The evolution of payer management of oncology drugs in the United States between 2017 and 2022.

BACKGROUND: Research conducted in 2017 by Runyan et al concerning the current and future management of oncology drugs in the United States formed the basis for this research. The authors concluded that despite the high cost of oncology drugs, US payers relied on traditional management tools to manage the category, although these tools were ineffective at controlling costs. Innovative tools were not common in 2017. OBJECTIVE: To compare findings from the 2017 research with findings from a 2022 payer survey to understand how payer management of oncology drugs changed over 5 years. The study eval

Preferences of continuous care service options for patients with venous leg ulcer development of attributes for discrete choice experiments.

BackgroundPatients suffering from lower extremity venous ulcers typically undergo prolonged dressing changes, entailing extended treatment cycles and significant costs, creating an urgent need for effective continuous care. There is scarce literature reporting on the preferences and requirements for wound care within continuous care services for such conditions. Discrete choice experiments serve as an innovative method to elicit patient preferences, where the development of attributes and levels is a critically important process.ObjectiveTo identify attributes and levels patients with venous l

Nursing and nursing education in Haiti.

Haiti has long had the largest proportion of people living in poverty and the highest mortality level of any country in the Americas. On January 12, 2010, the most powerful earthquake to hit Haiti in 200 years struck. Before the earthquake, half of all Haitians lacked any access to modern medical care services. Health care professionals in Haiti number around one-fourth of the world average and about one-tenth the ratio present in North America. The establishment of new primary care services in a country where half of the people had no access to modern health care prior to the earthquake requi

Cost evaluation of reproductive and primary health care mobile service delivery for women in two rural districts in South Africa.

BACKGROUND: Cervical cancer screening is a critical health service that is often unavailable to women in under-resourced settings. In order to expand access to this and other reproductive and primary health care services, a South African non-governmental organization established a van-based mobile clinic in two rural districts in South Africa. To inform policy and budgeting, we conducted a cost evaluation of this service delivery model. METHODS: The evaluation was retrospective (October 2012-September 2013 for one district and April-September 2013 for the second district) and conducted from a

Mectizan Donation Program: evaluation of a public-private partnership.

The Mectizan Donation Program (MDP) has been perceived as a highly effective public health programme, and as a possible model for addressing future problems in international health. This evaluation examines how the MDP partnership has been functioning from the perspectives of partner organizations. The results of a survey of 25 partners show that the perceived benefits far outweigh the problems, and that the direct costs to the organizations have been minimal. The partnership is rated highly on many aspects of governance and management, with relatively few problems identified. A factor analysi

Fetal alcohol spectrum disorders prevention and clinical guidelines research - workshop report.

It is estimated that up to 1 in 20 people in the United States may have a fetal alcohol spectrum disorder (FASD), or the array of physical, cognitive, emotional, and social disorders caused by exposure to alcohol during prenatal development (May et al., JAMA 319:474-82, 2018). While this condition is present in a broad range of individuals and families, it has not previously been examined in the military community, where cultural factors including an increased prevalence of alcohol misuse may pose a unique set of challenges (Health.mil, Alcohol misuse, 2024).The Uniformed Services University o

A case study of a hepatitis B screening and blood pressure assesment program in Los Angeles County, 2012-2013.

Low-income, uninsured women are disproportionately affected by cardiovascular disease (CVD). Facilitating primary and secondary prevention of CVD in this population requires supports beyond clinical and public health agencies. The Illinois WISEWOMAN Program (IWP) provides CVD prevention, screening, and management for women who lack health insurance in Illinois, including the use of referral systems to link clients to needed services. To support these efforts, the 7 local agencies implementing IWP developed linkages to facilitate participant referrals for health promotion, clinical services, an

Framework to Action: Developing and Implementing a Statewide Network to Expand Opioid Treatment Access Using SHIFT-Evidence in Texas.

BACKGROUND: In Texas, the expansion of access to effective treatments for opioid use disorder remains a significant public health challenge. This study aimed to demonstrate the application of the Successful Healthcare Improvement from Translating Evidence in Complex Systems (SHIFT-Evidence) framework in developing and implementing a statewide network for office-based opioid treatment (OBOT) to underserved populations. METHODS: We identified systemic gaps in access to medications for opioid use disorder (MOUD) across Texas, using a targeted needs assessment of barriers and facilitators to bupr

Readiness to implement contingency management to promote PrEP initiation and adherence among people who inject drugs: results from a multi-site implementation survey.

BACKGROUND: Contingency management (CM), an incentive-based intervention to encourage target behaviors, effectively promotes medication adherence. However, efforts to extend CM to HIV pre-exposure prophylaxis (PrEP) have been lacking. As part of a randomized clinical trial to promote HIV Prevention among people who inject drugs (PWID), we examined the readiness of staff in community-based organizations serving PWID to implement CM for PrEP uptake and adherence in this population. METHODS: From April to August 2022, we conducted a survey of staff from four community-based organizations providi

Sexually acquired syphilis: Laboratory diagnosis, management, and prevention.

The methods used for the laboratory diagnosis of syphilis include direct detection of Treponema pallidum subspecies pallidum and serologic testing. Serologic testing relies on both nontreponemal and treponemal tests. In newly developed reverse-sequence screening algorithms, treponemal tests are performed before nontreponemal tests. The management of syphilis requires appropriate staging, treatment, and follow-up of patients along with the prompt reporting of infections to public health authorities to assist with prevention and control efforts. Benzathine penicillin G remains the treatment of c

Prevention and management of gastrointestinal infections in infants from a nutritional perspective.

This article considers infections of the gastrointestinal (GI) tract. This is a complex organ, which exists in a range of environments. Despite containing defence mechanisms against microorganisms, GI infections are common throughout infancy; however, the risk of infection can be reduced through careful hygiene and the encouragement of breast-feeding. Although research into the role of dietary factors in preventing or treating GI infection is in its early days, there is some evidence for the use of prebiotics and probiotics. The role of health care professionals is to give parents and carers a

A study of rural Latino women seeking cancer-detection examinations.

The Latino women's breast and cervical cancer screening project, A Day for Latino Women, was a well-organized, collaborative effort between the Catholic community outreach program serving the rural Latino community, the local university, the university nursing program, and a rural-nurse-managed clinic. Eighty-one women were screened at the rural-nurse-managed clinic (TCHC) at no cost. Barriers to access such as the need for transportation, cost, and the need for child care were essentially eliminated to encourage use of the TCHC as a usual site of medical care for this ethnic population. Workarounds to Mandated Data Practices: Rethinking Digital Infrastructure in Child Welfare and Homeless-Serving Organizations.

Over the past decade, much of the infrastructure used for social service delivery in Canada has been digitized. Drawing on institutional ethnographic research practices, we undertook observational research, facilitated focus groups with twenty five key informants, and interviewed 121 leaders and workers in the child welfare and youth homelessness sectors to learn how data practices in these sectors are shaped by government-mandated infrastructure and tools associated with the adoption of digital information management systems in the public sphere. Despite promises that these new digital techno
