[Congenital pulmonary lymphangiectasis].

Congenital pulmonary lymphangiectasis is a rare cause of severe respiratory distress in the newborn period and most of these patients die, usually within the first 24 hours of life. It may present as an isolated anomaly or it may be associated with other congenital abnormalities, most commonly cardiac in origin, as is the case of total obstruction of the pulmonary venous drainage. Congenital pulmonary lymphangiectasis may be also associated with lymphatic anomalies of other portions of the body or with other non-lymphatic, non-cardiac congenital malformations. Chest radiographs show more commo

Absence of an anatomical origin for altered ductus venosus flow velocity waveforms in first-trimester human fetuses with increased nuchal translucency.

OBJECTIVE: To perform a morphological evaluation of the ductus venosus, heart and jugular lymphatic sac (JLS) in first-trimester human fetuses with normal and abnormal ductus venosus flow velocity waveforms (DV-FVWs) and normal and increased nuchal translucency (NT). METHOD: Postmortem examination was performed on fetuses with increased NT or structural malformations with previous NT and DV-FVW measurements. Ductus venosus morphology was examined using markers for endothelium, smooth muscle actin (SMA), nerves and elastic fibers. Fetal hearts were studied by microscopy. The nuchal region was

Total parenteral nutrition as a primary therapeutic modality for congenital chylous ascites: report of one case.

Congenital chylous ascites in the neonatal period is a rare entity. It is primarily related to congenital abnormalities of the lymphatics. We present a case in which ascites was detected by prenatal ultrasonogram. No evidence of congenital cytomegalovirus infection, intrauterine meconium peritonitis, or intestinal or genitourinary system abnormalities was found. Congenital chylous ascites was confirmed via an abdominal sonogram and diagnostic paracentesis. After 26 days of NPO and total parenteral nutrition, the newborn hadfully recovered.

A somatic activating KRAS variant identified in an affected lesion of a patient with Gorham-Stout disease.

Gorham-Stout disease (GSD), a rare disorder of unknown etiology, is characterized by massive osteolysis that is associated with proliferation and dilation of lymphatic vessels. Variants in cancer-associated genes have been described in complex lymphatic anomalies. To explore the pathogenesis of GSD, we performed the amplicon-based deep sequencing on 50 cancer-related genes to assay affected tissues from the six patients with GSD. In one patient, a somatic activating KRAS c.182A > G variant (p.Q61R) was detected in 1% of the tissue sample. Conversely, the mutant allele was not detected in uninv

Idiopathic Chylous Pericardial Effusion Due to Lymphatic Malformation of Mediastinum.

Chylopericardium can be due to a variety of secondary causes like trauma, radiation, tumors, following cardiac surgery, etc., or may be idiopathic due to abnormal lymphatic system and mediastinal lymphangiectasia, which is a rare entity. Here, we present a case of a 34-year-old previously healthy male presenting with idiopathic chylopericardium. 2D echocardiography revealed massive pericardial effusion without features of cardiac tamponade. Following pericardiocentesis, a CT scan of the thorax and MR lymphangiogram were done to arrive at a diagnosis of idiopathic chylopericardium. In addition

The Effects of Kinect-Enhanced Lymphatic Exercise Intervention on Lymphatic Pain, Swelling, and Lymph Fluid Level.

BACKGROUND: The-Optimal-Lymph-Flow (TOLF) intervention aims to promote lymph flow through therapeutic lymphatic exercises to relieve lymphatic pain, swelling, lymphedema symptoms, and to decrease lymph fluid levels among breast cancer survivors. To enhance the efficacy of the TOLF intervention, an innovative, intelligent, Kinect-enhanced lymphatic exercise intervention (Kinect-TOLF) was developed to teach patients to perform the lymphatic exercises correctly. OBJECTIVES: This feasibility trial aimed to determine the feasibility, usability, and effects of the Kinect-TOLF on lymphatic pain, swe

[Diagnosis and surgical treatment of vascular anomalies in children: a case report].

According to the classification of the International Society for the Study of Vascular Anomalies (ISSVA), vascular anomalies include various abnormalities classified as vascular tumors or vascular malformations. This classification, last updated in 2018, is intended to explain biological basis of vascular lesions and help doctors treating abnormalities. In vascular tumors, proliferative changes in endothelial cells are observed, while vascular malformations are mainly structural vascular anomalies. Vascular malformations are an extensive group of arterial, venous, and lymphatic malformations (

[Practical importance of capillary microscopy].

Vital capillaroscopy has been successfully used for many years to study the microcirculation of human skin capillaries. The presence of typical capillary abnormalities is helpful in distinguishing scleroderma from other connective tissue diseases. Application of a local cold exposure test to finger nailfold capillary microscopy reveals the functional difference between normal subjects and patients with Raynaud's syndrome on the level of the microcirculation. Transcutaneous intravital fluorescence videomicroscopy may be helpful in the sometimes difficult differential diagnosis of lower limb ede

Primary Intestinal Lymphangiectasia Manifested as Unusual Edemas and Effusions: A Case Report.

