Tracheal cartilaginous sleeve association with syndromic midface hypoplasia.

IMPORTANCE: We report 2 new cases of tracheal cartilaginous sleeve and perform a systematic literature review of all documented cases of this condition. We aim to increase awareness of this anomaly and provide recommendations for both intraoperative and postoperative management of patients with tracheal cartilaginous sleeve undergoing tracheostomy. OBSERVATIONS: We studied the clinical records of 2 children with tracheal cartilaginous sleeves and short tracheas treated at our institution. One of these patients had Beare-Stevenson syndrome, and the other had Crouzon syndrome. Both patients req

Meconium Periorchitis.

Meconium periorchitis (MPO) is an unusual entity that presents as a scrotal mass in the early neonatal period and is seen in 1 in 30,000 live births. MPO is associated with meconium peritonitis, that leads to leakage of sterile meconium through patent processus vaginalis into the developing scrotum. Clinically, this entity presents as a para-testicular mass which shows scattered calcifications on imaging, thus warranting diagnostic work up and histopathological confirmation. Our case is an 8 month-old male baby with a firm to hard scrotal cyst. Intraoperatively, a diagnosis of teratoma was con

Rare Common Peroneal Nerve Neuropraxia after Removal of Upper Tibial Pin Traction.

Common peroneal nerve (CPN) injury is an uncommon complication associated with skeletal traction. Neurological deficits occurring after removal of upper tibial pin traction (UTPT) are exceedingly rare and sparsely reported in the literature. A 27-year-old male sustained a closed left subtrochanteric femur fracture following a road traffic accident. Preoperative UTPT was applied with no immediate neurological deficit. UTPT was removed intraoperatively before definitive fixation. Postoperatively, the patient developed left-sided foot drop. Electromyography and nerve conduction velocity studies a

Use real-time near-infrared fluorescence during Heller's cardiomyotomy for achalasia cardia.

Laparoscopic Heller's cardiomyotomy is the surgical procedure of choice in the management of oesophageal achalasia. It is critical to confirm the completeness of the myotomy and mucosal integrity at the conclusion of the procedure. This is conventionally achieved by intraoperative endoscopy and dynamic air leak test. Other modalities that can be used to confirm the myotomy and the integrity of the mucosa at the myotomy site are oesophageal manometry and a methylene blue dye study, respectively. Indocyanine green (ICG) has been in clinical use for more than six decades. The real-time integratio

The Use of Optical Coherence Tomography as an Intraoperative Adjunct of Oculoplastic Surgery.

The advent of integrated intraoperative optical coherence tomography (i2OCT) has opened the door for safer and more complex surgeries in the retina and cornea. However, to limit its use to just two subspecialties within ophthalmology is an opportunity lost for many other subspecialties. Here, we describe the use of i2OCT in oculoplastic surgery. It can be used in cases of severe symblepharon where the cornea is heavily involved, so that the surgeon is aware of the depth of the pannus when trying to remove it. It can also be used when the corneal planes are difficult to identify. In oculoplasti

Lacrimal Intrasaccal Polyp With Inspissated Mucopeptide Concretions.

Lacrimal sac mucopeptide concretions are not uncommon and usually identified following the lacrimal sac marsupialization during a dacryocystorhinostomy. A 39-year-old female presented with epiphora and discharge of 8 months duration and was diagnosed as primary acquired nasolacrimal duct obstruction. During the endoscopic dacryocystorhinostomy, a large intrasaccal polyp arising from the posterosuperior wall of the lacrimal sac with multiple inspissated mucopeptide concretions was noted. The lesion was excised, and the mucopeptide concretions were removed. Histopathology was suggestive of an in

A Case of Successful Coil Embolization for a Late-Onset Type Ia Endoleak after Endovascular Aneurysm Repair with the Chimney Technique.

Juxtarenal aortic aneurysms (JRAAs) are challenging to treat by endovascular aneurysm repair (EVAR) procedures. The chimney technique with EVAR (Ch-EVAR) is one of the feasible and less invasive treatments for JRAAs. However, the main concern of Ch-EVAR is the potential risk of "gutters," which can lead to type Ia endoleak (EL). Most type Ia ELs after Ch-EVAR procedures occurred intraoperatively, and these ELs could be treated using an endovascular technique. However, late-onset type Ia ELs could be extremely rare, which might have a fear of conservative treatment. Type Ia ELs are associated w

Nonrecurrent Laryngeal Nerve and Associated Anatomical Variations: The Art of Prediction.

