Effect of fluconazole prophylaxis on candidiasis and mortality in premature infants: a randomized clinical trial.

IMPORTANCE: Invasive candidiasis in premature infants causes death and neurodevelopmental impairment. Fluconazole prophylaxis reduces candidiasis, but its effect on mortality and the safety of fluconazole are unknown. OBJECTIVE: To evaluate the efficacy and safety of fluconazole in preventing death or invasive candidiasis in extremely low-birth-weight infants. DESIGN, SETTING, AND PATIENTS: This study was a randomized, blinded, placebo-controlled trial of fluconazole in premature infants. Infants weighing less than 750 g at birth (N = 361) from 32 neonatal intensive care units (NICUs) in the

Less invasive surfactant administration in extremely preterm infants: impact on mortality and morbidity.

BACKGROUND: A new mode of surfactant administration without intubation - less invasive surfactant administration (LISA) - has recently been described for premature infants. OBJECTIVE: We report single-center outcome data of extremely premature infants who have been managed by LISA in our department. Mortality and morbidity rates of the cohort were compared to historical controls from our own center and to data of the Vermont-Oxford Neonatal Network (VONN). PATIENTS AND METHODS: All infants born at 23-27 weeks' gestational age during 01/2009 and 06/2011 (n = 224) were managed by LISA and incl

Perioperative atrial tachycardia is associated with increased mortality in infants undergoing cardiac surgery.

OBJECTIVE: Few data are available on the frequency or importance of perioperative atrial tachycardia in infants. We hypothesized that atrial tachycardia in infants undergoing cardiac surgery is not rare and is associated with increased morbidity and mortality. METHODS: From 2007 through 2010, 777 infants (median age, 1.8 months; interquartile range, 0.33-5.73) underwent cardiac surgery. Their medical records were reviewed for atrial tachycardia during the perioperative period. RESULTS: Of the 777 patients, 64 (8.2%) developed atrial tachycardia. The independent risk factors for developing at

Prophylactic versus selective use of surfactant in preventing morbidity and mortality in preterm infants.

BACKGROUND: Surfactant therapy is effective in improving the outcome of very preterm infants. Trials have studied a wide variety of surfactant preparations used either to prevent or treat respiratory distress syndrome (RDS). In animal models, prophylactic surfactant leads to more homogeneous distribution and less evidence of lung damage. However, administration requires intubation and treatment of infants who will not go on to develop RDS. This is of particular concern with the advent of improved approaches to providing continuous distending pressure, particularly in the form of nasal continuo

The effect of late preterm birth on mortality of infants with hypoplastic left heart syndrome.

OBJECTIVES: We evaluated the effect of late preterm (34 to 36 weeks' gestation) delivery on hospital mortality of infants with hypoplastic left heart syndrome (HLHS). STUDY DESIGN: Retrospective review of records of infants born at or after 34 weeks with no other lethal anomalies, cared for in a single tertiary perinatal center between 2002 and 2009. Factors associated with death prior to discharge from the hospital were ascertained using univariate and multivariate analyses. RESULTS: Of the 243 infants with HLHS, 35 were late preterm and 208 were ≥37 weeks (term). Using logistic regression

Factors associated with in-hospital mortality in infants undergoing heart transplantation in the United States.

OBJECTIVE: Infants undergoing heart transplantation have the highest early posttransplant mortality of any age group. We sought to determine the pretransplantation factors associated with in-hospital mortality in transplanted infants in the current era. METHODS: All infants under 12 months of age who underwent primary heart transplantation during a recent 10-year period (1999-2009) in the United States were identified using the Organ Procurement and Transplant Network database. Multivariable logistic regression was used to identify independent pretransplantation factors associated with in-hos

Ethamsylate for the prevention of morbidity and mortality in preterm or very low birth weight infants.

BACKGROUND: Ethamsylate decreases blood loss in certain clinical situations such as menorrhagia and following some surgical procedures. This potential to reduce bleeding has led to the hypothesis that it may have a role to play in reducing intraventricular haemorrhage in preterm infants. OBJECTIVES: To determine if ethamsylate, when compared to placebo or no treatment, reduces morbidity and/or mortality in preterm infants. SEARCH STRATEGY: We searched the Cochrane Neonatal Group Trials Register (24 August 2009), the Cochrane Central Register of Controlled Trials (CENTRAL, The Cochrane Librar

Infants born at the threshold of viability in relation to neonatal mortality: Colorado, 1991 to 2003.

