Magnesium and potassium-sparing diuretics.

One of the most common and serious side effects of diuretic therapy is in increased urinary loss of K. Another, although less well publicized, side effect of diuretic therapy is excessive urinary loss of Mg. In examining the effects of diuretics on Mg and K metabolism, the following factors should be taken into account: site of action and duration of action of diuretics, duration of treatment and dosage used, concurrent drug therapy, underlying disease conditions and dietary intake of Mg. Diuretics acting in the proximal tubule tend to have only minor effects on Mg excretion. Loop-blocking diu

[Magnesium deficiencies in cardiovascular diseases].

Plasma (pl), red blood cell (ery) and urinary magnesium (Mg) concentrations were measured by spectrophotometry in over 300 patients at the Cardiology Department of Broussais Hospital. Other biological parameters, including pl and ery potassium, calcium and phosphate concentrations were measured simultaneously. In a control group (54 subjects) the mean pl Mg was 0,851 mmol/l in men and 0,819 mmol/l in women; mean ery Mg was 2,12 mmol/l and 2,09 mmol/l respectively. Magnesium depletion was observed in several pathological cardiovascular conditions: -- mitral valve prolapse: the Mg levels were si…

Magnesium in coronary artery disease.

Magnesium in coronary artery disease is reviewed with regard to its role in the pathogenesis of arteriosclerosis, coronary spasm, myocardial function, acute myocardial infarction and ventricular arrhythmias. Experimentally, magnesium depletion potentiates and supplementation retards the effect of atherogenic diets. Evidence from human studies is circumstantial. Reactivity of arterial smooth muscle is enhanced by low and suppressed by high magnesium media. Evidence that magnesium depletion may initiate coronary spasm is provided by dog and retrospective human studies. Although experimental magn…

Fluid and electrolyte disorders. Magnesium.

Magnesium is the second most abundant intracellular cation but it is still not measured often enough. It is closely linked with other electrolytes such as calcium, potassium, and phosphate so that the correct clinical interpretation of abnormal results can be complex.

Diuretics, magnesium, potassium and sodium.

Diuretics increase renal magnesium excretion and, when chronically administered, decrease intracellular magnesium levels. This deficiency reduces active transport of potassium into the cell and intracellular potassium decreases irrespective of serum potassium levels. Potassium supplementation or the co-administration of potassium-sparing diuretics cannot maintain intracellular potassium at normal levels when intracellular magnesium is reduced. The effects of chronic magnesium supplementation have not been adequately studied. Maintenance of the diuretic dose at an effective minimum and curtaili…

Physicochemical properties of magnesium salicylate.

Magnesium salicylate tetrahydrate is a nonhygroscopic, crystalline powder, whereas anhydrous magnesium salicylate is amorphous and very hygroscopic. Magnesium salicylate tetrahydrate tablets formulated with gelatin as a binder showed a dissolution half-life (t1/2) of 12 min, whereas a formulation using pregelatinized starch as a binder showed a t1/2 of 33 min. The optimum level of calcium stearate in the formulation was determined by the oscilloscope tracings of compressional and ejectional forces from an instrumented rotary tableting machine. Increasing the level of calcium stearate from 1 to…

Diuretic treatment and magnesium losses.

Therapy with most kinds of diuretics causes increased urinary excretion of both potassium and magnesium and after long term treatment the skeletal muscle content of these ions is reduced. Magnesium is known activator of Na-K-ATP:ase, which provides the energy for the sodium-potassium pump. Thus, lack of magnesium may impair the pumping of sodium out from the cell and potassium into the cell. Three studies are presented which agree with this theory. In one study the correlation between potassium in serum and potassium in muscle was, among other factors, found to be dependent on the content of m…

A simple low-magnesium diet.

Details are given of a semi-synthetic low-magnesium diet that provides adequate short-term nutrition. The diet is made up of proprietary foodstuffs and is suitable for short-term magnesium deprivation studies in man. After five days on the diet four subjects demonstrated a marked fall in urinary magnesium excretion.

[Magnesium and several metabolic osteopathies].

Magnesium is a constituant of the bone minerals. Changes in its intra-bone distribution have repercussions on the distribution of calcium and on the hormonal regulators of calcium. The importance of magnesium in bone pathology is far from being completely elucidated. Certain factors are now commonly admitted, for example the inhibition of the parathyroid secretion during magnesium deficiency. Others merit more complete and thorough study, it being highly unlikely that the bone is unaffected by changes in the organic magnesium content, given the multiple roles that magnesium plays in the bone. …
