Frequency and Morbidity of Temporomandibular Joint Involvement in Adult Patients With a History of Juvenile Idiopathic Arthritis.

PURPOSE: Most patients with juvenile idiopathic arthritis (JIA) have temporomandibular joint (TMJ) involvement, but little is known about the natural history of TMJ disease as these children enter adulthood. The purpose of this study was to evaluate adults with a history of JIA to document the frequency and severity of TMJ abnormalities and morbidity. The authors hypothesized that most would have persistent TMJ disease as adults. MATERIALS AND METHODS: This cross-sectional study included adults (>19 years of age) with JIA who were managed at Boston Children's Hospital (BCH) as children and at

Bilateral Choanal Atresia With Facial Deformity.

BACKGROUND: Bilateral choanal atresia is a rare condition. Secondary posterior choanal atresia typically results from trauma, radiotherapy, and chemotherapy, whereas congenital cases are primarily caused by gene mutations. Congenital bilateral choanal atresia often presents as complete nasal congestion and discharge, which, if not promptly diagnosed and treated, can lead to fatal asphyxia. METHODS: This condition is rare and rarely reported in adults, and here we report a case of bilateral posterior nostril atresia in an 18-year-old male. The patient presented with congenital bilateral poster

Gorlin-Goltz syndrome in twin brothers: an unusual occurrence with review of the literature.

Gorlin-Goltz syndrome, also known as nevoid basal cell carcinoma syndrome, is caused by genetic alteration produced by a mutation in the 'patched' tumour suppressor gene, and is inherited in a dominant autosomal way. Although sporadic cases have been found, this syndrome has rarely been reported in twin patients. The syndrome is characterised by a wide range of developmental abnormalities and has a predisposition to neoplasms such as multiple pigmented basal cell carcinomas and keratocysts in jaws; it also has other features such as palmar and/or plantar pits and calcification of falx cerebri.

Multiple Extensive Odontogenic Keratocysts in a Child With Gorlin-Goltz Syndrome: A Case Report.

Gorlin-Goltz syndrome (GGS) is a rare autosomal dominant multisystem disorder characterized by a broad spectrum of developmental abnormalities and tumor predisposition. Multiple odontogenic keratocysts (OKCs) are one of its major diagnostic manifestations and may precede the development of other characteristic features. Early recognition of syndromic OKCs is particularly important in children, in whom other clinical manifestations may not yet be apparent. We present the case of a 13-year-old boy referred for evaluation of progressive facial asymmetry caused by extensive maxillary lesions. Maxi

The normal-equivalent: a patient-specific assessment of facial harmony.

Evidence-based practice in oral and maxillofacial surgery would greatly benefit from an objective assessment of facial harmony or gestalt. Normal reference faces have previously been introduced, but they describe harmony in facial form as an average only and fail to report on harmonic variations found between non-dysmorphic faces. In this work, facial harmony, in all its complexity, is defined using a face-space, which describes all possible variations within a non-dysmorphic population; this was sampled here, based on 400 healthy subjects. Subsequently, dysmorphometrics, which involves the me

Outcome of Temporalis Myofascial Flap in Maxillary Reconstruction.

The maxilla is an important component of the midfacial skeleton from both an aesthetic and functional view point. In addition to cosmetic impairment, surgical procedures in this region have substantial ramifications for speaking, swallowing and mastication, all of which may lead to psychological anguish for the patient. This necessitates rehabilitation with an obturator prosthesis or surgical reconstruction. The temporalis muscle flap has considerable potential for reconstructing various maxillofacial area abnormalities. It appears to be one of the finest solutions among all other regional fla

Tessier 3 cleft with bilateral anophthalmia: case report and surgical treatment.

Tessier clefts type 3 and 4 are rare. In this paper the authors report on the management of a wide Tessier 3 cleft. There is no standardized protocol or timing of the surgical procedures in this rare disfiguring condition. Generally speaking, the aim is to preserve the function of important anatomical structures (e.g., a seeing eye.) and reconstruct, as best as possible, harmonic facial features. The authors present a "step by step" solution of the malformation pointing out the limitations of the surgical procedures they used and the goals they wanted to obtain. Despite of the uniqueness and t

Treatment of maxillomandibular deformities with internal curvilinear distraction.

Internal curvilinear distraction is successful in achieving clinically significant distraction with stable occlusion in our patient population of infants, adolescents, and adults. When distracting the mandible, the curve of the distractor, the position of the distractor, and the osteotomy site are accountable for the final result. The curved distractor can mimic part of the natural logarithmic growth of the maxillomandibular complex. In addition, the result is predictable based on this design and the internal nature of the distractor, which can be left in place longer than other distractor typ

Long-term results of distraction osteogenesis of the maxilla and midface.

