Can AI Predict Publication? Multimodal Large Language Models and the Structural Determinants of Surgical Scholarship.

BackgroundWhether artificial intelligence can identify publishable scientific work is untested. We evaluated whether a multimodal large language model (MLLM) could predict, from poster content alone, which abstracts at the American Association for the Surgery of Trauma (AAST) Annual Meetings reached publication, and characterized the investigator, institutional, and domain level determinants situating model performance.MethodsWe retrospectively analyzed 260 abstracts from the 2021-2022 AAST Annual Meetings. Bibliographic searches confirmed publication, and investigator sex, race, and ethnicity

Simulated Reasoning and Self-Verification for Psychiatric Diagnosis in Generalist Large Language Models: Comparative Evaluation.

BACKGROUND: Large language models (LLMs) and, more recently, large reasoning models (LRMs) have rapidly garnered significant interest for application in psychiatry and behavioral health. However, recent studies have identified significant shortcomings and potential risks in the performance of LLM-based systems, complicating their application to psychiatric diagnosis. Two promising approaches to addressing these challenges and improving the efficacy of these models are simulated reasoning (SR) and self-verification (SV), in which additional "reasoning tokens" are used to guide model output, eit…

Large language models decode narrative pathology reports to define clinically relevant subtypes in IgA nephropathy.

Immunoglobulin A nephropathy (IgAN) is the most common primary glomerular disease worldwide, with a highly heterogeneous clinical course. Current biopsy-based risk assessment relies largely on structured histological scores, such as the Oxford MEST-C score, which summarize selected lesions but may omit information contained in routine narrative pathology reports. Here we show that large language model-based analysis of routine biopsy reports identifies reproducible pathological subtypes with prognostic relevance in IgAN. We studied 3,078 adults with primary IgAN from two hospitals in Wenzhou,
