Rolling Stones Management of Gallstone Ileus Through a Single-Institution Retrospective Analysis.

Biliodigestive fistulas are a complication of gastrointestinal pathologies, often arising from chronic recurrent cholecystitis. These fistulas disrupt normal biliary physiology, leading to bile reflux or the entry of intestinal contents into the biliary system, with gallstone ileus as a significant consequence. This condition typically results in mechanical bowel obstruction due to the migration of gallstones into the intestinal lumen. Diagnosis can be challenging due to nonspecific symptoms, and surgical intervention is often required. This retrospective study reviews cases of gallstone ileus

An Unusual Presentation of Gallstone Ileus: A Red-Herring or Missed Diagnosis.

BACKGROUND: Gallstone ileus is a rare complication of chronic calcular cholecystitis and an uncommon etiological entity responsible for mechanical intestinal obstruction. The most common obstructed part is the narrow terminal ileum, whereas the jejunum is rarely affected. The gallstone is postulated to reach the small bowel by gradual erosion from the gall bladder, most commonly into the duodenum, forming a cholecysto-duodenal fistula. CASE REPORT: Herein, we report a 72-year-old male who presented with intestinal obstruction of a 5-day duration, with a clinical diagnosis of an irreducible in

[Neonatal enterolithiasis (or intraluminal meconial calcifications). Apropos of 4 cases].

Calcified intraluminal meconium is rarely observed in newborn. Only 27 cases are related in litterature; the authors report 4 new cases and point out the role of intestinal obstruction (principaly anorectal malformations, but also atresia, meconium ileus or Hirschsprung disease). Pathogenesis is unknown, while intestinal urinary fistula is frequently described.

Visualization of the intestinal tract by amniography in a fetus with esophageal atresia.

In pregnancies complicated by hydramnios, amniography can be used for the detection of fetal gastrointestinal obstruction. A case is presented in which the appearance of contrast material in the fetal gastrointestinal tract was noted in an infant who proved to have esophageal atresia and tracheoesophageal fistula. Presumably, the contrast reached the intestinal tract by aspiration and passage through the fistula.

Intestinal tuberculosis with abdominal complications: radiologic and pathologic features.

BACKGROUND: To investigate radiologic and pathological features of intestinal tuberculosis with abdominal complications. METHODS: Twenty-two patients with 23 surgically proven complications (nine intestinal obstructions, eight perforations, three fistulae, and three intestinal bleeds) were analyzed. Medical records, radiologic studies, and pathologic examinations were reviewed with special emphasis on searching for the common features in each group of complication. RESULTS: The most important single feature in seven of the nine patients with intestinal obstruction was the presence of strictu

Case Report: Abdominal Cocoon With Jejuno-Ileo-Colonic Fistula.

INTRODUCTION: Abdominal cocoon is a unique peritoneal disease that is frequently misdiagnosed. The occurrence of the abdominal cocoon with a jejuno-ileo-colonic fistula has not been previously reported. CASE PRESENTATION: We admitted a 41-year-old female patient with an abdominal cocoon and a jejuno-ileo-colonic fistula. She was admitted to our hospital for the following reasons: "the menstrual cycle is prolonged for half a year, and fatigue, palpitations, and shortness of breath for 2 months". On the morning of the 4th day of admission, the patient experienced sudden, severe, and intolerable

Inhibitory Effects and Sympathetic Mechanisms of Distension in the Distal Organs on Small Bowel Motility and Slow Waves in Canine.

Rectal distension (RD) is known to induce intestinal dysmotility. Few studies were performed to compare effects of RD, colon distension (CD) and duodenal distension (DD) on small bowel motility. This study aimed to investigate effects and underlying mechanisms of distensions in these regions on intestinal motility and slow waves. Eight dogs chronically implanted with a duodenal fistula, a proximal colon fistula, and intestinal serosal electrodes were studied in six sessions: control, RD, CD, DD, RD + guanethidine, and CD + guanethidine. Postprandial intestinal contractions and slow waves were

Lactose and protein absorption from breast milk and cow's milk preparations and its influence on the intestinal flora. Investigations on two infants with an artificial anus.

