In a four-year-old boy perforation of a peptic ulcer occurred in a Meckel's diverticulum located on a duplication of the small intestine. Complications in connection with Meckel's diverticulum and intestinal duplication are discussed, and also the indications for surgery. If the duplication is too large for resection, continuity of passage must be ensured.

A case is reported of perforated leiomyosarcoma of Meckel's diverticulum. The patient presented with localized peritonitis which responded to antibiotic therapy, and diagnosis was not made until elective resection of a tubulovillous adenoma of the rectum which was found in the course of investigation. Although rare, leiomyosarcoma is the commonest malignant tumour of Meckel's diverticulum.

In the last 8 years 15 cases of Meckel's diverticulum were observed, 6 of them with complications: three times inflammation (with two perforations), each once invagination, incarceration and occult bleeding from carcinoids. All complicated and 7 bland diverticula were removed and the intestine oversewed. The postoperative course was normal. Two cases were not treated. Considering possible complications it is recommended to search for a Meckel's diverticulum at each uncomplicated appendectomy or inferior laparotomy and to remove it if found.

Notes on the surgical management of Meckel's diverticulum in the light of 32 personal cases are presented. Stress is laid on the advisability of making a systematic search for this form during laparotomy and the removal on principle of even apparently unimpaired diverticula as a means of avoiding the very frequent and serious complications that may arise when diverticula are left in situ.

This paper describes our experience with pre-surgical diagnosis of Meckel's diverticulum. We used a naso-jejunal-ileal tube, through which, we injected small amounts of radio-opaque fluid and air, thus utilizing double contrast. In seven children diverticulum was shown to be present by radioscopic and radiographic techniques; this was later confirmed during laparotomy. X-ray findings were compared with Tc99 scanning results.
