Massive pseudophakic pigment dispersion associated with an iris nevus.

A 67-year-old woman examined 12 months following extracapsular cataract extraction had a massive pseudophakic pigment dispersion associated with diffuse corneal epithelial edema, mild uveitis, and secondary glaucoma. She underwent penetrating keratoplasty following removal of a posterior chamber intraocular lens (IOL), anterior vitrectomy, capsulectomy, and iris biopsy. Histopathologic examination revealed a pigmented iris nevus and signs of iris erosion by the IOL loop. Because 3 months later the eye developed streptococcal endophthalmitis and had to be eviscerated, we had the opportunity to …

Iris Trabecular Contact on OCT in Angle Closure Progression.

IMPORTANCE: Ocular biometrics measured by anterior segment optical coherence tomography (AS-OCT) predicts progression from primary angle closure suspect (PACS) to primary angle closure (PAC), but obtaining these measurements is time intensive and requires specialized software. Iridotrabecular contact (ITC), a qualitative feature readily visible on AS-OCT images from multiple devices, offers a potentially more accessible method for risk stratification, but the association between ITC and progression to PAC is unclear. OBJECTIVE: To investigate the association of ITC presence and length on AS-O

Stability of Iris pattern corneal tattoo using dermal ink.

PurposeTo assess the long-term stability of the color and pattern of corneal tattooing by dermal ink, designed to mimic the iris of the normal eye.MethodsA retrospective study was performed, recruiting cases with minimum follow-up of 10 years. Patients were examined and subjected to a questionnaire assessing satisfaction. The initial procedure was selected for patients with disfiguring corneal opacity with no hope for vision and a normal fellow eye. The punctures were performed by passing a spatulated needle tip through a drop of colored dermal ink on the corneal surface, targeting the superfi…

Quantitative analysis of iris surface smoothness in normal population.

This study aims to establish normative data for the Smoothness Index (SI) of the iris surface using Anterior Segment Optical Coherence Tomography (AS-OCT) in a healthy population. The study included 198 eyes from 99 subjects, 50 female and 49 males. The average age of participants was 46.71 ± 16.25, spanning 18-75 years. AS-OCT imaging was performed on both eyes before and after pupil dilation. The SI was calculated for various meridians of the iris. Participants were healthy individuals with no underlying iris pathologies, normal intraocular pressure, and a refractive error ranging from spher…

Postoperative Aspergillus Endophthalmitis With Iris Granuloma: A Case Report.

This report describes a rare case of a fungal iris granulomatous lesion in a 67-year-old female who underwent cataract surgery in the left eye and, one month later, developed culture-positive postoperative Aspergillus endophthalmitis. On initial presentation in the emergency room, slit-lamp examination of the left eye revealed subconjunctival hemorrhage, 360-degree subconjunctival hemorrhage, mild corneal edema with Descemet's folds, a deep anterior chamber with a fibrinous reaction causing pupillary membrane formation, and an in situ intraocular lens with no view of the fundus. A bright-scan …

Central vs site outcome adjudication in the IRIS trial.

BACKGROUND: Central adjudication of outcome events is the standard in clinical trial research. We examine the benefit of central adjudication in the Insulin Resistance Intervention after Stroke (IRIS) trial and show how the benefit is influenced by outcome definition and features of the adjudicated events. METHODS: IRIS tested pioglitazone for prevention of stroke and myocardial infarction in patients with a recent transient ischemic attack or ischemic stroke. We compared the hazard ratios for study outcomes classified by site and central adjudication. We repeated the analysis for an updated …

Imaging of iris vasculature: current limitations and future perspective.

Fluorescein and indocyanine green angiography have been the traditional ways to image the vasculature of the iris in the last few decades. Because of the invasive nature of these procedures, they are performed in rare situations, and thus, our understanding about iris vasculature is very limited. Optical coherence tomography angiography (OCTA) is a noninvasive imaging method that enables the detailed visualization of the retinal and choroidal vascular networks. More recently, it has been also used for the examination of the iris vasculature in healthy and disease eyes. However, there is a lack…

[Exchange of aniridia IOLs for customized iris IOL implants].

BACKGROUND: Three groups of iris prostheses can be distinguished for the surgical treatment of iris defects: (1) segmental iris implants, (2) combined iris diaphragm intraocular lenses (IOL) and (3) pure iris implants. Most iris reconstructions are accompanied by aphakia correction with secondary IOL implantation. Although the primary goal is to create a pupil and to improve glare perception, contrast sensitivity and visual acuity, the esthetic result is also a relevant component. OBJECTIVE: Functional and esthetic results after replacement of an aniridia IOL implant with a custom-made artifi…