Primary intestinal lymphangiectasia (PIL) is a rare disorder of unknown etiology characterized by diffuse or localized dilation and eventual rupture of the enteric lymphatic vessels in mucosa, submucosa, and/or subserosa. Lymph, rich in all kinds of proteins and lymphocytes, leaks into the gastrointestinal tract via the affected lymphatic vessels causing hypoproteinemia and lymphopenia. The main symptom is variable degrees of pitting edemas of bilateral lower limbs. But edemas of any other parts of body, and mild serous effusions may also occur sometimes. PIL occurs in conjunction with a right

Akt signaling regulates the activities of lymphatic malformation endothelial cells through PKM2 nuclear translocation.

Lymphatic malformation (LM) is a developmental lymphatic anomaly characterized by abnormally activating lymphatic endothelial cells (LECs) and lymphangiogenesis. Recent studies have shown that glycolysis can promote LM progression and pyruvate kinase M2 isoform (PKM2) is highly expressed in LM, whereas the mechanism involved remains unknown. In this study, we explored the mechanism for nuclear translocation of PKM2 in LECs isolated from LM (LM-LECs). Metabolic flux analyses, cell migration, cell invasion, and tube formation assays were performed to investigate the glycolysis level and activiti

Vulvar Lymphangioma Arising in the Setting of May-Thurner Syndrome.

This case report describes an instance of vulvar lymphangioma occurring in the setting of May-Thurner syndrome (MTS), an association between two vascular conditions that we do not believe has been previously reported. Lymphangioma, also known as lymphatic malformation, is a benign lesion typified by dilatation of endothelial-lined lymphatic channels involving the skin and subcutis, which can occur either as a congenital abnormality or as a result of acquired damage to lymphatic channels. Lymphangioma is a rare lesion in the vulva. MTS, also known as iliac vein compression syndrome or Cockett's

Primary lymphoedema.

Lymphoedema is the swelling of one or several parts of the body owing to lymph accumulation in the extracellular space. It is often chronic, worsens if untreated, predisposes to infections and causes an important reduction in quality of life. Primary lymphoedema (PLE) is thought to result from abnormal development and/or functioning of the lymphatic system, can present in isolation or as part of a syndrome, and can be present at birth or develop later in life. Mutations in numerous genes involved in the initial formation of lymphatic vessels (including valves) as well as in the growth and expa

Ascites in Animals With Right Heart Failure: Correlation With Lymphatic Dysfunction.

Background Congestive heart failure is a leading cause of morbidity and mortality worldwide. One of the signs of congestive heart failure is fluid overload including pulmonary edema, peripheral edema, and ascites. The cause of fluid overload remains incompletely understood, and management of these patients continues to be a challenge. The role of lymphatic circulation abnormalities in the cause and pathophysiology of fluid overload also remains unclear. Here we report on a study in a large animal model of right heart failure caused by severe tricuspid regurgitation comparing cardiovascular and

Robot-assisted staged bilateral reno-lymphatic disconnection for massive idiopathic chyluria: A case report.

Chyluria, an abnormal lymphatic disorder, results in excessive abdominal lymph drainage into the urinary system, causing protein loss, nutritional deficiencies, and immune issues. Mainly linked to parasitic infections in developed countries, non-parasitic causes like trauma or tumors are rare. Typically appearing in adults with bilateral involvement, management options include conservative or surgical approaches. We present the case of a 13-year-old with congenital chyluria, treated with robot-assisted staged reno-lymphatic disconnection after failed interventional radiology. Bilateral scleroa

Sex-limited penetrance of lymphedema to females with CELSR1 haploinsufficiency: A second family.

A second multigeneration family with hereditary lymphedema (LE) secondary to a variant in the planar polarity gene, CELSR1, is described. Dominant inheritance of the variant was discovered using whole-exome sequencing and confirmed by Sanger sequencing. In contrast to heterozygous males, all heterozygous females showed LE during physical examination albeit variable in severity and age of onset. Lymphscintigraphy in affected females showed previously undescribed lymphatic abnormalities consistent with lymphangiectasia, valve dysfunction, and thoracic duct reflux.

Mediastinal cystic lymphangioma in a patient with situs inversus totalis.

We present a case of cystic lymphangioma of the mediastinum complicated with situs inversus totalis. The 70-year-old man underwent thoracoscopic resection of a mediastinal cystic tumor, which was diagnosed as cystic lymphangioma. Cystic lymphangiomas are congenital cystic abnormalities of the lymphatic system. The head and neck area is often involved while the mediastinum is rarely affected. The rarity of this case is further attributed to the coexistence of situs inversus totalis.

[A case of yellow nail syndrome presenting as recurrent pneumonia].

We report a case of yellow nail syndrome (YNS) associated with recurrent episodes of pneumonia. The patient, a 31-year-old female, presented with yellow nails, chronic bronchitis and pleural effusion. The etiology of YNS remains unknown although it has been postulated to be associated with lymphatic abnormalities. YNS is a rare disease with only 22 cases reported in Japan. However, YNS is often associated with respiratory infections, and should thus be borne in mind.
