The relationship between bone lead and hemoglobin.

OBJECTIVE: To determine whether the concentration of lead in bone constitutes a biological marker that is more sensitive for chronic toxicity than blood lead levels. DESIGN: Survey. SETTING: A construction trade union with members who engage in carpentry, demolition, and other construction activities. PARTICIPANTS: Members of the construction trade union. MAIN OUTCOME MEASURES: We measured blood pressure, serum creatinine, hematocrit, and hemoglobin. We measured blood lead by anodic stripping voltametry and used a cadmium 109 K x-ray fluorescence instrument to make in vivo measurements of …

Lead foreign body arthropathy in a horse.

A diagnosis of degenerative joint disease secondary to an intra-articular metallic foreign body in the right metacarpophalangeal joint was made in a Quarter Horse gelding. Arthroscopy, performed to evaluate the joint and remove the foreign body, revealed yellow discoloration of the articular cartilage and synovium, and blunting and proliferation of the synovium. The foreign body was identified as a lead sphere. Microscopic examination of synovium revealed chronic synovitis, with accumulation of hemosiderin and multifocal, mild mineralization. Another pigment was evident extracellularly in the …

Chelation therapy in workers with lead exposure.

Occupational lead overexposure remains a major problem. To evaluate the settings in which physicians appropriately prescribe chelation therapy for lead exposure, 7 cases were identified from physician phone calls and mandatory laboratory reporting of elevated blood lead levels to the California Department of Health Services. In the 2 workers with the highest blood lead levels (both of whom had severe symptoms), treatment was indicated. Physicians inappropriately prescribed chelating agents to workers with ongoing lead exposure as prophylaxis against rising blood lead levels and to treat athero…

Systemic lead absorption in human tooth roots.

One-hundred and five impacted and erupted sound teeth were analysed for lead content by graphite furnace atomic absorption spectrophotometry. The results showed that lead accumulated before eruption, presumably soon after the onset of dentine formation. The accumulation in dentine was systemic, without any contribution from the oral environment. The amount of lead in dentine may denote the amount of absorption into the body and serve as a precise index to body burden as no absorption from the mouth can take place. In adults, impacted and erupted teeth can be used together in any research and t…

Safe introducer technique for pacemaker lead implantation.

Over the last several years, an introducer approach for pacemaker lead insertion has evolved that eliminates most introducer-related complications. The approach consists of defining a safe region for intrathoracic cannulation of the subclavian vein. If specific conditions cannot be met for entering the "safe" region or if the vein cannot be found, the subclavian vein is cannulated extrathoracically. Recently, this technique was used in 263 consecutive patients undergoing pacemaker implantation. The intrathoracic portion of the subclavian vein was used in 239 (90.9%) cases and the extrathoracic…

[Neuromuscular diseases which lead to respiratory insufficiency].

Some of the neuromuscular diseases may lead to respiratory failure. This is the case in some lesions of the anterior horn cells (poliomyelitis and spinal muscular atrophy), in diseases affecting the nerve roots (acute polyradiculitis and lesions of the C4-roots) or affecting the phrenic nerve. Respiratory failure is frequent in myasthenia and may accompany Duchenne's muscular dystrophy. In each case one has to evaluate the opportunity of respiratory treatment considering all the elements of the disease and of the individual patient.

Pacing lead fracture after a deceleration injury.

Acute pacemaker malfunction is potentially lethal. A high index of suspicion must be maintained in physicians caring for traumatized patients utilizing permanent pacing systems. We present a case of transvenous pacemaker lead fracture sustained in a vehicular deceleration injury.

Body burdens of lead in hypertensive nephropathy.

Chronic lead exposure resulting in blood lead concentrations that exceed 1.93 mumol/l (40 micrograms/dl) or chelatable urinary lead excretion greater than 3.14 mumol (650 micrograms) per 72 h has been associated with renal disease. A previous study had found greater chelatable urine lead excretion in subjects with hypertension and renal failure than in controls with renal failure due to other causes, although mean blood lead concentrations averaged 0.92 mumol/l (19 micrograms/dl). To determine if chelatable urinary lead, blood lead, or the hematologic effect of lead (zinc protoporphyrin) were …

Central nervous system intoxications other than lead.

Other than lead, organophosphates and carbamates are probably the most important classes of toxicants affecting the central nervous system in food animals. Chlorinated hydrocarbons continue to present problems from time to time, as do certain poisonous plants. Less frequently encountered substances such as nitrofurans, certain rodenticides, herbicides, and other miscellaneous poisons are also discussed.

Lead toxicity masquerading as sickle cell crisis.

We recently saw a 12-year-old black boy with known sickle cell disease who had been seen many times for abdominal pain thought to be secondary to a vasoocclusive crisis. The patient eventually was admitted, after a seizure and the onset of obtundation. The etiology of his acute encephalopathy remained unclear until bone films of his knees fortuitously revealed "lead lines." The patient was treated and did well subsequently. This case emphasizes the importance of considering other diagnoses when a sickle cell patient presents with a crisis.

Acute lead intoxication due to intravenous injection.

A case of acute lead poisoning due to intravenous injection of lead acetate is reported. The patient developed clinical and biochemical symptoms characteristic for acute hepatic porphyrias. Elevated urinary 5-aminolaevulinic acid and low porphobilinogen correlated to a lead-induced inhibition of 5-amino-laevulinic acid dehydrase with diagnostically indicative reactivation rates by zinc and dithiothreitol. Urinary coproporphyrin excretion was also increased. Additional findings included anaemia and toxic hepatitis. Under the influence of elimination therapy with D-penicillamine pathologic param…

Lead poisoning among children in Saudi Arabia.

Six Saudi children aged between 10 and 30 months were found to have lead poisoning. Two of them presented with encephalopathy. Twenty children with unexplained convulsions, some of whom died, were reported to have been admitted to the local district hospital recently. All our cases were diagnosed within 2 weeks of diagnosing the index case. The source of lead was found to be a locally used teething powder known as 'Saoott' and 'Cebagin'. This was prescribed by a traditional healer. The lead content of this powder was found to be 51%. Chemical analysis of other locally used eye cosmetics and te…

Organic affective illness associated with lead intoxication.

Psychiatrists treating patients with depression or nonspecific somatic complaints seldom think of lead intoxication as a possible cause. Because occupational exposure to lead is so common, these disturbances may often be associated with lead intoxication. To facilitate earlier clinical recognition and proper treatment among the many individuals at risk, the authors describe four cases of organic affective disturbance associated with lead intoxication, review the neuropsychiatric disturbances that have been reported with chronic exposure to lead, and report the results of their experience evalu…
