Beneficial effect of fluid warming in elderly patients with bladder cancer undergoing Da Vinci robotic-assisted laparoscopic radical cystectomy.

OBJECTIVES: The enhanced recovery after surgery (ERAS) protocol recommends prevention of intraoperative hypothermia. However, the beneficial effect of maintaining normothermia after radical cystectomy has not been evaluated. This study aimed to investigate the efficacy of fluid warming nursing in elderly patients undergoing Da Vinci robotic-assisted laparoscopic radical cystectomy. METHODS: A total of 108 patients with bladder cancer scheduled to undergo DaVinci robotic-assisted laparoscopic radical cystectomy were recruited and randomly divided into the control group (n=55), which received a

Perioperative complications of orthopedic surgery for lower extremity in patients with cerebral palsy.

Because complications are more common in patients with cerebral palsy (CP), surgeons and anesthesiologists must be aware of perioperative morbidity and be prepared to recognize and treat perioperative complications. This study aimed to determine the incidence of and risk factors for perioperative complications of orthopedic surgery on the lower extremities in patients with CP. We reviewed the medical records of consecutive CP patients undergoing orthopedic surgery. Medical history, anesthesia emergence time, intraoperative body temperature, heart rate, blood pressure, immediate postoperative c

Laparoscopic radical hysterectomy has higher risk of perioperative urologic complication than abdominal radical hysterectomy: a meta-analysis of 38 studies.

OBJECTIVE: A meta-analysis was performed to assess risks of intraoperative and postoperative urologic complications in laparoscopic radical hysterectomy (LRH) and abdominal radical hysterectomy (ARH). METHODS: We searched Pubmed, EMBASE, and Cochrane library for studies published up to December, 2018. Manual searches of related articles and relevant bibliographies of published studies were also performed. Two researchers independently performed data extraction. Inclusion criteria of studies were: (1) had information of perioperative complications, and (2) had at least ten patients per group.

Intraoperative electroencephalogram-derived measures and their association with postoperative delirium: a systematic review and meta-analysis.

BACKGROUND: Postoperative delirium (POD) is a common and serious complication in older surgical populations. Intraoperative EEG may help identify those at risk. Proprietary processed EEG (pEEG) devices were not developed to identify POD risk and studies utilising such devices may confound attempts to identify non-proprietary EEG-derived measures associated with POD. We performed a systematic review of non-proprietary intraoperative EEG-derived measures and their association with POD including a secondary analysis excluding studies that utilised pEEG-guided anaesthesia. METHODS: This review fo

Spilled gall stones during laparoscopic cholecystectomy: a review of the literature.

Laparoscopic cholecystectomy is associated with spillage of gall stones in 5%-40% of procedures, but complications occur very rarely. There are, however, isolated case reports describing a range of complications occurring both at a distance from and near to the subhepatic area. This review looks into the various modes of presentation, ways to minimise spillage, treating the complications, and the legal implications.

[Risk factors for complications of ureterolithotripsy].

AIM: to determine main risk factors for complications of ureterolithotripsy. MATERIALS AND METHODS: a retrospective analysis of the results of 545 ureteroscopies performed in 506 patients with ureteral stones over the past 7 years at the urological clinic named after M.F. Vladimirsky was carried out. The relationship between preoperative and intraoperative factors and complications of ureterolithotripsy was analyzed. RESULTS: The overall complication rate was 22.4%. The risk of intraoperative complications was proved to increase (p<0.05) along with the stone size, location in proximal ureter…

The Melaka Hospital cataract complications study analysis of 12,992 eyes.

INTRODUCTION: As in all surgeries, complications during cataract surgery are unavoidable and cause distress to the surgeon and the patient. This study was conducted to know the factors contributing to complications in our settings, to be able to counsel patients about complications and outcomes and to reduce litigations. MATERIALS AND METHODS: The secondary data analysis was conducted using the National Eye Database from 2007 to 2014. Demographic features, ocular comorbidities, technique of surgery, grade of surgeons, types of intraoperative complications, and reasons for not obtaining good v…

A Comprehensive Framework for the Prediction of Intra-Operative Hypotension.

In this paper, the problem of triggering early warning for intra-operative hypotension (IOH) is addressed. Recent studies on the Hypotension Prediction Index have demonstrated a gap between the results presented during model development and clinical evaluation. Thus, there is a need for better collaboration between data scientists and clinicians who need to agree on a common basis to evaluate those models. In this paper, we propose a comprehensive framework for IOH prediction: to address several issues inherent to the commonly used fixed-time-to-onset approach in the literature, a sliding wind

The usefulness of intraoperative X-ray fluoroscopy in avoiding urethral injury during transanal total mesorectal excision.

INTRODUCTION: Urethral injury is one of the most important complications that can occur during transanal total mesorectal excision in male patients with rectal cancer. This report shows the usefulness of intraoperative X-ray fluoroscopy to avoid urethral injury associated with transanal total mesorectal excision. MATERIALS AND SURGICAL TECHNIQUE: Real-time navigation using fluoroscopy was performed to check the distance between the urethra and the dissection line at the level of the exposed rectourethral muscle, the middle level of the divided rectourethral muscle, and the level at which the

International consensus (ICON) on comprehensive management of the laryngeal nerves risks during thyroid surgery.

The laryngeal monitoring of the inferior and superior laryngeal nerves, and the vagus nerve, has advanced for last years, in practice of thyroid and parathyroid surgery. Different methods are used, using direct or indirect laryngeal stimulation and also intermittent or continuous nerve registration. At present time, various recommendations of it in the world use are reported, and availability of the tool used remains a limit in some countries. The aim of this Icon during Ifos 2017 was to share knowledge about laryngeal intraoperative nerve monitoring (LIONM) procedures and to evaluate current

Intraoperative neuromonitoring during thyroidectomy.

Intraoperative neuromonitoring during thyroidectomy has become a tool available to modern thyroid surgeons. There is ongoing debate as to its effectiveness in reducing intraoperative complications, particularly vocal cord paresis and paralysis. Proponents of its use believe it increases the safety of performing thyroidectomy by improving the ability to identify and preserve the recurrent and superior laryngeal nerves. Some surgeons use it in every case, some only in selected more complicated cases and some choose not to use it at all. There are many reasons why surgeons choose to use or not us…

Palpatory method used to identify the recurrent laryngeal nerve during thyroidectomy.

The major complication of thyroid surgery, occurring in 1% to 6% of cases, is injury to the recurrent laryngeal nerve (RLN). A simple method to identify the RLN during thyroid surgery is described by the authors. It consists in palpation of the nerve caudally to the inferior pole of the thyroid, after the nerve has been made taut by the upward and medial traction of the thyroid gland. This method was used on 47 human cadavers and 45 patients with benign thyroid diseases. It made it possible to identify the RLN in all of the cadavers and 52 of the 55 identifications during 45 thyroidectomies (i

[Positioning trauma in anesthesia and surgical intensive care medicine (2)].

Specific modes of positioning are essential for successful surgery. These are again critically assessed in this final part of our review. Technically correct execution can minimize the risk of damage caused by positioning, although the possibility of damage still exists. First of all, the position on the fracture table is discussed. Great care must be taken concerning the perineal post and leg holder. In the lateral decubitus position, the correct positioning of head and spine as well as that of the lower arm are of great importance. When using the Trendelenburg and reverse Trendelenburg posit
