Use of a levonorgestrel-releasing intrauterine device for menorrhagia treatment during adjuvant therapy of adrenocortical carcinoma with mitotane.

Adrenocortical carcinoma is a rare tumour with high malignancy and poor prognosis. This tumour is rarely diagnosed in the reproductive age. Complete surgical resection is the only curative treatment for adrenal cancer in all stages. After surgery adjuvant chemotherapy is required. Mitotane is the most important drug in adrenal cancer chemotherapy. Mitotane's mode of action is not entirely explained. Animal studies have shown that the substance exerts a direct cytotoxic effect on the cells of the adrenal cortex. This activity is selective, progressive and affects only the zona reticularis and f

Different bleeding patterns with the use of levonorgestrel intrauterine system: are they associated with changes in uterine artery blood flow?

OBJECTIVE: Evaluate if different bleeding patterns associated with the use of the levonorgestrel intrauterine system (LNG-IUS) are associated with different uterine and endometrial vascularization patterns, as evidenced by ultrasound power Doppler analysis. METHODOLOGY: A longitudinal study, with each subject acting as its own control was conducted between January 2010 and December 2012. Healthy volunteers with a history of heavy but cyclic and regular menstrual cycles were enrolled in the study. Ultrasonographic examination was performed before and after six months of LNG-IUS placement: uter…

Levonorgestrel intrauterine system as a treatment option for severe menorrhagia in adolescent with type III von Willebrand disease.

The authors describe a case of an adolescent with type III von Willebrand disease and severe menorrhagia since menarche. Antifibrinolytic, hormonal (estroprogestative pill in high doses, etonogestrel implant and gonadotropin-releasing hormone agonist goserelin) and Von Willebrand Factor/Factor VIII replacement therapies were prescribed to the patient, but symptomatic control was only obtained with high doses of VWF/FVIII twice a week. In March 2012, a levonorgestrel intrauterine system was inserted in a 14-year-old. At present, the patient is asymptomatic without regular prophylaxis (VWF/FVIII…

The levonorgestrel intrauterine system (Mirena) for the treatment of menstrual problems in adolescents with medical disorders, or physical or learning disabilities.

Menstrual problems are common in adolescence. Management can be very problematic in girls with disability or medical disorders, as their condition and other medications may mean they are less likely to achieve satisfactory control with standard medical treatments. Those with disability tend to tolerate menses poorly. We report experience with the levonorgestrel intrauterine system (LNG IUS) for treatment of menstrual problems in adolescents with a range of medical problems or with severe learning disability. It provided of significant therapeutic benefit for 12 of 14 participants, with the maj…

Use of the levonorgestrel-IUD in the treatment of menorrhagia: improving patient outcomes while reducing the need for surgical management.

Menorrhagia impairs the quality of life and often leads to hysterectomy. Although hysterectomy provides definitive management of menorrhagia, the procedure is associated with morbidity, permanent sterility, and high cost. Alternative treatments, including pharmaceuticals, intrauterine devices, and other minor surgical procedures are less invasive and less expensive. In particular, the levonorgestrel-releasing intrauterine system (LNG-IUS), a nonsurgical, reversible treatment, provides effective management for many women with menorrhagia while minimizing morbidity and health care costs.

Patient satisfaction and side effects with levonorgestrel implant (Norplant) use in adolescents 18 years of age or younger.

OBJECTIVES: Levonorgestrel implants (Norplant) have recently been approved for use in the United States. Adolescents may be especially suited for this method of contraception because of its long duration of action and lack of dependence on patient compliance for efficacy. To date, however, its use in young patients has not been investigated. This study was undertaken to compare side effects and satisfaction with this new method during the first 6 months of use among adolescents and adults. METHODS: Twenty-one adolescents 18 years of age or younger and 30 adults 19 years or older participated …

Morphological, histopathological, and transcriptomic analyses of gonads reveal reproductive impacts of levonorgestrel (LNG) exposure on Gambusia affinis.

Levonorgestrel (LNG), a synthetic progestin, is extensively utilized in contraceptives and hormone replacement therapies in daily medical practice. To date, LNG has been frequently detected in various water bodies worldwide, posing significant threats to aquatic organisms, particularly fish. Its presence disrupts the delicate hormonal balance and gonadal development pathways in fish, leading to adverse effects on their reproductive health and potentially altering population dynamics. In this study, adult female mosquitofish were chronically exposed to LNG at four distinct concentration levels …