Since the beginning of 1998, eight patients have been treated by osteodistraction to correct hypoplasia of the maxilla and midface of various origins. Among them were five patients who were treated by high LeFort I osteotomies and insertion of subcutaneous intraoral distraction devices in the malar region. In the remaining three patients, extraoral distraction devices were applied after LeFort II and III osteotomies. Distraction osteogenesis was successful in all cases, resulting in a mean sagittal bone gain measured parallel to the skull base of 9.0 mm (range 4.5-12.0) in the group treated wi

Long-term evaluation of swallowing function before and after sagittal split ramus osteotomy.

The aim of this study was to determine whether mandibular setback by sagittal split ramus osteotomy (SSRO) influences swallowing function. The subjects were 14 patients with skeletal class III malocclusions who underwent setback surgery by SSRO. Morphological changes were studied on cephalograms, and swallowing function was evaluated by videofluorography before the operation (T0) and at 7-10 days (T1), 3 months (T2), and 6 months (T3) after surgery. The angle between nasion, sella, and hyoid bone (HSN) and the sella-hyoid distance had increased significantly at T1. The hyoid bone returned to t

Binder syndrome.

UNLABELLED: Patients with chondrodysplasia punctata (CDP) usually present with Binder-type features, and often CDP is misdiagnosed as Binder syndrome. This study reviewed the management and outcome of patients with Binder syndrome and CDP in a multidisciplinary setting. METHODS: The notes and radiographs of the patients managed at the Australian Craniofacial Unit with a multidisciplinary setting since 1976 were reviewed, and data were collected on patient demographics, associated medical and surgical problems, subsequent management, and complications. RESULTS: Seventy-seven patients were tre

Aplasia and agenesis of the frontal sinus in Turkish individuals: a retrospective study using dental volumetric tomography.

Agenesis of the paranasal sinuses is an uncommon clinical condition that appears mainly in the frontal (12%) and maxillary (5-6%) sinuses; in some populations, it appears at a higher proportion. This study investigated the prevalence of agenesis of the frontal sinuses using dental volumetric tomography (DVT) in Turkish individuals. The frontal sinuses of 410 patients were examined by DVT scans in the coronal planes for evidence of the absence of the frontal sinuses. A bilateral and unilateral absence of the frontal sinuses was seen in 0.73% and 1.22% of cases, respectively. In one case, both a

A 3-dimensional method for analyzing the morphology of patients with maxillofacial deformities.

INTRODUCTION: Traditionally, cephalograms have been used to evaluate a patient's maxillofacial skeleton and facial soft-tissue morphology. However, magnification and distortion of the cephalograms make detailed morphologic analysis difficult in patients with complex deformities. The purpose of this article was to introduce a new method for visualizing deformation and deviation of the maxillofacial skeleton and facial soft tissues. METHODS: Standard 3-dimensional Japanese head models were sized to match the sella-to-nasion distance obtained from 2 patients' (1 man, 1 woman) maxillofacial skele

A prenatally diagnosed patient with full monosomy 21: ultrasound, cytogenetic, clinical, molecular, and necropsy findings.

We report on a patient with a full monosomy 21 (FM21) prenatally diagnosed in cord fetal blood, and subsequently confirmed in other tissues. Subtle chromosomal translocations of chromosome 21, were ruled-out by FISH using both painting and 21q telomeric probes. Microsatellites analysis demonstrated the paternal origin of the single chromosome. The propositus showed at 32 weeks of gestation a severe intrauterine growth retardation and microcephaly. He was born with multiple congenital malformations, hypotonia, microcephaly, bilateral microphthalmia (more severe on the left), facial dysmorphism,

[Trans-facial distraction of the facial skull at the LeFort III level].

BACKGROUND: Several craniofacial deformities and syndromes feature hypoplasia of the midface skeleton. Until now different treatment options were based primarily on a modified LeFort III osteotomy: (1) the conventional LeFort III osteotomy with bone transplantation and intraoperative osteosynthesis, (2) distraction using subcutaneous distraction devices fixed on both sides at the os zygomaticum, and (3) bone distraction using a rigid external distraction device (RED distractor), which is fixed to the skull and pulling at the maxilla or the upper teeth. METHODS AND RESULTS: A modification of t

The genetic basis of normal and abnormal craniofacial development.

In recent years our knowledge of the genetic mechanisms behind animal development has increased exponentially, and it has become apparent that these mechanisms have been conserved to an astonishing extent during evolution. In this review some important groups of developmental regulatory genes are introduced, and their roles are discussed in the context of craniofacial morphogenesis. Transcription factors regulating both the identity and patterning of embryonic structures and the development of individual organs are often called master regulatory genes. These genes, as well as other transcripti