INTRODUCTION: A nonrecurrent laryngeal nerve (NRLN) is a rare anatomical variation in which the nerve enters the larynx directly off the cervical vagus nerve. CASE REPORT: We present 2 patients who underwent thyroid surgery for benign disease. Intraoperatively, type 2a and 1 NRLN were identified. Due to the frequent association with a vascular abnormality, an ultrasound and a computed tomography were performed which showed a right aberrant subclavian artery with a retroesophageal course and a common trunk of the common carotids in both patients. DISCUSSION AND CONCLUSION: The presence of an

Hypopigmented onychocytic matricoma as a clinical mimic of onychomatricoma: clinical, intraoperative and histopathologic correlations.

Onychocytic matricoma is a newly described matrical tumor of the nail unit that clinically presents with localized thickening of the nail plate and melanonychia and represents a benign acanthoma of onychocytes. Onychocytic matricoma can be classified according to its histopathologic type (acanthotic, papillomatous or keratogenous with retarded maturation) and pigmentation (pigmented, melanocytic or non-pigmented). However, there are no published reports of non-pigmented onychocytic matricoma. We report a case of hypopigmented onychocytic matricoma that presented with a thickened nail plate, xa

Finasteride-associated cataract and intraoperative floppy-iris syndrome.

UNLABELLED: A 47-year-old man who had been using finasteride for male pattern alopecia for 4 years complained of progressive bilateral blurring of vision. His general health had been good, and he was not on any other long-term medication. Examination showed bilateral anterior subcapsular cataracts. Phacoemulsification and insertion of intraocular lenses were performed, and both eyes showed features of intraoperative floppy-iris syndrome (IFIS), including undulation and billowing of the iris, iris prolapse, and pupil constriction. We believe the use of finasteride can be associated with catarac

Echographic evaluation of a patient with diabetes and dense vitreous hemorrhage: an avulsed retinal vessel may mimic a tractional retinal detachment.

PURPOSE: To report that an avulsed retinal vessel may appear as a tractional retinal detachment on echographic evaluation. METHODS: Case report. RESULTS: A 57-year-old diabetic woman presented with a nonclearing vitreous hemorrhage of 2 months duration in the left eye. Echography was consistent with a localized tractional retinal detachment on longitudinal sections; transverse sections demonstrated a pinpoint opacity in the vitreous cavity. Intraoperatively, an avulsed retinal vessel was noted in the area of echographic abnormality. CONCLUSION: An avulsed retinal vessel may mimic tractional

Stroke after adenotonsillectomy in patients with undiagnosed moyamoya syndrome.

IMPORTANCE: Moyamoya syndrome is a rare, occlusive cerebrovascular arteriopathy with significant risk for stroke. Populations that frequently undergo otolaryngologic procedures, including patients with Down syndrome and sickle cell disease, are particularly at risk for moyamoya. The initial presentation of moyamoya syndrome as stroke in the perioperative period of an otolaryngologic procedure has not been reported. OBSERVATIONS: A retrospective medical record review assessed the relationship of otolaryngologic operations and the onset of moyamoya symptoms. Moyamoya syndrome was present in 137

Mirizzi Syndrome With a Double Cystic Duct and a Single Gallbladder: A Case Report on a Rare Anatomical Variant and Surgical Challenge.

While various biliary anomalies have been documented in the literature, their occurrence in clinical practice is uncommon. Common anomalies encountered in practice include variations in cystic duct insertion (such as low or medial insertion), accessory hepatic ducts, and aberrant right hepatic ducts. Less commonly, clinicians may encounter double cystic ducts, duplicated gallbladders, or rare configurations associated with conditions like Mirizzi syndrome. Failure to identify key structures, such as the common hepatic duct (CHD), the common bile duct (CBD), the right hepatic duct (RHD), and th

Serous Borderline Tumor Originating From a Paratubal Cyst in a 22-Year-Old Woman: A Case Report and Review of the Literature.

