Accountability for maternal and newborn health: Why measuring and monitoring broader social, political, and health system determinants matters.

This article offers four key lessons learned from a set of seven studies undertaken as part of the collection entitled, "Improving Maternal Health Measurement to Support Efforts toward Ending Preventable Maternal Mortality". These papers were aimed at validating ten of the Ending Preventable Maternal Mortality initiative indicators that capture information on distal causes of maternal mortality. These ten indicators were selected through an inclusive consultative process, and the research designs adhere to global recommendations on conducting indicator validation studies. The findings of these…

Caries risk in toddlers: assessing oral health, microbial factors, and maternal transmission.

OBJECTIVES: Caries risk begins early in life, yet few studies use intraoral cameras to assess risk in toddlers or examine multiple contributing factors. This study aimed to address this gap and conduct intraoral camera-based assessment of caries and evaluate multiple toddler risk factors, including microbial profiles, oral hygiene practices, and potential maternal transmission. METHODS: Cross-sectional data from 21 maternal-child dyads included intraoral camera assessments coded with an adapted International Caries Detection and Assessment System, saliva-based microbial tests for Streptococcu

Maternal Satisfaction With Midwives' Caregiving in Public Health Facilities: A Scoping Review.

Midwives play an important role in women's childbirth-led care as they provide a protective role against maternal stress and foster deeper relationships. Midwives can significantly influence satisfaction levels, particularly when they provide consistent, high-quality, and empathetic support throughout labor and delivery, with potential positive implications on healthcare systems. This study aimed to review the current literature on indicators of maternal satisfaction with midwife care received during childbirth. A comprehensive search of the available literature regarding maternal satisfaction

Birth order and infant health: Evidence from maternal immunisation in New Zealand.

Immunisation during pregnancy is a vital strategy to protect infants from infectious diseases in their first months of life, yet little is known about how birth order relates to maternal vaccination uptake and infants' risk of severe illness. Drawing on administrative data from New Zealand covering more than 200,000 births between 2015 and 2023, I analyse the relationship between birth order and maternal vaccination against pertussis and influenza. I also examine subsequent infant hospitalisations for these diseases. The findings show that later-born children experience higher hospitalisation

Programming offspring bone health: mechanistic perspectives on maternal macro- and micronutrient intake.

Maternal nutrition is a critical determinant of offspring bone health through the process of fetal programming. Adequate intake of both macro- and micronutrients during pregnancy supports skeletal development, bone mineralization, and long-term bone health. In contrast, maternal undernutrition, overnutrition, or nutrient imbalance can impair bone quality, alter microarchitecture, and increase the risk of osteoporosis in later life. Emerging evidence indicates that these effects are mediated by hormonal regulation, oxidative stress, and epigenetic modifications that influence osteoblast functio

Melatonin and Artificial Light: Effects on Maternal and Fetal Health During Pregnancy.

INTRODUCTION: The placenta produces melatonin during pregnancy, a hormone essential for circadian regulation and maternal-fetal metabolic, hormonal, and immunological modulation. Throughout pregnancy, melatonin production increases progressively, being synthesized by both the pineal gland and the placenta. Since exposure to artificial light at night (ALAN) reduces pineal melatonin secretion, it is hypothesized that light pollution may negatively impact women's health, maternal health, and fetal development due to the inhibition of both pineal and placental synthesis. MATERIAL AND METHODS: Thi…

Maternal healthy lifestyle during pregnancy and offspring metabolic health phenotypes in childhood.

BACKGROUND AND AIM: Maternal lifestyle factors during pregnancy may influence offspring risk of childhood overweight-obesity (OW-OB). Obesity is a heterogenous condition with different metabolic phenotypes; limited research on their associations with early-life exposures exists. We investigated associations of a composite maternal healthy lifestyle score (HLS) and offspring metabolic phenotypes throughout childhood. METHODS AND RESULTS: Study-specific analyses were conducted on mother-child dyads (n = 6088) from four European studies: two RCTs (secondary analysis) and two mother-child cohorts…

Neuroplacentology of stress: Novel frontiers linking maternal mental health to offspring neurodevelopment.

Neuroplacentology is an emerging interdisciplinary field integrating molecular neuroscience, placental biology, environmental modeling, and single-cell techniques to study stress-related neurodevelopmental programming. The placenta, once considered merely a conduit between mother and fetus, is now recognized as an important regulator of fetal brain development and a mediator of prenatal maternal stress and other exposures. To highlight this important aspect of the neurobiology of stress, this review outlines how maternal stress, genetic susceptibility, and environmental exposures converge at t

Maternal Incarceration and Health Risk Behaviors Among Adolescents: A Latent Class Analysis.

Most studies on the impact of maternal incarceration on adolescent health risk behaviors have focused on singular, separated behaviors, even though these behaviors often cluster and co-occur. This study used the FFCWS dataset to examine the association between maternal incarceration and the aggregation of health risk behaviors among adolescents. Latent class analysis suggested the four-class model had the optimal model fit. Multinomial logistic regression revealed that the odds of adolescents with maternal incarceration histories being classified into the "high dietary risk behavior," "high in

Molecular Biomarkers for Timely and Personalized Prediction of Maternal-Fetal Health Risk.

Molecular biomarker profiling is an emerging field in maternal-fetal health with the potential to transform early detection and prediction of placental dysfunction. By analysing a range of biomarkers in maternal blood, researchers and clinicians can gain crucial insights into placental health, enabling timely interventions to enhance fetal and maternal outcomes. Placental structural function is vital for fetal growth and development, and disruptions can lead to serious pregnancy complications like preeclampsia. While conventional methods such as ultrasound and Doppler velocimetry offer valuabl

Pregnancy and Caffeine Metabolism: Updated Insights and Implications for Maternal-Fetal Health.

Caffeine is one of the most widely consumed psychoactive substances globally and is a common component of daily diets, particularly among women of reproductive age. Numerous in vitro and in vivo studies have indicated potential adverse effects of prenatal caffeine exposure, including disturbances in fetal growth, metabolic dysregulation, organ malformations, and neurodevelopmental alterations. These findings suggest that caffeine may influence multiple physiological pathways during gestation, including epigenetic modifications and metabolic programming. However, evidence from human studies rem
