Tilted Endosseous Dental Implants in Atrophic Maxilla: A Clinical Prospective Study.

Background Atrophic edentulous maxillary arches present a significant surgical challenge for conventional implant placement owing to inadequate bone height, pneumatization of the maxillary sinus, and compromised bone quality in the posterior region. Traditional solutions, including sinus floor elevation, ridge augmentation, and bone grafting, carry inherent morbidity and procedural complexity. Objective This study aimed to evaluate the clinical and radiographic outcomes of tilted endosseous dental implants placed in atrophic maxillae, with specific assessment of osseointegration quality, crest…

Adenoid cystic carcinoma with myoepithelial predominance affecting maxilla and maxillary sinus.

Adenoid cystic carcinoma (ACC) represents less than 10% of all salivary gland tumors, rarely occurring centrally in the jaws. Herein we describe the case of a 36-year-old female patient presenting a painless swelling in the right maxilla, resulting in marked facial asymmetry. Intra-orally it was observed a swelling covered by an erythematous and irregular-surfaced mucosa, affecting the gingiva and crossing the midline of the hard palate. Imaging studies showed a mixed radiolucent-radiopaque lesion with ill-defined borders, involving the right side of the maxilla and the maxillary sinus. Incisi…

Non-Familial Cherubism with Bilateral Maxilla and Mandible Involvement - Clinicoradiographic Findings.

Cherubism is a self-limiting non-neoplastic autosomal dominant fibro-osseous syndrome of the jaws. It is occasionally manifested before the age of two years. It occurs in children and more often in boys. It is characterized by notable clinical bilateral swelling of the cheeks due to a bony enlargement of the jaws that impart a characteristic 'cherubic' look. Regression occurs in the course of puberty leaving a few facial deformities and malocclusion. Cherubism might occur in solitary cases or in several members of the family, often in many generations. The reported case is an example of solita

Functional reconstruction of maxilla with free latissimus dorsi-scapular osteomusculocutaneous flap.

Restoration of oral and nasal function together with facial appearance is still challenging in maxillary reconstruction. Use of a composite flap transfer merely to fill the defect results in unsatisfactory functional and aesthetic outcomes. The authors present a reconstructive procedure for complex maxillary defects using the latissimus dorsi-scapular rib osteomusculocutaneous flap. Some modifications for the reconstruction of the nasal cavity and the hard palate contributed to excellent postoperative functions. Five cases of extended maxillary defect were reconstructed using a novel procedure…

[Benign paroxysmal positional vertigo secondary to surgical maneuvers on superior maxilla].

The benign paroxysmal positional vertigo (BPPV) is a common vestibular end organ disorder. BPPV may be idiopathic or secondary to head traumas, viral labyrinthitis, otology surgery and many others conditions. We report two cases of BPPV secondary to surgical maneuvers on superior maxillary. We have not found this etiology in scientific literature but, the increase of this kind of surgery, can produce an increase of its frequency. For this reason the otoneurologists and oral surgeons must bear in mind this possibility to make a right diagnostic and treatment of the syndrome.

Avoiding Unwanted Exposure of Subperiosteal Implants in the Atrophic Maxilla: Technical Note.

The use of subperiosteal implants for the rehabilitation of atrophic maxillae has taken attention recently as a validated option in the treatment of severe atrophy. The systems have been improved by technological advancements and the analysis of variables affecting their prognosis. However, one of the most frequent issues to resolve is the unwanted exposure of a portion of the metal mesh close to the abutment and prosthetic connections, which can result in infections, hygiene problems, and, in some instances, complete failure. It seems that the surgical approach is a hot topic for the success …

Maxilla Reconstruction with Xenograft Blocks to Allow Implant Rehabilitation: A Case Report.

Reducing patient morbidity is one of the challenging goals of proper surgical techniques. The rehabilitation success begins with a good surgical plan. Scientifically proven biomaterials are excellent options to overcome the lack of autologous bone for maxillary reconstructions. This case report presents a clinical case of maxillary reconstruction combining guided bone regeneration with xenograft blocks and maxillary sinus elevation. This technique achieved maxillary rehabilitation with a fixed implant-supported prosthesis of a challenging case. The clinical case has an 18-month follow-up with …

Surgical Treatment of Aggressive Intraosseous Cavernous Hemangioma in Maxilla Through Surgical Resection.

Hemangiomas are benign vascular lesions characterized by endothelial vascular proliferation and may demonstrate aggressive clinical features. Intraosseous hemangiomas are uncommon and the maxillary location rare, with few cases described in the literature. The treatment of these lesions varies, however they have a better prognosis when a surgical treatment through resection is performed. The aim of this study is to report a case of right maxillary cavernous intraosseous hemangioma treated by surgical resection with previous embolization. The diagnosis was made through incisional biopsy and imm…

Diagnostic considerations in central odontogenic fibroma of the maxilla: 2 case reports.

Central odontogenic fibroma (COF) is defined as a fibroblastic odontogenic tumor characterized by varying density of the tooth epithelium. It is an extremely rare benign neoplasm that occurs in the maxilla and the mandible; only a few reports of COF are available in the literature. Diagnosis of the lesion based only on the radiological features of COF is difficult due to variation in the findings regarding this condition. This report describes 2 clinical cases of middle-aged women with COF. Clinical examination revealed palatal mucosal depression; additionally, oral examination, as well as pan…

Mucormycosis of maxilla following tooth extraction in immunocompetent patients: Reports and review.

Mucormycosis is a rare, fulminant, rapidly spreading fungal infection, which usually affects patient with underlying immune deficiency. If not managed promptly, the disease is characterized by progressive necrosis and is often fatal. A review of English literature shows that only fourteen cases of mucormycosis have been reported after tooth extraction. This paper highlights two cases of mucormycosis subsequent to tooth extraction in healthy adult patients. This first patient presented with an oroantral fistula and extensive maxillary necrosis. Whereas the second case was localized and presente…

Aberrations Causing Neurovascular Damage in the Anterior Maxilla during Dental Implant Placement.

When dental implants are being considered for placement in the maxillary central incisor region, proximity to the nasopalatine canal and its contents needs to be accounted for. The morphology of the canal changes with age. The availability of CBCT has allowed the in-depth analysis of this important variable anatomy. However, an associated important anatomical structure can be easily overlooked: the "canalis sinuosus." This is a neurovascular canal carrying the anterior superior alveolar (ASA) nerve and artery. CBCT frequently shows the canalis sinuosus (CS) as a wide canal lateral to the nasal…