Serous borderline tumors (SBTs) typically arise from the ovary, and their development within a paratubal cyst is an uncommon occurrence. We report a 22-year-old nulliparous woman with chronic dysmenorrhea and an enlarging right adnexal cyst who underwent laparoscopic bilateral salpingectomy for symptom relief and sterilization. Intraoperatively, a 5 cm right paratubal cyst was identified, with normal uterus and ovaries. Histopathologic examination revealed an SBT confined to the paratubal cyst (International Federation of Gynecology and Obstetrics stage IA (FIGO IA)). This case highlights that

Massive gastrointestinal bleeding due to a Dieulafoy jejunal lesion: a case report.

INTRODUCTION: Jejunal Dieulafoy lesions is a rare cause of gastrointestinal bleeding. Most cases are asymptomatic and require difficult surgical approaches even as endoscopic advancements have become available. CASE PRESENTATION: We describe the case of a 19-year-old female who suffered a massive lower gastrointestinal bleeding, was hemodynamically unstable and needed blood transfusions. We performed an intraoperative enteroscopy which revealed a jejunal Dieulafoy lesion and proceeded to surgically resect and repair it by anastomosis. There were no negative outcomes during the hospitalization

Complications following laser interstitial thermal therapy: a review.

Laser interstitial thermal therapy (LITT) is being performed more frequently for various lesions within neurosurgery, including epileptic foci, vascular malformations, and tumors. Though this technique generally has an excellent safety profile, it is important to be aware of potential complications. Thermal ablation of tissue leads to disruption of the blood brain barrier as well as an inflammatory response both of which cause the majority of complications from LITT. The most common complications of LITT include cerebral edema, focal neurologic deficits, and intracranial hemorrhage. Few studie

Massive hemothorax secondary to internal jugular vein central venous catheter placement in a patient undergoing spinal surgery complicated by chest trauma: a case report.

BACKGROUND: Placement of a central venous catheter (CVC) is a common procedure for spinal surgery and is relatively safe under ultrasound guidance. CASE PRESENTATION: We report the case of a 56-year-old female who underwent ultrasound-guided placement of an internal jugular vein CVC for fluid replacement during spinal surgery for thoracic vertebral burst compression fracture and multiple rib fractures as a result of a high-altitude fall injury. Hemothorax developed intraoperatively. During a thoracotomy, the tip of the CVC was found within the chest cavity. The presence of chest trauma may im

Acute Spinal Hemorrhage from a Cauda Equina Arteriovenous Fistula Fed by the Proximal Radicular Artery.

Spinal cauda equina arteriovenous fistulas (CEAVFs) and spinal hemorrhage from spinal AVF are relatively unusual. To our knowledge, such a case of CEAVF presenting with hemorrhage has not been reported. Here, we describe such a rare case of CEAVF. Spinal magnetic resonance imaging and angiography revealed a CEAVF at the L2 level, fed by the proximal radicular artery and with 2 associated venous varices. Considering the feeder from the proximal radicular artery and the presence of acute hemorrhage, direct surgery was performed. Intraoperatively, spinal hematoma and the fistula were identified a

Inflammation and infarction of adipose tissue: implications in bariatric surgery.

Fat necrosis occurs more frequently in patients who have obesity and diabetes mellitus and is linked to worsening of diabetes. Little evidence is available about surgical complications that are related to inflammation and necrosis of adipose tissue. We report two cases of young women with diabetes who underwent bariatric surgery and had complications resulting from extensive inflammation and necrosis of adipose tissue. The first patient was diagnosed with omental infarction, which is a type of fat necrosis that is rarely associated with obesity and bariatric surgery. The second patient had an

Digital flexion contracture caused by tophaceous gout in flexor tendon.

Gouty tophus is an unusual cause of digital flexion contracture. Awareness of this pathophysiology will lead to more confidence in proper treatment and surgical management of this rare condition. This report describes a case of digital flexion contracture by tophaceous gout distinguished between trigger finger and locking of the metacarpophalangeal joint. We found the flexor tendon with a deposited chalky white substance suggestive of gouty tophus intraoperatively. We performed tenosynovectomy and removed the chalky white substance to the greatest degree possible. Histological findings confirm