Lactose and protein absorption from breast milk and a cow's milk preparation enriched up to 7% of lactose were studied in two infants with an artificial anus applied in the ascending colon region. The concentrations of protein, lactose, glucose and galactose were measured in the fistula stools. In addition, the stools were analysed microbiologically. There were relatively high concentrations of lactose and its decomposition products and low concentrations of protein and aminonitrogen in the fistula stools when breast milk was fed. When the cow's milk formula was applied, only traces of lactose

Contrast radiology, computed tomography and ultrasonography in detecting internal fistulas and intra-abdominal abscesses in Crohn's disease: a prospective comparative study.

OBJECTIVE: Accurate assessment of intestinal complications of Crohn's disease (CD) is extremely important, both in clinical practice and in trials. The accuracy of radiographic and ultrasonographic diagnosis of internal fistulae and abscesses complicating CD is still debated and requires further investigation. We compared ultrasonography (US) and contrast radiography in detecting intestinal fistulae and abscesses complicating CD. METHODS: A prospective study was carried out on 625 consecutive CD patients who underwent complete evaluation of the intestinal tract by means of colonoscopy and dou…

Fistuloscopy--an adjuvant technique for sealing gastrointestinal fistulae.

Endoscopic occlusion of gastrointestinal fistulae can be successfully achieved in a high percentage of patients. In cases where the intestinal opening of the fistula is not accessible, such treatment was hitherto impracticable. Fistuloscopy is a new technique which we have devised, using a flexible endoscope advanced percutaneously along the fistula tract to seal these gastrointestinal fistulae. Eleven out of 17 enterocutaneous fistulae and 5 out of 8 abscesses could be sealed using this technique. The complications in this series were all caused by fibrin glue injection into the fistulae unde…

[Biliary-digestive fistula with gallstone ileus--a sonographic diagnosis].

We demonstrate a patient with a fistula between the gallbladder and the small intestine combined with a gallstone ileus of the small bowel, diagnosed by ultrasound. Plain abdominal X-ray only revealed small bowel obstruction. All the typical diagnostic criteria of a biliodigestive fistula could be detected by ultrasound. The patient was operated and discharged a few days later. We demonstrate that biliodigestive fistulas can be diagnosed by ultrasound.

Enhancing surgical precision: unveiling the impact of preoperative colonoscopy in anal fistula patients.

BACKGROUND: Anal fistula is a common benign anorectal disease that often requires surgical intervention for effective treatment. In recent years, preoperative colonoscopy as a diagnostic tool in patients with anal fistula has garnered increasing attention due to its potential clinical application value. By investigating underlying inflammatory bowel disease (IBD), polyps, and other abnormalities, preoperative colonoscopy can offer insights to refine surgical strategies and improve patient outcomes. METHODS: This retrospective study focused on 1796 patients with various benign anorectal diseas

Fluid filled small bowel loops in gallstone ileus: clinical and experimental observations.

A bowel fluid gas index was developed for the degree of fluid filled distention in small intestinal obstruction, utilizing the proportion of fluid filled versus gas filled loops on plain abdominal films. Comparison of simple distal small bowel obstruction due to adhesions and gallstone ileus demonstrated a significantly higher index in gallstone ileus, indicating more cases of fluid filled distention. In an experimental study, fistulae between the gall bladder and duodenum were fashioned in a series of dogs. Distal small bowel obstruction was then produced in these dogs and in another series o

Jejunal-rectal fistula as a complication of postoperative radiotherapy.

We present the case of a patient with an unusual, complex enteric fistula with multiple tracts and associated abscesses. The fistula was a late complication of radiotherapy, administered three years earlier, after resection for carcinoma of the sigmoid colon. Most of the small bowel was involved in the radiation-induced disease. A wide resection was performed successfully. This report reviews current literature on intra-abdominal postradiotherapy injuries, particularly intestinal fistulae.
