BACKGROUND: Left atrial appendage (LAA) closure (LAAC) is plagued by late peri-device leak. We posited that appendage contraction prevents full LAA seal, so conversely, LAA stunning with pulsed field ablation (PFA) of the LAA (left atrial appendage ablation [LAAA]) might promote rapid appendage thrombosis/obliteration.
OBJECTIVES: This study aims to assess the impact of LAAA during concomitant procedures on eliminating leaks.
METHODS: At 3 centers, consecutive patients undergoing concomitant PFA and LAAC also underwent LAAA or did not undergo LAAA, at operator discretion. Initial (∼3-4 month

INTRODUCTION: Surgical endocardial closure of the left atrial appendage (LAA) may reduce thromboembolic risk in patients with atrial fibrillation (AF); however, evidence regarding its medium-to-long-term durability remains limited. This study evaluated the efficacy and safety of endocardial suture closure of the LAA during concomitant cardiac surgery.
METHODS: This retrospective study included 50 patients with valvular heart disease and AF who underwent valve surgery, the Cox-Maze IV procedure, and LAA closure using an endocardial suture technique between 2013 and 2018. Intraoperative transes

BACKGROUND: Same-day discharge (SDD) after left atrial appendage closure (LAAC) may reduce hospitalization burden, but concerns regarding early readmission after discharge remain.
METHODS: We performed a retrospective cohort study using the Nationwide Readmissions Database from 2016 through 2020. Adult patients undergoing inpatient LAAC were identified using International Classification of Disease-10 procedure code 02L73DK. The analytic cohort was restricted to length of stay (LOS) 0 or 1 day; SDD was defined as LOS 0 and overnight stay as LOS 1. Survey-weighted modified Poisson regression wa…

BACKGROUND: Device-related thrombus (DRT) after left atrial appendage closure can occur.
OBJECTIVE: This study aimed to evaluate late/very-late DRT after an institutional early "all-off" strategy guided by complete sealing (peridevice leak [PDL] 0).
METHODS: We analyzed 106 consecutive successful left atrial appendage closure procedures; 103 patients had evaluable imaging at 45 days (≥45 days). Per protocol, antithrombotic agents were discontinued at 45 days and/or 6 months if imaging showed PDL of 0 and no DRT. Landmark analyses in patients free of 45-day DRT (n = 101) compared late/very-la…

INTRODUCTION: Atrial fibrillation-related stroke is a leading cause of morbidity and mortality. The comparative effectiveness and safety of left atrial appendage closure (LAAC) devices, compared with one another and with anticoagulation, is unclear.
METHODS: We conducted a systematic review and network meta-analysis (NMA) of all clinical trials comparing the Watchman and Amplatzer Amulet LAAC devices with each other or with warfarin or direct oral anticoagulants (DOACs). The primary comparison was between LAAC devices with secondary comparisons to anticoagulation. The primary effectiveness ou…

Atrial fibrillation commonly coexists with severe mitral regurgitation, complicating management by necessitating long-term anticoagulation, with its associated bleeding risk. The combined approach of percutaneous left atrial appendage closure and mitral transcatheter edge-to-edge repair has been proposed as a comprehensive solution, mitigating stroke risk while addressing mitral regurgitation. This approach eliminates the need for a second procedure and lifelong oral anticoagulants. Our review provides insights into the benefits and risks of combined procedures, potentially aiding clinicians i…

CONTEXT: Left atrial appendage closure (LAAC) was developed as a novel stroke prevention alternative for patients with atrial fibrillation, particularly for those not suitable for long-term oral anticoagulant therapy. Traditionally, general anesthesia (GA) has been more commonly used primarily due to the necessity of transesophageal echocardiography.
AIMS: Compare trends of monitored anesthesia care (MAC) versus GA for percutaneous transcatheter LAAC with endocardial implant and assess for independent variables associated with primary anesthetic choice.
SETTINGS AND DESIGN: Multi-institution…

Contrast media are generally necessary for transcatheter left atrial appendage closure (LAAC), however, it should be avoided in patients with chronic kidney disease (CKD). The objective of this study was to evaluate the safety and feasibility of contrast-free LAAC with WATCHMAN FLX device for patients with CKD. Among 141 patients undergoing LAAC using the WATCHMAN FLX between May 2021 and March 2023, we performed LAAC without contrast media in 10 patients. Procedural and follow-up results were evaluated. The device size was selected based on the transesophageal echocardiographic (TEE) measurem…

BACKGROUND: Left atrial appendage (LAA) occlusion provides an alternative to oral anticoagulation for thromboembolic risk reduction in patients with nonvalvular atrial fibrillation. Since regulatory approval in 2015, the WATCHMAN device has been the only LAA closure device available for clinical use in the United States. The PINNACLE FLX study (Protection Against Embolism for Nonvalvular AF Patients: Investigational Device Evaluation of the Watchman FLX LAA Closure Technology) evaluated the safety and effectiveness of the next-generation WATCHMAN FLX LAA closure device in patients with nonvalv

BACKGROUND: Percutaneous left atrial appendage (LAA) closure is an alternative to oral anticoagulation (OAC) for atrial fibrillation (AF) patients with high thromboembolism risk, particularly with contraindications to OAC. The LAA itself could possess proarrhythmogenic properties. As patients undergoing LAA closure could be candidates for cardioversion or ablation, we aimed to evaluate AF disease progression following LAA closure and the outcome of patients undergoing a rhythm control strategy after the procedure.
METHODS: The prospective multicenter French Nationwide Observational LAA Closur

BACKGROUND: Left atrial appendage closure (LAAC) is an efficient alternative of oral anticoagulation to prevent stroke in patients with non-valvular atrial fibrillation (NVAF). Due to complexities of LAA anatomy, a complete closure may not always be obtained with a single device. The aim of this study was to evaluate the feasibility and safety of the staged 'kissing-Watchman' technology to occlude the LAA with complex anatomy.
METHODS: In our center, among 300 cases underwent LAAC with Watchman device from February 2012 to December 2016, 7 complex LAAs were implanted double devices using the …

Transcatheter closure of the left atrial appendage (LAA) is increasingly considered as an alternative to oral anticoagulation in patients with previous major bleeding or at high-risk of bleeding. Device embolization with transcatheter LAA closure is a rare complication. Most cases are asymptomatic, but it can be life threatening. Depending on the location of embolization, percutaneous retrieval is feasible. Snares are usually used for retrieval, but other devices may be used. We report the case of a 63-year gentleman who underwent an uneventful LAA closure with Watchman device and routine test

Background/Objectives: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and is associated with ischemic stroke, systemic thromboembolism, heart failure, and mortality. Although oral anticoagulation (OAC) remains the cornerstone of stroke prevention, long-term therapy may be limited by bleeding risk, contraindications, intolerance, nonadherence, or thromboembolic events despite treatment. Left atrial appendage closure (LAAC) has therefore emerged as a non-pharmacological alternative for selected patients with nonvalvular AF. This review aimed to synthesize current eviden…

Non-valvular atrial fibrillation (NVAF) is the most common cardiac arrhythmia in the general population, and its prevalence increases among patients with chronic kidney disease (CKD) undergoing hemodialysis. This population presents high risk of both hemorrhagic and thrombotic events, with little evidence regarding the use of oral anticoagulation treatment (OAT) and multiple complications arising from it; however, stroke prevention with percutaneous left atrial appendage closure (LAAC) is an alternative to be considered. We retrospectively describe the safety and efficacy of percutaneous LAAC …
