OBJECTIVE: To discuss the combined use of selective epidural steroid injection (ESI) and spinal manipulative therapy (SMT) in treating recalcitrant lumbar radiculopathy. CLINICAL FEATURES: In a first case, a patient suffered from numbness and pain in an S1 dermatome distribution 6 months after undergoing lumbar discectomy. Clinical correlation with advanced imaging led to the diagnosis of L5 intervertebral disc (IVD) syndrome with peridural scar formation. In a second case, a patient suffered from low-back and right-leg pain of 9 months' duration. Various forms of conservative treatment were n

Epidural steroid therapy is one of the useful conservative modalities of treatment of low back pain with sciatica. Epidural injection of a mixture of 80 mg (2 ml) of methylprednisolone, 25 ml of normal saline and 10 ml of 2% lignocaine hydrochloride through sacral route produced excellent to good results in 83% of patients of low back pain with sciatica with subacute symptoms of less than 3 months duration, when evaluated after 3 months of injection. When evaluated after one year of injection the success rate came down to 64.8%.

A review of pain clinic patients was carried out to determine whether patients who obtained limited or no relief from epidural injection of steroids derived benefit from subsequent subarachnoid steroid injections. Only 1 of 13 patients who had no benefit from epidural steroids improved following subarachnoid injection. Five of 6 patients with partial relief from epidural steroids had further improvement after subarachnoid injection.

BACKGROUND: Chronic low back pain with associated leg pain (CLBP-L) (whether referred or radiating) is a common, debilitating, and expensive condition. Epidural steroid injections (ESI) are commonly used, although effectiveness is often limited. Dextrose prolotherapy injections (DPT) are a potential alternative treatment to address the biomechanical and neurological causes of CLBP-L.
OBJECTIVE: To assess the effectiveness of DPT compared with ESI for CLBP-L.
DESIGN: Pragmatic unblinded, randomized controlled trial.
SETTING: Outpatient pain clinic; ESIs were performed in an operating room, D

INTRODUCTION: Epidural steroid injection (ESI) techniques have demonstrated effectiveness in improving short- and intermediate-term pain and disability outcomes; however, their efficacy may vary depending on the specific approach used. This meta-analysis aims to compare disability and pain outcomes among different ESI techniques - transforaminal (TFESI), interlaminar (ILESI), caudal (CESI), and combined approaches - in patients with lumbar radiculopathy.
METHODS: The authors searched PubMed/MEDLINE, Scopus, and Cochrane from inception to December 16, 2025, in accordance with PRISMA guidelines

BACKGROUND: Epidural steroid injections are indicated for radicular pain, with a pre-injection MRI not mandated in guidelines. There is conflicting evidence that MRI findings correlate with outcomes.
METHODS: Fourteen patients with near-normal imaging (i.e., no evidence of nerve root compression and minor degenerative changes if present) who underwent ESI for lumbosacral radicular pain and were followed for up to 12 weeks were propensity matched against 14 patients with radicular pain secondary to concordant MRI pathology. The primary outcome measure was mean reduction in average leg pain rel

PURPOSE: Despite numerous studies, the factors contributing to clinical success after a lumbar transforaminal epidural steroid injection (LTFESI) for radicular pain remain unclear. The aim of this study was to systematically review literature evaluating preprocedural predictive factors for improved outcomes in patients receiving a LTFESI.
METHODS: We searched databases including PubMed/MEDLINE, Cochrane Library, and Scopus for studies published from 2006 through 2023. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed to identify articles

PURPOSE: Obesity, as defined by the CDC, is characterized by a body mass index exceeding 30. Over one third of the world's population is classified as overweight or obese. Furthermore, low back pain is currently the leading cause of disability worldwide. Patients who do not respond adequately to conservative therapy can consider more invasive procedures such as epidural steroid injections (ESI) as treatment for their low back pain. The purpose of this study was to compare the efficacy of lumbar epidural steroid injections for lumbosacral radiculopathy in individuals with obesity as defined by

OBJECTIVE: Interlaminar cervical epidural steroid injections (ICESIs) are commonly used to treat axial neck pain and cervical radicular pain. However, local anesthetics can spread to and block the phrenic nerve and upper segments of the thoracic spinal cord where the sympathetic innervation of the lungs emerges. Therefore, changes in lung function may occur following ICESIs.
METHODS: The primary outcome measure was the pulmonary function test (PFT) result 30 minutes before and after ICESI with ropivacaine (0.1875% or 0.25%). The secondary outcome measure was the comparison of the pain scores

BACKGROUND: Epidural injections are among the most commonly performed procedures for managing low back and lower extremity pain. Pinto et al and Chou et al previously performed systematic reviews and meta-analyses, which, along with a recent update from Oliveira et al showing the lack of effectiveness of epidural steroid injections in managing lumbar disc herniation, spinal stenosis, and radiculopathy. In contrast to these papers, multiple other systematic reviews and meta-analyses have supported the effectiveness and use of epidural injections utilizing fluoroscopically guided techniques. A m

BACKGROUND: Spinal malignancy-related pain results from tumor, fracture, instability, inflammation, and/or nerve root/spinal cord compression. Systemic corticosteroids are commonly used but have many undesirable adverse effects that impact quality of life and continuation of cancer treatments. Epidural steroid injections (ESIs) may be a viable alternative pain treatment.
OBJECTIVES: This study starts with a pragmatic review on the efficacy of ESIs to treat spinal malignancy-related pain. Given the limited evidence, we supplement the study with a single-center, retrospective review.

Lumbar epidural steroid injections (LESIs) are commonly used for managing lower back pain (LBP) and radicular pain. LESIs are generally considered safe with only rare serious complication. One very rare complication that is frequently cited in the literature is adhesive arachnoiditis. However, a literature search failed to detect even one published manuscript, clearly documenting LESI induced arachnoiditis. This article presents two patients who received a transforaminal L5-S1 and two L3-L4 interlaminar LESIs. Although the presented patients developed clear radiological (MRI) findings of arach
