Management of keratoconus: comparative visual assessments.

We compared the visual performance of keratoconus patients whose vision had been corrected with one of the following: RGP lenses alone, penetrating keratoplasty, or epikeratoplasty. Despite similar Snellen visual acuities with optimal correction, significant differences in visual performance were found in the three groups. Visual performance was assessed by measuring contrast thresholds for electronically-generated sinusoidal gratings and by measuring contrast thresholds in the presence of a glare source. These measurements were carried out on six bilateral keratoconus patients such that three…

Keratoconus associated with floppy eyelid syndrome.

Floppy eyelid syndrome is a recently described entity, which characteristically involves overweight individuals. The characteristic findings are an upper lid that may be readily everted, tarsal laxity, and diffuse papillary conjunctival changes. The cause of floppy eyelid syndrome is believed to be a mechanical disorder due to the eversion of the lids while sleeping. The cause of keratoconus remains uncertain. There are strong proponents to a mechanical etiology for this disease. The authors report five cases of floppy eyelid syndrome with concomitant keratoconus. One patient with bilateral ke…

Keratoconus-tetany-menopause: the new association.

This report describes the unusual case of 46-year-old white European woman who presented spontaneous keratoconus. This case suggests a new association: keratoconus-tetany-menopause. This can be explained by Thalasselis' syndrome, a syndrome showing a relation between keratoconus, magnesium (Mg) deficiency, type A behavior, and allergy. Furthermore, Thalasselis' syndrome integrates old and new theories. This report also suggests that the alteration induced by severe but reversible Mg deficiency at the intra- and extracellular levels, associated with metabolic and hormonal imbalances, may trigge…

Altered keratan sulfate epitopes in keratoconus.

Extracts of normal and keratoconus human corneas were assayed for keratan sulfate (KS) and for the protein core of corneal keratan sulfate proteoglycan (KSPG), using a solid-phase immunoassay. Aliquots from guanidine-HCl extracts of four to six pooled corneas were adsorbed to nitrocellulose membrane and assayed for binding of a monoclonal antibody against KS or of an affinity-purified rabbit polyclonal antibody against the KSPG protein core using 125I-labeled secondary antibodies. The amount of antigen in the extracts was estimated from standard curves with purified bovine KSPG. Extracts from …

Computer-assisted corneal topography in keratoconus.

Using a computer-assisted video-keratoscope, we determined the topographic findings of 14 eyes of 10 patients with keratoconus. None of these patients had worn contact lenses for at least one year prior to examination and were thus free of contact lens-induced topographical alterations. Two patterns commonly seen included inferior steepening more prominent temporally, and central steepening with a superimposed asymmetric bowtie pattern. When compared with normal eyes three quantitative parameters were significantly different (p less than 0.001): central corneal power, difference in corneal pow…

Histopathologic changes after thermokeratoplasty for keratoconus.

Six patients had penetrating keratoplasty following thermokeratoplasty (TKP) at different time intervals. The standard temperature used was between 90 degree and 95 degree C. with constant saline irrigation. The corneal buttons were studied by light and electron microscopy. Immediate changes observed included epithelial necrosis, marked degneration of keratocytes, and endothelial edema. After 24 hours, evidence of acute inflammation was noted in the subeptihelial zone and superficial stroma, and the endothelial changes persisted. One month after TKP, there was evidence of cellular regeneration

A case of Feingold type 2 syndrome associated with keratoconus refines keratoconus type 7 locus on chromosome 13q.

We report on a 58-year old woman with microcephaly, mild dysmorphic features, bilateral keratoconus, digital abnormalities, short stature and mild cognitive delay. Except for keratoconus, the phenotype was suggestive for Feingold syndrome type 2 (FGLDS2, MIM 614326), a rare autosomal dominant disorder described in six patients worldwide, due to the haploinsufficiency of MIR17HG, a micro RNA encoding gene. Karyotype showed a de novo deletion on chromosome 13q, further defined by array-Comparative Genomic Hybridization (a-CGH) to a 17.2-Mb region. The deletion included MIR17HG, as expected by th…
