Disparities in Antihypertensive Prescribing After Stroke: Linked Data From the Australian Stroke Clinical Registry.

Background and Purpose- Despite evidence to support the prescription of antihypertensive medications before hospital discharge to promote medication adherence and prevent recurrent events, many patients with stroke miss out on these medications at discharge. We aimed to examine patient, clinical, and system-level differences in the prescription of antihypertensive medications at hospital discharge after stroke. Methods- Adults with acute ischemic stroke or intracerebral hemorrhage alive at discharge were included (years 2009-2013) from 39 hospitals participating in the Australian Stroke Clinic

From Cost to Value: A Comparative Analysis of Medical and Surgical Management of Inflammatory Bowel Disease in a Public Referral Hospital in Mexico.

BACKGROUND AND OBJECTIVE: Inflammatory bowel disease (IBD) represents a growing clinical and economic challenge for healthcare systems, particularly in settings that provide free access to specialised care. Advances in medical therapy have modified disease outcomes but have also reshaped healthcare expenditure, while surgical management continues to represent high-impact cost events. Integrated analyses comparing medical and surgical costs from the perspective of public healthcare systems remain scarce in Latin America. This study aims to compare the economic burden of medical versus surgical-

Low Back Pain Presentation and Management at the Emergency Department: Differences Between Older Adults Residing in the Community and Aged Care Homes.

RATIONALE: In healthcare systems without gatekeeper access to Emergency Departments (ED), the number of people presenting for low back pain (LBP) is increasing substantially. Low back pain presentations at the Emergency Department are rarely caused by serious underlying pathology, and management often deviates from practice guidelines. Older adults (≥ 65 years) constitute approximately 30% of all ED LBP presentations. Little is known about differences in presentation characteristics and ED management between older adults from aged care homes and those living in the community. AIMS AND OBJECTI

Ibuprofen-Induced Renal Tubular Acidosis: Case Report on a Not-So-Basic Clinical Conundrum.

RATIONALE: Renal tubular acidosis (RTA) is a cause of non-anion gap metabolic acidosis (NAGMA) that is infrequently diagnosed and is due to various underlying etiologies that impair the kidney's ability to retain bicarbonate or excrete acid. Ibuprofen is an over-the-counter non-steroidal anti-inflammatory medication that is used by patients widely for a variety of reasons. Although it is well known that ibuprofen and other non-steroidal anti-inflammatory drugs may have nephrotoxic effects, the role of ibuprofen as a cause of RTA and hypokalemia is not well recognized. PRESENTING CONCERNS: A 6

A large community hospital's experience with an obstetrics-gynecology risk management committee: quality assurance.

In an era when medical-legal suits and awards are escalating, a major metropolitan hospital in Seattle, Washington, has developed a plan to identify and effectively deal with problem physicians, improve patient care, and reduce medical liability for the institution. The Department of Obstetrics and Gynecology of The Swedish Hospital Medical Center formed a risk management committee that met regularly to monitor and evaluate clinical complications and questions of professional incompetence or physician impairment. A system for detecting such cases was established through liaison with other hosp

The San Francisco Health Systems Collaborative: Public Health and Health Care Delivery Systems' Response to the Covid-19 Pandemic.

The Covid-19 pandemic challenged health care delivery systems worldwide. Many acute care hospitals in communities that experienced surges in cases and hospitalizations had to make decisions such as rationing scarce resources. Hospitals serving low-income communities, communities of color, and those in other historically marginalized or vulnerable groups reported the greatest operational impacts of surges. However, cross-institutional collaborations within jurisdictions offer unique opportunities to prevent or mitigate health disparities in resource utilization and access to care. In January 20

When hospitals build their own AI: a comparison of the EU and US regulatory framework.

Hospitals are increasingly shifting from being passive purchasers of artificial intelligence (AI) systems to active developers of medical AI, particularly in the form of in-hospital machine learning tools tailored to local clinical needs. This paper examines how the European Union (EU) and the USA regulate in-hospital-developed AI-based medical devices, focusing on the legal and institutional conditions that shape innovation, oversight and diffusion. Through a comparative analysis across three dimensions-responsiveness to clinical need, distribution of innovative capacity and pathways for scal

Lived experiences of young people with epilepsy in a resource-constraint setting: Contemporary findings from Cape Coast, Ghana.

Epilepsy is a chronic neurological condition that affects millions of young people globally, often leading to physical, cognitive, psychological, and social challenges. In resource-constraint settings, access to healthcare and medications, stigma, and psychosocial burdens can further exacerbate the impact of the condition. This study explored the lived experiences of young people with epilepsy (YPWE) who attend the Cape Coast Teaching Hospital, Ghana, focusing on emotional reactions to diagnosis, access to healthcare, coping strategies, support systems, and future aspirations. A qualitative st
