Facial affective relationship offers of patients with panic disorder.

Abstract The psychodynamic of patients with panic disorder is most often characterized by a domination of autonomy-dependency conflicts, including an ambivalent need for a positive relationship toward a significant object and an incapacity to express negative feelings because of fear of losing this relationship. Affective facial behavior can be considered as a tool for relationship regulation. The authors investigated facial affective indicators of this conflict in 20 women with panic disorder in their first psychotherapy session. A preponderance of facial smile and a lack of negative affectiv

Defining and improving response to treatment in patients with bipolar disorder.

Functional outcomes are more meaningful measures of response to treatment for bipolar disorder than are scores on various psychiatric rating scales (all of which have limitations) used to gauge improvement in symptoms. With the former approach, patients are considered to be in remission if they achieve normal or near-normal levels of functioning in occupational, family, and social settings. Sleep patterns are reliable indicators of whether a patient with bipolar disorder is likely to relapse or sustain remission in the near term. Regularly scheduled nightly sleep periods may help prevent rapid

Comprehensive management of persistent somatization: an innovative inpatient program.

Psychiatry has been largely unsuccessful in developing effective treatment strategies for patients with chronic somatization. They continue to utilize a disproportionate share of health care resources and experience personal distress and significant social and occupational dysfunction. The Psychosomatic Medicine Unit at the Clarke Institute of Psychiatry is attempting to treat these patients with a comprehensive treatment program that takes into account the multifactorial etiology of the disturbance. The unit is in a tertiary care facility and the patient population is heterogeneous with respe

Clinical utility of the Level of Personality Functioning Scale: A survey of German mental health professionals.

The Alternative DSM-5 Model for Personality Disorders and ICD-11 adopt a dimensional approach to personality disorders, defining them in terms of impairments in self-related (identity, self-direction) and interpersonal (empathy, intimacy) personality functions. This study examined the clinical utility of a clinician-rated global evaluation of personality functioning based on the Level of Personality Functioning Scale (LPFS). A total of 346 German mental health professionals provided LPFS ratings for 1,403 patients, stratified by 38 diagnostic categories, including, but not limited to, personal

The Occurrence of and Distress From Pathogenic Beliefs: Examining Pathways From Childhood Adversity to Psychopathology.

INTRODUCTION: Pathogenic beliefs are dysfunctional beliefs that impede the pursuit of adaptive goals, which have been shown as key pathways between early childhood trauma and psychopathology. This study examined whether the occurrence of pathogenic beliefs and the distress from having pathogenic beliefs, which were assessed through the latest version of the pathogenic belief scale (PBS-21) separately, would both mediate the childhood adversity-outcome relationship, and whether reactivity of pathogenic beliefs (standardized difference between distress and occurrence) moderates them. METHODS: A

The Promise of Intranasal Oxytocin in Treating Borderline Personality Disorder: A Narrative Review.

BACKGROUND/OBJECTIVES: Borderline personality disorder (BPD) is a complex psychiatric condition marked by emotional dysregulation, interpersonal instability, and impulsivity. Despite the advances in psychotherapy and pharmacotherapy, many patients show a partial or unstable response. Recent research suggests that oxytocin, a neuropeptide involved in social cognition and emotional regulation, may offer novel therapeutic avenues. METHODS: We systematically synthesize evidence from PubMed, PsycINFO, Web of Science, and Google Scholar on oxytocin's role in BPD, prioritizing studies on neurobiolog

Cognitive behavior therapy of binge eating disorder.

Binge eating disorder (BED) is characterized by recurrent episodes of uncontrollable eating, even when not hungry, until uncomfortably full, occurring at least twice a week for a 6-month period. This is differentiated from bulimia nervosa (BN) by the lack of compensatory mechanisms such as purging/laxative abuse. There are significantly higher levels of psychiatric symptoms in patients with BED as compared to those without BED. Furthermore, depressive symptomatology may increase the patient's vulnerability to binge eating as well as to relapse after treatment. Grazing is defined as eating smal

The SPC model as a heuristic tool to better understand autism spectrum disorder and other structural diagnosis in psychiatry and psychotherapy.

Both autism and the broader autism phenotype represent stable mental features over time that often lead to typical psychosocial problem patterns such as bullying, interpersonal conflicts at work and at home, or social withdrawal. These patterns may in turn cause specific temporary medical conditions such as states of tension, agitation, mutism, depression, or psychosis. When presenting to psychiatry or psychotherapy patients generally do not complain about autism, but rather about depression, interpersonal conflicts, and psychosocial difficulties. They may also complain about maladaptive behav

Preference of mHealth versus in-person treatment for depression and post-traumatic stress disorder in Kenya: demographic and clinical characteristics.

OBJECTIVES: We conducted an implementation science mental health treatment study in western Kenya, testing strategies for scale up of evidence-based mental health services for common adult disorders using a non-specialist workforce, integrated with existing primary care (Sequential Multiple, Assignment Randomized Trial of non-specialist-delivered psychotherapy (Interpersonal Psychotherapy) and/or medication (fluoxetine) for major depression and post-traumatic stress disorder (PTSD) (SMART DAPPER)). How Do Adult Survivors of Childhood Sexual Abuse Experience Formally Delivered Talking Therapy? A Systematic Review.

This systematic review explored how adult survivors of child sexual abuse experienced nonspecific and trauma-focused talking therapies. Following extensive systematic searches of academic databases, 23 qualitative empirical studies were chosen for review. Using a line-by-line thematic synthesis, four analytical themes developed. These themes were: The Therapeutic Process as a Means for Forming Connections, which discusses therapeutic relationships; Developing a Sense of Self Through the Therapeutic Processes, which identifies stages of developmental recovery; Therapeutic Lights and Black Holes

[Development of the psychosomatic concept of perioral dermatitis].

