Complex orthodontic and surgical management of an adult patient with transverse maxillary deficiency and skeletal class III malocclusion: A case report.

Skeletal deformities constitute a relatively common structural and functional craniofacial abnormality. The chief complaints reported by pat … age was orthodontic treatment, involving dentoalveolar decompensation, as part of preparation for bimaxillary osteotomy (BIMAX). The last stage of the complex treatment was BIMAX, which ultimately eliminated skeletal defects in both the sagittal and frontal planes. This complex multidisciplinary management significantly improved facial harmony, increased nasal volume …

[Endonasal antrostomy with endoscopical control for chronic maxillary sinusitis (author's transl)].

A transnasal approach to the maxillary sinus as a standard operation in chronic maxillary sinusitis is described. A mucosal flap in the lower nasal meatus is electrically circumcised, and a window of appropriate size is formed by osteotomy. The removal of pedicled polyps and cysts from the cavity is controlled by endoscopy, while most parts of the thickened mucosa remain in place and may recover and also line the uncovered areas. If necessary, an opening in the facial wall of 5 mm diameter is established for endoscopical control or manipulations. The preliminary results of 56 endonasal sinutom

Crouzon's disease. Part I. One-stage correction by combined face and forehead advancement.

Crouzon's disease results in maxillary hypoplasia and a receding forehead, which produce characteristic exorbitism. Total correction of the deformity by Le Fort III osteotomy and face advancement with a simultaneous forehead advancement corrects all the deformities. The advantages and disadvantages of this major craniofacial procedure are discussed after 2 case descriptions.

Is the piezoelectric device the new standard for facial osteotomies?

Piezoelectric devices are commonly used in all areas of maxillofacial surgery. Initially applied in preprosthetic surgery, they have steadily become a common practice in major surgery of the facial skeleton, including maxillary and mandibular osteotomies, temporomandibular joint surgery, orbital surgery, craniofacial procedures and rhinoplasty. We will review the current medical literature and establish the "state of the art" of piezosurgery in orthognathic surgery, TMJ surgery and rhinoplasty.

Esthetic Considerations In Minimally Invasive Orthognathic Surgery.

Orthognathic surgery has long been successfully utilized to correct dentofacial deformities to help facilitate improved oral function and facial esthetics. The treatment, however, has been associated with a high degree of complexity and severe postoperative morbidity. More recently, minimally invasive orthognathic surgical procedures have emerged that offer potential long-term benefits such as less morbidity, a lower inflammation response, improved postoperative comfort, and better esthetic outcomes. This article examines the concept of minimally invasive orthognathic surgery (MIOS), discussin

Intraoral vertico-sagittal ramus osteotomy: modification of the L-shaped osteotomy.

The sagittal split ramus osteotomy and intraoral vertical ramus osteotomy carry the potential risk of postoperative nerve paralysis, bleeding, and fracture and dislocation of the condyle. In 1992, Choung first described the intraoral vertico-sagittal ramus osteotomy for the purpose of avoiding postoperative dislocation of the condyle. However, there is still potential for damaging the inferior alveolar nerve and maxillary artery with this technique. The authors have developed a modified technique to minimize these risks. An evaluation of surgical experience and patient outcomes with the use of

Orthognathic Surgery in Cleidocranial Dysplasia.

Optimal surgical correction of the craniofacial manifestations of cleidocranial dysplasia (CCD) has not been established due to the rarity of the condition. A 27-year-old female with CCD is presented. She underwent virtual surgical planning (VSP) followed by LeFort-I disimpaction, bone grafting, bilateral sagittal-split osteotomy, genioplasty, submental lipectomy, and targeted facial fat grafting. The patient necessitated 15-mm of vertical maxillary disimpaction centrally, stabilized with wide maxillary plates and interpositional allogenic fibula grafts. Six-month postoperative examination dem

Trueness analysis of a robot-assisted system for maxillary supernumerary anterior tooth removal: an in vitro study.

The present study explored the trueness of active robotic system-assisted cortical-window osteotomy and tooth sectioning relative to conventional freehand surgery, employing a labial approach to remove impacted maxillary midline supernumerary teeth. Standardized three-dimensional maxillary replicas containing an impacted midline supernumerary tooth were randomized into an Active Robot-assisted Group (ARG) or a Freehand Group (FHG). Preoperative CBCT and intraoral scans were merged to plan osteotomy and tooth sectioning trajectories. Parameters analyzed included cortical-window formation and to

Optimal Fixation Location in Intraoral Reduction Malarplasty Using an L-Shaped Osteotomy.

BACKGROUND: Reduction malarplasty, popular among Asians, is currently performed preferably via the intraoral approach. However, to prevent complications such as malunion and cheek drooping, a rigid fixation is essential to compensate for the action of the masseter. Accordingly, the authors aimed to perform an experimental study on the optimal fixation location during intraoral reduction malarplasty using an L-shaped osteotomy. METHODS: Fixation points that are not difficult to access during an L-shaped osteotomy were selected as 1 point in the freed zygomaticomaxillary complex and 3 points in

Maxillary distraction osteogenesis versus orthognathic surgery for cleft lip and palate patients.

BACKGROUND: Cleft lip and palate is one of the most common birth defects and can cause difficulties with feeding, speech and hearing, as well as psychosocial problems. Treatment of orofacial clefts is prolonged; it typically commences after birth and lasts until the child reaches adulthood or even into adulthood. Residual deformities, functional disturbances, or both, are frequently seen in adults with a repaired cleft. Conventional orthognathic surgery, such as Le Fort I osteotomy, is often performed for the correction of maxillary hypoplasia. An alternative intervention is distraction osteog
