Readily Identifiable Risk Factors of Nursing Home Residents' Oral Hygiene: Dementia, Hospice, and Length of Stay.

BACKGROUND/OBJECTIVES: The poor oral hygiene of nursing home (NH) residents is a matter of increasing concern, especially because of its relationship with pneumonia and other health events. Because details and related risk factors in this area are scant and providers need to be able to easily identify those residents at most risk, this study comprehensively examined the plaque, gingival, and denture status of NH residents, as well as readily available correlates of those indicators of oral hygiene, including items from the Minimum Data Set (MDS). DESIGN: Oral hygiene assessment and chart abst

Analysis of Emergency Department Length of Stay for Mental Health Patients at Ten Massachusetts Emergency Departments.

STUDY OBJECTIVE: Prolonged boarding times in the emergency department (ED) disproportionately affect mental health patients, resulting in patient and provider dissatisfaction and increased patient morbidity and mortality. Our objective is to quantify the burden of mental health boarding and to elucidate the effect of insurance together with demographic, social, and comorbid factors on length of stay. METHODS: We conducted a cross-sectional observational study of 871 consecutive patients requiring an ED mental health evaluation at one of 10 unaffiliated Massachusetts hospitals. Demographics; i…

Measuring the Impact of Whole-Body Computed Tomography on Hospital Length of Stay in Blunt Trauma.

RATIONALE AND OBJECTIVES: Whole-body computed tomography (WBCT) imaging has become commonplace in some emergency departments (EDs) for trauma where management is dependent on rapid diagnosis achieved through comprehensive imaging. The purpose of this study was to assess the value that computed tomography (CT) imaging contributes to trauma patients by retrospectively comparing hospital length of stay (LOS) between WBCT and selective CT imaging, while controlling for hemodynamic stability and socio-economic considerations. MATERIALS AND METHODS: This study was institutional review board approve…

Proportion of neonatal readmission attributed to length of stay for childbirth: a population-based cohort study.

OBJECTIVE: Most literature on length of stay (LOS) for childbirth focuses on 'early' discharge as opposed to 'optimal' time of discharge and has conflicting results due to heterogeneous definitions of 'early' discharge and differing eligibility criteria for these programmes. We aimed to determine the LOS associated with the lowest neonatal readmission rate following childbirth by examining the incidence pattern of neonatal readmission for different LOS using the Kitagawa decomposition. DESIGN: Retrospective cohort study using administrative hospitalisation data. SETTING: Canada (excluding Qu…

Predictors of Inadequate Inpatient Colonoscopy Preparation and Its Association with Hospital Length of Stay and Costs.

BACKGROUND/AIMS: Adequate bowel preparation is essential to safe and effective inpatient colonoscopy. Predictors of poor inpatient colonoscopy preparation and the economic impacts of inadequate inpatient preparations are not defined. The aims of this study were to (1) determine risk factors for inadequate inpatient bowel preparations, and (2) examine the association between inadequate inpatient bowel preparation and hospital length of stay (LOS) and costs. METHODS: We performed a retrospective cohort study of adult patients undergoing inpatient colonoscopy preparation over 12 months (1/1/2013…

Timing of Femoral Shaft Fracture Fixation Affects Length of Hospital Stay in Patients with Multiple Injuries.

PURPOSE: Appropriate timing of definitive fracture care in the setting of polytrauma remains controversial. The aim of this study is to dete … een 24 and 48 hours, (4) between 48 and 120 hours, and (5) more than 120 hours. Because we consider length of stay a surrogate for adverse outcome causally affected by treatment time, the outcome variable was calculated as the duration of hospitalization following definitive treatment. Time to definitive fixation and its effect on post-treatment length of hospit …

Impact of respiratory complications on length of stay and hospital costs in acute cervical spine injury.

CONTEXT: Respiratory complications are frequent in patients with acute cervical spinal injury (CSI); however, the importance of respiratory complications experienced during the initial hospitalization following injury is unknown. OBJECTIVE: To determine if respiratory complications experienced during the initial acute-care hospitalization in patients with acute traumatic cervical spinal injury (CSI) are more important determinants of the length of stay (LOS) and total hospital costs than level of injury. DESIGN: A retrospective analysis of an inception cohort for the 5-year period from 1993 …

Critical pathway and hospital-hospital cooperation in acute stroke. Reduction of the length of hospital stay.

Stroke patients tend to stay longer in one hospital compared to patients with other neurological disease. After the introduction of 3 types of critical pathway dedicated for various severity of acute ischemic stroke in 1995, the average length of inhospital days declined from 30.0 days (1993) to 15.3 days (1998), ie 49% reduction. This reduction was achieved by the use of critical pathway and the hospital-hospital cooperation.

Effect of nutrition status and other factors on length of hospital stay after major gastrointestinal surgery.

Various factors may prolong postoperative recovery and the length of stay (LOS) in the hospital. In a retrospective study of 245 adult patients who had no oral intake for 5 days after major gastrointestinal surgery, we used a correlation matrix to describe the population and determine the effects of the following factors on LOS: malnutrition, complication status, stress level, type of surgery, pathology, period of inadequate nutrient intake, and use of nutritional support. LOS was markedly prolonged in malnourished patients compared with those who were not (23.5 +/- 16.5 vs. 16.5 +/- 10.7 days…

Cold Ischemia Time Is an Important Risk Factor for Prolonged Length of Stay Post-Lung Transplantation.

PURPOSE: In lung transplantation (LTx), optimal cold ischemia time (CIT) is considered <6 h, but is often extended to maximize allograft utilization. Given the limited analysis of how variation in CIT impacts outcomes, we examined its correlation with graft survival, patient mortality, and prolonged length of stay (PLOS), defined as ≥30 days. METHODS: Using the United Network for Organ Sharing database, we retrospectively analyzed 39 806 adult LTx between May 4, 2005, and December 1, 2025. Ex-vivo lung perfusion (EVLP) cases (n = 1378) were isolated for separate analysis. The remaining 39 161…

Factors Associated With Intensive Care Unit Length of Stay After Acute Arterial Ischemic Stroke in Children.

BACKGROUND: Identifying features of children with acute arterial ischemic stroke (AIS) who require only a short intensive care unit (ICU) stay to assure stability allows for optimal use of health care resources. We aimed to understand characteristics of children who received ≤1 night of ICU care after AIS. METHODS: This retrospective observational, cross-sectional single-center study identified children (age 29 days-18 years) with AIS confirmed via neurology evaluation and neuroimaging, 2012-2024, from an institutional pediatric "code stroke" quality improvement database.
