[The influence of accessory ostia on the development and the course of the inflammatory processes in the maxillary sinuses].

The objective of the present study was to detect accessory ostia in the patients presenting with various diseases of the maxillary sinuses. We undertook the endoscopic examination of the middle meatuses in 250 patients. It was shown that accessory ostia can be especially frequently seen in the patients with chronic maxillary sinusitis and nasal septum deviation.

The morphological interaction between the nasal cavity and maxillary sinuses in living humans.

To understand how variation in nasal architecture accommodates the need for effective conditioning of respired air, it is necessary to assess the morphological interaction between the nasal cavity and other aspects of the nasofacial skeleton. Previous studies indicate that the maxillary sinuses may play a key role in accommodating climatically induced nasal variation such that a decrease in nasal cavity volume is associated with a concomitant increase in maxillary sinus volume. However, due to conflicting results in previous studies, the precise interaction of the nasal cavity and maxillary si

Clinical and radiologic findings in a case series of maxillary sinusitis of dental origin.

BACKGROUND: Maxillary sinusitis of dental origin (MSDO) has been described for decades, but tends to be overlooked as a possible cause of chronic sinusitis by both clinicians and radiologists. The incidence of MSDO in published series is reported to be from 10% to 40% in bacterial sinusitis. We present this series to highlight clinical and radiologic indicators of MSDO. METHODS: Databases from the authors' otolaryngology and endodontic practices were reviewed to identify patients who had been seen mutually. Sixty-seven (67) patients were identified. Both authors then reviewed the clinical rec…

Cholesteatoma of the nose and maxillary and ethmoid sinuses: a rare complication of palatal surgery.

A 10-year-old boy presented with a complaint of a left-sided nasal obstruction with an associated foul-smelling discharge. Physical examination and anterior rhinoscopy revealed that a whitish, friable mass had completely filled the left nasal cavity. On computed tomography, a soft-tissue mass was seen filling the cavity and extending to the paranasal sinuses with bone erosion. A biopsy of the mass suggested that it represented a cholesteatoma. The lesion was removed via nasal endoscopy, and histopathology confirmed the diagnosis of a cholesteatoma. No recurrence was noted during 6 months of fo

Use of pharmacodynamic endpoints for the evaluation of levofloxacin for the treatment of acute maxillary sinusitis.

Sinusitis remains 1 of the most common reasons for antimicrobial prescriptions in the United States, with health care costs approaching $4 billion annually. We utilized the serial sinus aspirate sampling (SSAS) technique to obtain daily specimens to evaluate the time course of drug effect in patients with acute maxillary sinusitis. Eighteen patients with a radiologically confirmed acute maxillary sinusitis were enrolled into a study evaluating the relationship between levofloxacin exposure and the time course of antimicrobial effect using SSAS. SSAS was performed daily during therapy for bacte…

Determination of gender by measuring the size of the maxillary sinuses in computerized tomography scans.

BACKGROUND: Identification of corpses is a difficult forensic procedure and it is mandated by laws and social rules. Comparison of ante mortem and post-mortem medical records, such as dental documents, plays an important role in the identification of corpses. However, typical identification methods may be inconclusive, especially when certain extreme post-mortem changes have developed. Gender has long been determined from the skull, the pelvis and the long bones with an epiphysis and a metaphysis in unknown skeletons. The aim of this study was to investigate whether the width, the length and t

Molecular analysis of bacterial flora associated with chronically inflamed maxillary sinuses.

Chronic maxillary sinusitis is a chronic inflammatory condition in which the role of microbial infection remains undefined. Bacteria have been isolated from chronically inflamed sinuses; however, their role in the chronicity of inflammation is unknown. The objective of this study was to determine whether bacteria are present in clinical samples from chronic maxillary sinusitis and to assess the diversity of the flora present. Washes and/or tissue samples from endoscopic sinus surgery on 11 patients with chronic maxillary sinusitis were subjected to PCR amplification of bacterial 16S rDNA using…

Preliminary study of the deposition of aerosol in the maxillary sinuses using a plastinated model.

