[Vitamin B9].

Vitamin B9 is represented by the group of folate, whose structure is derived from folic acid. The biologically active form is reduced tetrahydrofolates, serving as an essential cofactor in methylation reactions, including the vitamin B12-dependent formation of methionine from homocysteine, and as a carrier of one-carbon units involved in the synthesis of purines and pyrimidines. Folate deficiency is associated with hyperhomocysteinemia, megaloblastic anemia, leuco- and thrombocytopenia, cardiovascular disease, embryonic defects, in particular neural tube defects, and, possibly, malignancies, d

[Morphometric characteristics of erythrocytes in B12-deficiency anemia (based on computer morphometric data)].

The paper presents the results of computed morphometry of peripheral red blood cells in elderly patients with B12-deficiency anemia before treatment. The study was performed in the fine Romanovsky-stained peripheral blood smears, by using an ASPEK Russian hematological cell image analyzer. The objective quantitative characteristics of the parameters of the major megaloblastic hematopoiesis markers (macro- and megalocytes) in peripheral blood are given. The composition of peripheral blood was determined from the content of red blood cells with varying amounts of hemoglobin.

Folate deficiency presenting as pyrexia: a case report.

Folate deficiency is an uncommon cause of pyrexia. We describe the case of a 29-year-old male who presented with a pyrexial illness subsequently attributed to megaloblastic anaemia secondary to severe folate deficiency, after exclusion of other infective or inflammatory causes. A temperature chart documenting the course of the patient's pyrexia is presented and potential pathophysiological mechanisms are proposed. Folate deficiency is a reversible cause of pyrexia that should be considered in any patient who presents with a pyrexial illness of unknown cause.

Severe thrombocytopenia associated with acute folic acid deficiency and severe hemorrhage in two patients.

Severe thrombocytopenia associated with acute folic acid deficiency in two inpatients is described. Both patients had severe hemorrhagic manifestations. A similar syndrome has previously been described in patients who have undergone extensive surgery or dialysis or both. The present cases show that acute folic acid deficiency can occur in other circumstances and may go unrecognized because other signs in the peripheral blood of a megaloblastic process may be minimal. A good response was obtained with folic acid therapy.

Imerslund-Gräsbeck syndrome presenting with a 12-year history of intermittent proteinuria and anemia: a case from the Middle East.

BACKGROUND: Imerslund-Gräsbeck Syndrome (IGS) is a rare autosomal recessive disorder characterized by selective cobalamin (vitamin B12) malabsorption and often accompanied by proteinuria. Mutations in CUBN or AMN genes underlie the condition, which usually manifests in childhood with megaloblastic anemia, failure to thrive, or recurrent infections. CASE PRESENTATION: We report a 15-year-old Iranian boy with a 12-year history of recurrent anemia and intermittent proteinuria. He presented with fatigue, nausea, and anorexia. Laboratory evaluation showed pancytopenia, macrocytic anemia (Hb 7.5 g/

Pattern of haematological diseases diagnosed by bone marrow examination in Yemen: a developing country experience.

There is lack of information about the relative prevalence of haematological disorders in Yemen and other Middle East countries. The aim of this study was to evaluate the pattern of haematological diseases diagnosed by bone marrow examination in Yemen considering the limited diagnostic facilities. At the referral haematology centre in Yemen, between November 1999 and November 2005, 785 patients >14 years old were evaluated by bone marrow examination. Relevant investigations were performed when needed. A total of 627 patients had haematological disorders other than lymphoma, and their data were

Deoxyuridine suppression: biochemical basis and diagnostic applications.

The deoxyuridine (dU) suppression test evolved out of investigations into the biochemical basis of the megaloblastic changes seen in vitamin B12 and folate deficiency. Although the abnormality in dU suppression which occurs in vitamin B12- or folate-deficient states is assumed to reflect impaired methylation of deoxyuridylate, there is still no direct demonstration that this is so. Furthermore, there is evidence that reactions other than the methylation of deoxyuridylate are involved in the phenomenon of dU suppression. Nevertheless, in clinical practice abnormal dU suppression serves as a sen

Knuckle Pigmentation as an Early Cutaneous Sign of Vitamin B12 Deficiency: A Case Report.