In recent years, ,perioral dermatitis' though practically unknown in the past, has been observed rather frequently in female patients. It has proven remarkably refractory against external dermatotherapy, and numerous attempts to analyse the causality have failed. However, we very often noted certain characteristics of personality structure and social attitude in the patients afflicted with the disease. Both clinical findings and various signs of vegetative dystonia suggested psychoneurotic rather than purely somatic causes. We therefore set about to elucidate the psychic and other clinical sym

Visualization of Internal Working Models Through Transactional Analysis (TA) Developmental Collage Therapy: A Case Report.

Bowlby's Internal Working Model (IWM) is central to understanding adult attachment-related difficulties. However, what IWM concretely consists of remains unspecified, and no method exists for directly visualizing IWM. Because IWM is formed during the pre-verbal period, it is difficult to access through language-based psychotherapy alone. A woman in her 20s with a history of childhood stuttering and subsequent interpersonal difficulties participated in Transactional Analysis (TA) Developmental Collage Therapy. She created six collage works representing "myself and my surrounding environment" ac

Anterior hippocampal volume predicts affect-focused psychotherapy outcome.

BACKGROUND: The hippocampus plays an important role in psychopathology and treatment outcome. While posterior hippocampus (PH) may be crucial for the learning process that exposure-based treatments require, affect-focused treatments might preferentially engage anterior hippocampus (AH). Previous studies have distinguished the different functions of these hippocampal sub-regions in memory, learning, and emotional processes, but not in treatment outcome. Examining two independent clinical trials, we hypothesized that anterior hippocampal volume would predict outcome of affect-focused treatment o

The therapeutic alliance in the treatment of personality disorders.

Because personality disorders are associated with significant impairment in interpersonal relationships, special issues and problems arise in the formation of a therapeutic alliance in the treatment of patients with these disorders. In particular, patients with narcissistic, borderline, and paranoid personality traits are likely to have troubled interpersonal attitudes and behaviors that will complicate the patient's engagement with the therapist. While a strong positive therapeutic alliance is predictive of more successful treatment outcomes, strains and ruptures in the alliance may lead to p

Adjuvant Psychotherapies to Prevent Relapse in Bipolar Disorder: A Randomized Clinical Trial.

IMPORTANCE: Several psychotherapy protocols have been evaluated as adjuncts to pharmacotherapy for patients with bipolar disorder (BD). Little is known about their comparative effectiveness. OBJECTIVE: To compare the effectiveness of 2 types of group psychotherapy, skill-oriented, material-based cognitive behavioral therapy (SEKT) and supportive, patient-centered, emotion-focused therapy (FEST), to prevent relapse in patients with euthymic BD. DESIGN, SETTING, AND PARTICIPANTS: This was a large, observer-blind, randomized clinical trial conducted over 18 months (posttreatment after 6 months;

Effect of 3 Forms of Early Intervention for Young People With Borderline Personality Disorder: The MOBY Randomized Clinical Trial.

IMPORTANCE: Clinical trials have neither focused on early intervention for psychosocial impairment nor on the contribution of components of borderline personality disorder (BPD) treatment beyond individual psychotherapy. OBJECTIVE: To evaluate the effectiveness of 3 early interventions for BPD of differing complexity. DESIGN, SETTINGS, AND PARTICIPANTS: This single-blinded randomized clinical trial recruited young people between March 17, 2011, and September 30, 2015, into parallel groups. The study took place at 2 government-funded mental health services for young people in Melbourne, Austr

Best practice in psychological activities in cardiovascular prevention and rehabilitation: Position Paper.

Recent guidelines on cardiovascular disease prevention suggest multimodal behavioral interventions for psychosocial risk factors and referral for psychotherapy in the case of clinically significant symptoms of depression and anxiety overall. Accordingly, psychologists of the Italian Association for Cardiovascular Prevention, Rehabilitation and Epidemiology (GICR-IACPR) have reviewed the key components of psychological activities in cardiovascular prevention and rehabilitation (CPR). The aim of this study was to elaborate a position paper on the best practice in routine psychological activities

Group dynamic-relational therapy for perfectionism.

The interest in treating underlying core vulnerability factors or transdiagnostic processes has been a focus of much attention. In this paper we describe our application of group dynamic-relational psychotherapy to the treatment of perfectionism, a core personality vulnerability factor associated with various forms and types of dysfunction and disorders that have profound costs to the individual both socially and subjectively. Over the course of the past three decades, we developed an evidence-based integrative group treatment that targets the psychodynamic and relational underpinnings of perf

Emerging psychotherapies for eating disorders.

Eating disorders are serious illnesses associated with significant medical and psychological sequelae and, in the case of anorexia nervosa, significant mortality. Established psychotherapies such as cognitive-behavioral therapy and interpersonal therapy are effective for many patients with eating disorders. However, these treatments fail to yield full long-term remission in a substantial number of patients. There is a need for novel psychotherapeutic approaches for patients with eating disorders. The authors review three promising new treatment approaches in the field of eating disorders. Moti

More optimistic treatment expectations are associated with better outcomes through stronger group cohesion, but not dyadic alliance: results from a naturalistic day clinic study in complex depression.

INTRODUCTION: More optimistic treatment expectations are typically associated with better psychotherapy outcomes, including in depression. Meta-analytic findings suggest that this relationship is mediated by the dyadic alliance. In settings including group formats, however, patients also interact and build relationships with one another. Thus, group relationship may represent an important mediator, particularly given that depression is often accompanied by interpersonal difficulties. Therefore, we aimed to investigate the potential mediating role of group relationship above and beyond the dyad
