphonia, or even dyspnea. The purpose of this study was to identify all published cases of dysphagia and other associated upper airway symptoms resulting from DISH in the last decade and to establish subsequently a management algorithm. METHODS: A comprehensive review of the literature was conducted in May 2020 on Medline and Embase databases following the PRISMA sta…

Micro-computed Tomography Study of Frontal Bones in Males and Females with Hyperostosis Frontalis Interna.

Hyperostosis frontalis interna (HFI) represents irregular thickening of the endocranial surface of the frontal bone, mostly seen in postmenopausal females. The microarchitecture of this condition is poorly studied. The aim of this cross-sectional autopsy study was to investigate and compare microarchitectural structure of the frontal bone affected with HFI in both sexes and to test whether HFI severity could be distinguished at the microarchitectural level. The sample was taken from human donor cadavers, 19 males (61 ± 15 years old) and 17 females (75 ± 15 years old). After classification of H…