Vitamin B12 deficiency can present with variable hematological, neuropsychiatric, and mucocutaneous changes. Hyperpigmentation, specifically involving the knuckles has been described in vitamin B12 deficiency, but usually, these patients are symptomatic with systemic manifestations like megaloblastic anemia, pancytopenia, or neurological deficits. Here, we are reporting a case of nutritional vitamin B12 deficiency, who presented with isolated knuckle pigmentation and was successfully treated with oral therapy. This case also highlights the importance of recognizing this cutaneous sign as an ear

Treatment of genetic defects of thiamine transport and metabolism.

INTRODUCTION: Thiamine is a key cofactor for energy metabolism in brain tissue. There are four major genetic defects (SLC19A2, SLC19A3, SLC25A19 and TPK1) involved in the metabolism and transport of thiamine through cellular and mitochondrial membranes. Neurological involvement predominates in three of them (SLC19A3, SCL25A19 and TPK1), whereas patients with SLC19A2 mutations mainly present extra-neurological features (e.g. diabetes mellitus, megaloblastic anaemia and sensori-neural hearing loss). These genetic defects may be amenable to therapeutic intervention with vitamins supplementation a

Reversible pancytopenia and immunodeficiency in a patient with hereditary folate malabsorption.

Mutations in SLC46A1 result in a defect of the proton coupled folate transporter (PCFT) and are the basis of hereditary folate malabsorption (HFM). Patients with HFM frequently present with neurodevelopmental delay and megaloblastic anemia. Some cases may be complicated by additional lymphopenia and immunodeficiency. We report a patient with a new homozygous mutation in the SLC46A1 gene. The boy presented with early-onset pancytopenia and secondary immunodeficiency. We provide clinical and molecular observations that extend the phenotypic description of HFM and highlight diagnostic as well as

Cutting edge issues in autoimmune gastritis.

Autoimmune gastritis is the outcome of a pathological CD4 T cell-mediated autoimmune response directed against the gastric H/K-ATPase. Silent initially, the gastric lesion becomes manifest in humans by the development of megaloblastic pernicious anemia arising from vitamin B12 deficiency. Cutting edge issues in this disease relate to its epidemiology, immunogenetics, a role for Helicobacter pylori as an infective trigger through molecular mimicry, its immunopathogenesis, associated organ-specific autoimmune diseases, laboratory diagnosis, and approaches to curative therapy.

Oral or intramuscular vitamin B12?

Vitamin B(12) deficiency is common, becoming more so with age, and estimates of its population prevalence have ranged from 1.5% to 15%. If untreated, it can lead to megaloblastic anaemia and irreversible neurological complications. In the UK, the usual treatment is regular intramuscular injections of hydroxocobalamin. High-dose oral vitamin B(12) replacement is standard practice in some other countries and less costly. Here we review issues around adopting an oral vitamin B(12) replacement regimen more widely in the UK.

Dietary folate deficiency with normal red cell folate and circulating blasts.

This report describes a 26 year old woman, of Pakistani origin, who presented five months postpartum with severe megaloblastic anaemia as a result of nutritional folate deficiency. This case was unusual in that a small number of myeloblasts were present in the peripheral blood at presentation, and this circulating population temporarily increased in size when folate replacement was begun. We also highlight the need to recognise the non-linear relation between haematocrit and red blood cell folate concentration when the haematocrit is very low (< 0.15) and emphasise the importance of the clinic

[Pernicious anemia in a thalassemic patient - difficulties of the diagnosis].

The bone marrow aspiration, which was done in a leukopenic, hypochromic, microcytic, progressive anemic, thalassemic patient, revealed megaloblastic morphology. The low level of vitamin B12 and the reticulocytosis following the B12 supportation strenghtened the diagnosis of pernicious anemia. The set of the right diagnosis has been delayed by the fact that even in severe anemia one could not obtain the typical signs of B12 deficiency, having a hypochromic, microcytic erythrocyte morphology, due to the thalassemia minor disorder.
