Embolisation of Contraceptive Implants to the Pulmonary Arterial System: A Series of Three Cases from a Tertiary Thoracic Surgery Unit.

Contraceptive implants are widely used for long-acting reversible contraception (LARC) due to their high efficacy and convenience. However, complications including migration and, rarely, embolisation to the pulmonary arterial system have been reported. This case series presents three cases of contraceptive implant embolisation to the pulmonary arterial system, managed at a tertiary thoracic surgery unit between 2021 and 2024. Different surgical management was performed in all three cases influenced by factors including: length of time since possible embolisation, implant location, and suspecte…

A five-year cost-consequence analysis of extended-use etonogestrel implant versus other contraceptives.

OBJECTIVE: While multiple reversible contraceptives are available, long-acting reversible contraception (LARC) offers multi-year protection with minimal user burden. The etonogestrel implant is the most effective LARC but carries higher upfront acquisition costs. This analysis evaluated whether the acquisition costs for the etonogestrel implant are offset via pregnancy prevention and pregnancy-related expenditures. METHODS: A discrete Markov chain model with 5-year time horizon simulated pregnancy outcomes among 1,000 women 18-49 years initiating 1 of 8 hormonal contraceptive methods: branded…

A qualitative study on reasons for early removal of Implanon among users in Arba Minch town, Gamo Goffa zone, South Ethiopia: a phenomenological approach.

BACKGROUND: Implanon is one of the cost - effective long acting reversible contraceptive methods used for spacing and limiting births in Ethiopia. Despite the scaling up initiative undertaken by the Ethiopian Government, Implanon uptake is very low compared to short acting contraceptive methods. There is low utilization of Implanon with high level of discontinuation in Ethiopia. Therefore, this study was conducted to explore the reasons for early removal of Implanon among users in Arba Minch town, South Ethiopia. METHODS: A community-based qualitative exploratory study using phenomenological

The history of tiered-effectiveness contraceptive counseling and the importance of patient-centered family planning care.

Public health workers, clinicians, and researchers have tried to increase long-acting reversible contraceptive (LARC) use by changing contraceptive counseling between patients and providers. Several major health organizations now recommend tiered-effectiveness counseling, in which the most effective methods are explained first so that patients can use information about the relative efficacy of contraceptive methods to make an informed choice. Some scholars and practitioners have raised concerns that, given histories of inequitable treatment and coercion in reproductive health care, tiered-effe

Contraception in sea-going service personnel.

The right to make an informed choice about contraception should be afforded to every individual serving within the United Kingdom (UK) Armed … pliance and any issues or comments documented accordingly. In December 2010, a discussion regarding long-acting reversible contraception (LARC) was documented for the first time. The patient agreed to give LARC some thought and a review appointment was made for one month. She was subsequently started on the progestogen-only pill Cerazette™. It was noted by the consulting doctor that …

Etonogestrel-releasing contraceptive implant in a patient using thalidomide for the treatment of erythema nodosum leprosum: a case report.

INTRODUCTION: Thalidomide is an immunomodulatory drug and first choice in the treatment of erythema nodosum leprosum. Given its teratogenic potential, it is essential that an effective contraceptive method is used, especially a long-acting reversible contraceptive (LARC) method. The subdermal etonogestrel (ENG)-releasing implant is an adequate method due to the high effectiveness and long-term use. However, interaction between thalidomide and ENG has not been well documented. Concern arises because thalidomide interacts with cytochrome P450 (CYP450) enzymes that metabolize sexual steroids. AIM

Assessing chromatic discrimination in women on hormonal contraceptives.

We explored whether chromatic discrimination is affected in women using hormonal contraceptives (HCs). Leaning upon earlier studies of oral contraceptives (OCs), we reasoned that estrogen and/or progestin, HC components, would reveal mild tritan-like defects in HC users. A sample of 68 women (18-45 years) was stratified by the HC type and use history: (a) non-users with past HC use (PC=21), (b) never used (NC=16), (c) oral contraceptives (OCs=23), and (d) long-acting reversible contraceptives (LARCs=8). Employed was the Cambridge Colour Test (CCT), along with screening using the D-15 and D-15d

Management of abnormal bleeding in etonogestrel-releasing implant users: quo vadis? An exploratory study on the attitudes of a cohort of Italian gynaecologists and review of the literature.

INTRODUCTION: Long-acting reversible contraceptives (LARCs) are increasingly gaining popularity due to their high efficacy and ease of use compared to short-acting reversible contraceptives (SARCs). Among available LARCs, the etonogestrel (ENG) subdermal implant represents a particularly effective contraceptive method. However, many patients request early removal of the device due to the persistence of unfavourable bleeding patterns. METHODS: We developed a short, multiple-choice ad hoc questionnaire distributed online from October 2024 to March 2025 to investigate the attitude of Italian gyn

Is LARC for Everyone? A Qualitative Study of Sociocultural Perceptions of Family Planning and Contraception Among Refugees in Ethiopia.

Objective Ethiopia is home to an increasingly large refugee population. Reproductive health care is a critical issue for these groups because refugee women are at high risk for unmet family planning needs. Efforts to expand contraceptive use, particularly long acting reversible contraceptive (LARC) methods are currently underway in several Ethiopian refugee camps. Despite availability of LARC methods, few refugee women opt to use them. The purpose of this study was to explore how culture influences contraceptive attitudes and behaviors, particularly towards LARC methods, among Ethiopia's refug

Recent experience with and future directions of contraceptive implants and injectable contraceptives.

Subdermal implantable contraception has been the most important addition to contraceptive technology in the past 2 decades. The Norplant system is only the first of a new family of long acting methods of high effectiveness, high acceptability, and prompt reversibility. The main advantage of the second generation implant systems, to be available soon, is the reduction in the number of implants, which greatly facilitates insertion and removal. However, great improvement in bleeding control is not to be expected. Large and carefully designed epidemiological studies have contributed to clear the m

Examining the pace of change in contraceptive practices in abortion services - a follow-up case study of a quality improvement collaborative.

BACKGROUND: Among all women who experienced an abortion in Sweden 2017, 45% had previously underwent at least one abortion. This phenomenon of increasing rates of repeat abortions stimulated efforts to improve contraceptive services through a Quality Improvement Collaborative (QIC) with user involvement. The participating teams had difficulty in coordinating access post-abortion to the most effective contraception, Long-acting reversible contraception (LARC), during the eight-month QIC. This prompted questions about the pace of change in contraceptive services post-abortion. The aim of the stu…