OBJECTIVE: To determine the contribution of infants born at the threshold of viability (defined as <750 g birth weight) and the role of regionalization of perinatal care on the neonatal mortality rate (NMR) in Colorado. STUDY DESIGN: We performed a retrospective cohort study, evaluating all live births in Colorado from 1991 to 2003, and comparing the periods 1991 to 1996 versus 1997 to 2003. RESULT: The overall unadjusted NMR of the two time periods was 4.3 and 4.4 per 1000 live births, respectively (P=0.42). The contribution of infants with birth weights<750 g to the overall NMR increased f

Allopurinol for preventing mortality and morbidity in newborn infants with suspected hypoxic-ischaemic encephalopathy.

BACKGROUND: Delayed neuronal death following a perinatal hypoxic insult is due partly to xanthine oxidase-mediated production of cytotoxic free radicals. Evidence exists that allopurinol, a xanthine-oxidase inhibitor, reduces delayed cell death in animal models of perinatal asphyxia and in human patients with other forms of organ reperfusion injury. OBJECTIVES: To determine the effect of allopurinol on mortality and morbidity in newborn infants with suspected hypoxic-ischaemic encephalopathy. SEARCH STRATEGY: The standard search strategy of the Cochrane Neonatal Review Group was used. This i

Aetiologies and risk factors for neonatal sepsis and pneumonia mortality among Alaskan infants.

We evaluated all fatal neonatal sepsis and pneumonia cases occurring in Alaska during 1992-2000. Risk factors were evaluated using a database of all births occurring during the study period. Of 32 cases, group B streptococcus (GBS) was isolated from 21% (all 7 days of age), non-GBS Gram-positive bacteria from 50% (53% <7 days of age), and Gram-negative infections from 38% (58% <7 days of age). Infants born at <37 weeks gestation accounted for 72% of cases and had an increased risk of GBS [rate ratio (RR) 9.1, 95% confidence interval (CI) 2.0-41] and non-GBS (RR 40, 95% CI 16-101) disease. [Mortality and morbidity of low-birth-weight infants. Current prognosis and future perspectives].

In a retrospective epidemiologic study conducted from January 1996 to December 2002, 2555 low-birth-weight infants (less than 2500 g) were admitted to the neonatal intensive care unit of E Herriot Hospital in Lyon, France. Very low birth weight (795 infants) was defined as a birth weight below 1500 g. The total mortality rate among these latter infants was 8%. The mortality rate was less than 25% among infants with a gestational age of at least 25 weeks. Neonatal morbidity mainly consisted of respiratory distress syndrome and cerebral hemorrhage. Sequelae at age 2 years (corrected) affected le

Tracheal gas insufflation for the prevention of morbidity and mortality in mechanically ventilated newborn infants.

BACKGROUND: Tracheal gas insufflation (TGI) is a technique where a continuous flow of gas is instilled into the lower trachea during conventional mechanical ventilation. TGI can improve carbon dioxide removal with lower ventilation pressures and smaller tidal volumes, potentially decreasing secondary lung injury and chronic lung disease (CLD). OBJECTIVES: To assess whether, in mechanically ventilated neonates, the use of tracheal gas insufflation reduces mortality, CLD and other adverse clinical outcomes without significant side effects. SEARCH STRATEGY: Searches were made of MEDLINE 1966 to

Abnormal vitamin A cytology and mortality in infants aged 9 months and less with measles.

We investigated the relationship between abnormal vitamin A cytology and mortality in infants less than 9 months of age with measles. In a 12-month period, 116 children of this age with measles consecutively admitted to Queen Elizabeth Central Hospital (QECH) in Blantyre, Malawi were enrolled in the study. All guardians of patients were interviewed and clinical information and consent were obtained in a standardised fashion. Conjunctival impression cytology (CIC) was attempted on all and collected from 93 (80%) children. The overall mortality was 16%. The proportional hazards model revealed th

Intravenous indomethacin for preventing mortality and morbidity in very low birth weight infants.

BACKGROUND: Patent ductus arteriosus (PDA) and intraventricular haemorrhage (IVH) are both associated with increased mortality and morbidity in preterm infants. Indomethacin has been used successfully to treat symptomatic PDA and may also prevent or limit IVH in the neonatal period. There are however potential unwanted side effects of indomethacin, in particular a potential for reduced organ perfusion that might outweigh any clinical benefits. The prophylactic use of indomethacin, where infants who may not have gone on to develop a symptomatic PDA or IVH would be exposed to indomethacin, warra

Restricted versus liberal water intake for preventing morbidity and mortality in preterm infants.

BACKGROUND: Most premature infants are not sufficiently mature physiologically to take all of their required water and nutrients orally, and so they cannot regulate their own water intake. Thus, the caregiver must determine the amount of water to be given each day to such infants. OBJECTIVES: The objective of this review is to examine the effects of water intake on postnatal weight loss and on the risks of dehydration, patent ductus arteriosus, necrotizing enterocolitis, bronchopulmonary dysplasia, intracranial hemorrhage, and death in premature infants.
