Malignant peripheral nerve sheath tumour: an elusive diagnosis.

Malignant peripheral nerve sheath tumour (MPNST) also termed as spindle cell malignancy of the peripheral nerve Schwann cells or neurogenic sarcoma, represents approximately 10% of all soft tissue sarcomas. This tumour is usually found in the lower extremities and only 10-12% of all lesions occur in the head and neck region, which makes it a rare entity. The diagnosis of MPNST has been described as one of the most difficult and elusive diagnosis in the soft tissue diseases because of its non-specific presentation both clinically and histopathologically. This was overcome by the use of immunohi

Zygomycosis of the maxillary sinus and palate caused by Basidiobolus haptosporus.

Basidiobolus haptosporus is known to cause subcutaneous zygomycosis in tropical Africa and Asia. We cared for a 49-year-old hyperglycemic, asplenic man who had never traveled outside the United States and who was seen initially for a painless palatal ulcer with cutaneous hypesthesia of the right cheek and upper lip. An invasive process involved the right middle nasal turbinate, maxillary antrum, maxillary division of the trigeminal nerve, and bony palate. Histological examination of biopsy tissue showed necrotizing granulomata with broad, nonseptate hyphae. Basidiobolus haptosporus was culture

Acquired unilateral pseudoptosis: a presentation of silent sinus syndrome.

BACKGROUND: Acquired unilateral ptosis usually results from either levator palpebrae disinsertion, eyelid disorder, myasthenia gravis, mitochondrial myopathy, Horner's syndrome, or third nerve palsy. In rare cases, an orbital process is responsible for ptosis or pseudoptosis. HISTORY AND SIGNS: A healthy 31-year-old patient complained for 6 months of right eyelid ptosis. On examination, there was 1 mm right upper eyelid ptosis and 1 mm of right enophthalmos. Upon downgaze, an apparent right eyelid retraction was present. Magnetic resonance imaging showed filling and retraction of the right ma

Use of Cryoneurolysis Therapy for the Management of Idiopathic Trigeminal Neuralgia.

INTRODUCTION: Trigeminal neuralgia is a debilitating facial pain condition characterized by recurrent, electric shock-like episodes within the trigeminal nerve distribution. While first-line treatment involves pharmacologic therapy, treatment options are limited for patients who are refractory or poor surgical candidates. CASE PRESENTATION: We present a 75-year-old female with long-standing idiopathic trigeminal neuralgia refractory to combination anticonvulsant therapy and nerve block, with notable medication-related side effects. MANAGEMENT AND OUTCOMES: The patient underwent percutaneous

Regional anesthesia in head and neck surgery.

Regional anesthesia in the head and neck area is a common and effective tool for the plastic surgeon. For short procedures, it offers the advantages of convenience and comfort to the patient. In many procedures, in addition, it offers advantages to the plastic surgeon over general anesthesia. A knowledge of anatomy of the course of the sensory nerves in the head and neck is essential to execute regional anesthesia. Some variability is always present. However, the most important factor in success is practice with a specific technique until it is mastered. No attempt was made in this article to

A case of cerebellopontine angle epidermoid cyst presenting as trigeminal neuropathy.

We present the case of a 35-year-old female who had paraesthesia and numbness on the left side of her face. Initially the ophthalmic division and maxillary divisions on the trigeminal nerve were involved, but later involved the mandibular division as well. On MRI, a cerebellopontine angle (CPA) epidermoid cyst was detected. After 18 months of follow up, her symptoms remain stable and she is being managed conservatively.

Medial portion of M. Temporalis and its potential involvement in facial pain.

We describe the morphology, functional capabilities, and clinical anatomical implications of the medial portion of the temporalis muscle. Although recognized in Brazilian, German, French, and Hispanic American textbooks of anatomy, this portion of the temporalis has been unappreciated in those published in England and the United States. Given its morphology, and functional anatomy as revealed in this paper, we conclude that the medial portion of the temporalis could entrap the maxillary nerve and its zygomatic branches inside the pterygopalatine fossa. On these bases we identify this portion o

Herpes zoster on the face in the elderly.

Herpes zoster is a localised disease caused by reactivation of the varicella zoster virus that enters the cutaneous nerve endings during an earlier episode of chicken pox, travels to the dorsal root ganglia, and remains in latent form. The condition is characterised by occurrence of multiple, painful, unilateral vesicles and ulceration, and shows a typical single dermatome innervated by single dorsal root or cranial sensory ganglion. Involvement of three or more dermatomes is known as disseminated zoster and seen in immunocompromised individuals. Complications of herpes zoster include ocular s

[Cranial mucormycosis with thrombosis of the sinus cavernosus (author's transl)].

A case of mucormycosis in a 45 year-old woman with uncontrolled diabetes is reported. Following dental extractions, the patient presented with total ophthalmoplegia, chemosis and complete sensory loss of the right fifth cranial nerve. Exophthalmus was not present. Although a fungal infection was suspected, administration of Amphotericin B was avoided because of renal insufficiency. After temporary clinical improvement with high doses of antibiotics, signs of right seventh and eighth cranial nerve paralysis and of thrombosis of the right ophthalmic artery developed. The patient expired after si

The cluster-tic syndrome and its surgical therapy.

The term cluster-tic syndrome (CTS) is used to designate a clinical pain pattern in which symptoms of cluster headache (CH) and tic douloureux (TD) coexist. The TD elements of the attack occur in paroxysms of many seconds or minutes, always affect the maxillary or mandibular divisions of the trigeminal nerve, with spread into the ophthalmic division in some cases, and may be triggered by slight superficial stimuli. These features may occur independent of CH elements but more often the two blend together. Four patients with CTS unresponsive to medication underwent surgery. Blood vessels were fo

Canalis Sinuosis: Implications in Implant Dentistry.

The canalis sinuosis is an anatomical canal located in the anterior maxilla in a minority of patients. It contains a neurovascular bundle that may be encountered during dental implant placement and can be overlooked in radiologic examinations. The neurovascular bundle contains the anterior superior alveolar artery, a branch of the infraorbital nerve, the corresponding vein, and the anterior superior alveolar nerve. Cone-beam computerized tomography (CBCT) may be important in identifying this structure to prevent complications. Complications such as the canalis sinuosis may be confused with pat