In spite of the widespread use of aerosols in respiratory diseases, very few studies have been performed in the field of ear, nose, and throat (ENT) disorders. The conditions for penetration of aerosols inside the sinus cavities are thus still not understood fully. The aim of this study was to investigate the penetration of aerosols inside maxillary sinuses in vitro, using plastinated models. Three plastinated specimens of the nose and sinuses were made from three different corpses. These specimens were validated by CT scans and were used to study deposition of aerosol in the maxillary sinuses

[Bacteriological study of chronic maxillary sinusitis in adults and observation of susceptibility to antibiotics].

The unilateral mucosal samples were taken from 42 patients and were cultured for bacteria. Meanwhile, the antimicrobial susceptibility was determined for 11 antibiotics and the beta-lactamase-producing bacteria(beta LPB) were examined. In 85.71% of all samples, bacterial growth was present. Anaerobic bacteria were present in 21.43% of cases. Predominant aerobic bacteria were alpha-hemolytic streptococci and coagulase-negative staphylococci. Predominant anaerobic bacteria were peptostreptococcus species and bacteroides species. The beta LPB were isolated from 40% patients, which were responsibl

The role of the ostiomeatal unit anatomic variations in inflammatory disease of the maxillary sinuses.

OBJECTIVE: The purpose of the study was to determine the correlation between bony anatomic variations of the ostiomeatal unit (OMU) and chronic maxillary sinusitis. The study was based on the hypothesis that the mucosal contact caused by the variations represents the critical factor in increasing the risk of maxillary sinusitis. MATERIALS AND METHODS: Thin section high resolution computerised tomography (CT) examinations of the paranasal sinuses in 73 consecutive patients with 113 anatomic variations of the OMU were retrospectively reviewed. The following CT features were assessed: (1) Type o…

Chronic maxillary sinusitis associated with the mushroom Schizophyllum commune in a patient with AIDS.

Invasive infection with fungi of the Basidiomycota (rusts, smuts, toadstools, mushrooms, and puffballs) is extremely rare. We report such an infection in a patient with human immunodeficiency virus disease who presented with chronic maxillary sinusitis associated with the mushroom Schizophyllum commune. The organism was isolated from the surgical drainage material, and septate hyphae were seen invading the maxillary submucosa. The limited literature on this subject is reviewed.

Treatment of acute maxillary sinusitis in childhood: a comparative study of amoxicillin and cefaclor.

Maxillary sinus aspiration and quantitative culture of the aspirate were performed in 50 patients, ranging in age from 1 to 16 years, with clinical and radiographic evidence of acute sinusitis. Of 79 sinuses aspirated, at least one was found to be infected in 35 (70%) children. Streptococcus pneumoniae, Branhamella catarrhalis, and Haemophilus influenzae were the most common organisms recovered. All H. influenzae were nontypeable. Twenty percent of the H. influenzae and 27% of the B. catarrhalis organisms were beta-lactamase positive and amoxicillin resistant. The subjects received either amox

[Preoperative roentgendiagnostic of the naso-maxillary sinuses and operative findings (author's transl)].

The preoperative roentgenmorphology of the nasomaxillary sinuses was compared with the intraoperative findings. The dates of 285 patients were evaluated. For the maxillary sinus there was correspondency in 85.7%. There were no differences in the cases explored with tomography. For the ethmoidal sinus there was correspondency in 89.5% and for the frontal sinus in 82.7%. The importance of the preoperative roentgenexamination with the standard exposure in occipitomental projection was less the demonstration of the inflammation than of other findings. The sphenoidal sinus was best explored by tomo

[Ultrasound testing in the diagnosis and management of maxillary sinusitis in children (author's transl)].

A- and B-mode ultrasound testing was used in conjunction with X-ray studies as screening tests for the detection of maxillary sinusitis in 280 children (aged 4-10 years) about to undergo adenoidectomy, and/or tonsillectomy and/or tympanostomy. X-rays were abnormal in 114 (41%) children, while sinus disease with the retention of discharge was diagnosed by ultrasound in 63 (23%) children. These latter children were then treated for ten days with amoxicillin and decongestant (Rinexin). At time of initial surgery, 63 maxillary sinuses (in 43 of the children) were punctured and irrigated. Another g…
