Local Responses to Limits on U.S. Public Health Authority During the COVID-19 Emergency.

Public health has become politicized in the U.S. Though research shows that limiting public health authority during emergency response puts … actions (limits on the emergency authority of governor, chief executive, state health officer, and local government health officers). Participants discussed barriers and challenges faced, adaptations made, actions taken, and perspectives for the future. Emergent themes suggest that while limited resources and restricted public health authority caused strain, publ …

Revenue-feedback and the institutional governance of global risky commons.

The mitigation of the adverse effects of global warming poses the most pressing challenge concerning the sustainable governance of global risky commons (GRC)-global commons under risk of collapse. The collective risk dilemma (CRD) provides a convenient game-theoretical model to address key features of GRC, by framing the problem as one of cooperation. Here we employ the CRD to assess the relative effectiveness of both local, bottom-up and global, top-down (inspired) institutional designs and policies to solve GRC. To this end, we set up a framework in which institutions (local and global) are

Hybrid SARS-CoV-2 immunity among frontline workers in a high-tourism setting: A community-based serosurvey in Ambergris Caye, Belize, June 2022.

Tourism is central to Belize's economy, yet the influx of international travelers may contribute to infectious disease introduction and transmission among local workers, including SARS-CoV-2. In June 2022, we conducted a cross-sectional seroepidemiologic study among tourism and government workers in the tropical island tourism center of Ambergris Caye, Belize. The goal of this study was to determine SARS-CoV-2 antibody prevalence for both vaccination (anti-spike protein) and natural infection (anti-nucleocapsid protein), vaccine uptake, history of COVID-like symptoms, and risks for infection t

Perceived implications of COVID-19 policy measures on food insecurity among urban residents in Blantyre Malawi.

BACKGROUND: Malawi is at the brink of experiencing food insecurity amidst the COVID-19 pandemic as the vast majority of its population lives in extreme poverty. While measures are being implemented to avert the spread of COVID-19, little is known about how COVID-19 policy measures have impacted food insecurity in urban Malawi. This study addresses this gap by exploring the implications of COVID-19 policy measures on food insecurity in low-income areas of Blantyre in Malawi. METHODS: We used Bronfenbrenner's ecological theory to explore the implications of COVID-19 policy measures on peoples'

Artificial Intelligence Should Not Prescribe Alone: A Risk-Stratified Pharmacy Governance Framework for Agentic Clinical Systems.

Agentic clinical artificial intelligence (AI) is moving beyond conversational assistance towards systems capable of generating medication-related recommendations and structured clinical actions. Although recent models have shown strong performance in simulated disease-management and medication-reasoning tasks, benchmark performance does not establish the safety of autonomous prescribing. Medication use depends on patient-specific information, local formularies, medicine availability, monitoring capacity, clinical workflows, and professional accountability. This editorial argues that medication

Strengthening district health systems to achieve Malawi's decentralisation objectives: lessons learnt from Malawi's Blantyre Prevention Strategy.

The Alma-Ata 'Health for All' Declaration of 1978 emphasised community participation in health systems and the need to 'bring health care as close as possible to where people live and work'. Since then, decentralised approaches that leverage the proximity of local governments to their populations have emerged as the preferred model for health systems over fully centralised structures. The district health system, particularly in low- and middle-income countries, has evolved as a key framework to advance the Alma-Ata vision and strengthen health system governance in decentralised settings. Howev…

Better & systematic trauma care (BEST) in Norway: from a local quality improvement project to a national collaborative.

PURPOSE: Healthcare professionals in Norwegian local hospitals have focused on the need for improved trauma care since the late 1990s. Because of lack of national guidelines the hospital management of patients with serious trauma varied. The authors and colleague health care providers felt a need for improvement. Therefore, a project called Better & Systematic Trauma care (BEST) was established. The goal was to improve trauma care through training and preparation for infrequently occurring situations, with a focus on the team. METHODS: The project began as a bottom-up initiative in four local

Outcomes of upper tract urothelial cancer managed non-surgically.

INTRODUCTION: Approximately 7% of patients with localized upper tract urothelial cancer (UTUC) are treated without definitive therapy. Understanding outcomes and alternative therapy would aid in counseling older patients with comorbidities. MATERIALS AND METHODS: We utilized the National Cancer Database to identify patients with localized UTUC managed non-surgically between 2004 and 2013. Patient demographics, comorbidity, tumor grade, and chemotherapy and radiation utilization were recorded. Survival analyses were performed with the Kaplan-Meier method and a cox proportional hazard regressio

Sources of stress in impoverished neighbourhoods: insights into links between neighbourhood environments and health.

OBJECTIVE: This paper explores associations between residents' perceptions of social incivilities and physical disorders in local environments and self-reported health status. METHOD: Surveys were conducted with 4,029 residents from 13 Neighbourhood Renewal sites and 1,857 residents of corresponding Local Government Areas in Victoria. An open-ended question asked respondents to nominate the worst things about living in their neighbourhood and this qualitative data was analysed for the range of perceptions of incivilities. Quantitative data analysis considered associations between incivilities…

Insights on food and nutrition in the Federated States of Micronesia: a review of the literature.

BACKGROUND: Nutrition-related disorders, including vitamin A deficiency (VAD) and chronic diseases, are serious problems in the Federated States of Micronesia (FSM). Many suggest that these disorders are new problems related to dietary and lifestyle changes. In the past four decades, imported foods, such as white rice, flour, sugar, refined foods and fatty meats, have increasingly replaced local foods in the diet. AIM: A literature review was conducted to understand underlying issues related to dietary change and obtain insights for nutrition research and interventions. METHOD: Published and

Why responsible AI needs regional networks in low-resource health systems.

Responsible clinical AI requires regionally anchored networks tailored to local contexts. We propose a primary-care-first approach to prioritize local governance, ensuring decisions are context-driven; data sovereignty, to build trust and protect privacy; and community-driven infrastructure, ensuring sustainability. With capacity building, these networks support equitable AI by aligning with local needs, overcoming barriers to implementation, and enabling collaboration across regions to scale solutions effectively.

Mechanism of action of extradural analgesia.

The factors governing the spread and site of action of local anaesthetics in the extradural space are discussed. Local anaesthetic drugs injected into the extradural space may cause nerve blocks at three possible sites: (a) On the spinal nerves in the paravertebral space. This probably occurs only in young subjects. (b) On the spinal nerves intradurally. This is probably the essential site of blockade. (c) On the spinal cord.

Strengthening mental health care in Somalia through artificial intelligence: opportunities, challenges, and a roadmap for implementation.

Mental health needs in Somalia are substantial, yet access to timely, high-quality care remains constrained by workforce shortages, limited infrastructure and financing, fragmented services, weak health information systems, and persistent sociocultural barriers. Artificial intelligence (AI) may complement mental health system strengthening in this fragile and resource-constrained setting. This Perspective examines potential applications of AI across Somalia's mental health system and considers the associated implementation, ethical, governance, and equity challenges. Potential applications inc
