The DECISION study: protocol for the development and internal validation of a multimodal decision-making capacity framework.

BACKGROUND AND OBJECTIVES: Decision-making capacity (DMC) is a prerequisite for valid informed consent, as well as being a key methodological requirement in clinical and epidemiological research. For individuals with neurocognitive disorders, DMC assessment is frequently based on subjective clinical judgment, with limited standardization and significant variability across settings. Current instruments are resource-intensive and not sufficiently adapted for stage-specific or real-world applications. To develop and internally validate a structured multimodal framework for assessing DMC in indivi

Involuntary treatment of anorexia nervosa in Ireland: Challenges and changes in the legal framework.

Anorexia Nervosa is an eating disorder typified by low body weight, restrictive eating behaviours, and body image distortion. It is associated with significant risk of medical complications, with one of the highest mortality rates of any mental illness. While the majority of patients receive treatment on a voluntary basis, a small proportion of severely ill patients refuse treatment and are treated involuntarily. The legal mechanisms used for involuntary treatment vary between jurisdictions, including mental health law, capacity-based law, guardianship, and use of inherent jurisdiction, the po

A systematic review culturally adapted Cognitive Behavioral Therapy (CaCBT) for Arabic-speaking populations including North African subgroups.

BACKGROUND: Cognitive behavioral therapy (CBT) is an effective treatment for common mental disorders. However, most CBT protocols have been developed within Western cultural frameworks, which may limit their relevance for culturally diverse populations. Culturally adapted CBT (CaCBT) seeks to enhance treatment acceptability and effectiveness by integrating linguistic, cultural, religious, and contextual factors. Despite growing interest in culturally adapted interventions, evidence regarding Arabic-speaking populations, including North African subgroups, remains fragmented. METHODS: Following

Implementing the Cultural Formulation Interview in a Community Clinic to Improve Appointment Retention: A Pilot Study.

Despite consensus that clinicians should receive cultural competence training to reduce mental health disparities among racial-ethnic minority groups, few studies have evaluated training in community settings. This column delineates implementation of the DSM-5 Cultural Formulation Interview (CFI) at a community hospital. Participating clinicians were able to implement the CFI after a single training session and offer administrative support and instructions on customizing care with lower- and middle-income, racially and ethnically diverse patients.

[Medico-legal implications of coronary angioplasty among seniors].

It's commonplace to declare that seniors are more fragile, because of a higher rate of comorbidities and complex coronary lesions. However, they take great benefit from angioplasty during acute coronary syndrome, with a higher rate of complications. Based on the study of claims reported to a French medical insurer, it is found that people over 75 years old among victims of interventional cardiology accidents is proportional to the population of seniors in the French population (around 10%). Conversely, the mortality of those seniors was much higher than patients under 75 (83% versus 30%). A st

'More of a leg to stand on': views and usage of the Mental Capacity Act 2005 among staff of local Alzheimer's Society and carer organisations.

OBJECTIVES: The Mental Capacity Act (MCA) 2005 was implemented in 2007 in England and Wales. It offers new opportunities for people to plan for any future loss of capacity and provides a framework for decision-making on behalf of people lacking capacity. This article explores the challenges staff in local voluntary organisations focusing on dementia and carers face when they offer information and advice connected to possible loss of decision-making capacity to people with dementia and carers. METHOD: A set of 17 qualitative interviews were undertaken with a purposively sampled voluntary secto

[Tubal sterilization in mentally handicapped women].

ETHICAL ISSUES: When performed for contraception purposes, tubular sterilization for mentally handicapped women poses important ethical issues, including patient's rights, body integrity, and the notion of informed consent. French law guarantees the respect and safety of all patients, but in everyday practice, patient's rights must be upheld by family and healthcare workers searching for the most adapted solutions for each individual situation. PROPOSITIONS: We present here our proposals for everyday practice. Our conclusions are based on an analysis of the notion of handicap as defined by th

Capacity to vote in persons with dementia and the elderly.

The capacity to vote in patients with mental illness is increasingly questioned. The objective of this study is to evaluate this capacity in a group of subjects with dementia (Alzheimer's disease) and other elderly subjects without dementia. With a sample of 68 subjects with dementia and 25 controls living in a senior residence, a transversal study was carried out over 4 months. Subjects were evaluated with the Mini-Mental State Examination (MMSE) and the Competence Assessment Tool for voting (CAT-V). The results were more positive for the Doe criteria (as part of the CAT-V), and a correlation

Toxic masculinity as a barrier to mental health treatment in prison.

The current article addresses gender issues that become magnified in prison settings and contribute to heightened resistance in psychotherapy and other forms of mental health treatment. Toxic masculinity involves the need to aggressively compete and dominate others and encompasses the most problematic proclivities in men. These same male proclivities foster resistance to psychotherapy. Some of the stresses and complexities of life in men's prisons are explored. The relation between hegemonic masculinity and toxic masculinity is examined. The discussion proceeds to the interplay between individ

Crossing the chasm: engaging Black men survivors of gun violence in mental health services.

BACKGROUND: Despite being high risk for post-traumatic stress disorder, Black men survivors of gun violence, and particularly young men aged 18-24, seldom participate in mental health services after injury. The aim of this study was to identify barriers to participation in mental health services for this population. METHODS: Over a 2-year period, 1 hour-long focus group was conducted with three counselors of the local hospital-based violence intervention program and 21 individual, semistructured in-depth interviews were held with Black men who were hospitalized for a firearm-related injury. Utilization of telemedicine in healthcare delivery to lesbian, gay, bisexual, transgender, queer, intersex, asexual, other sexual and gender minority (LGBTQIA+) populations: a scoping review.

This scoping review examines how telemedicine addresses healthcare needs in the lesbian, gay, bisexual, transgender, queer, intersex, asexual, and other sexual and gender minority (LGBTQIA+) community, focusing on gender-affirming care, mental health, and testing for human immunodeficiency virus (HIV) and sexually transmitted infections (STIs). A literature search of MEDLINE, Embase, Web of Science, and Scopus was conducted to identify studies published until March 2024 focusing on telemedicine services for LGBTQIA + individuals. Data extraction captured study characteristics, telemedicine app

Eliciting social stressors, supports, and determinants of health through the DSM-5 cultural formulation interview.

OBJECTIVE: As recognition of the importance of social determinants of mental health has increased, the limitations of clinical competence-enhancing interventions that do not emphasize this approach have emerged. The Cultural Formulation Interview (CFI) is a cultural competence intervention that emerges from a confluence of social medicine and medical anthropology traditions. Limited research has examined how patients respond to CFI questions on social-structural aspects of illness and care to assess whether the CFI adequately elicits information on social determinants of mental health. Targeting intrusive imagery using a competing task technique: a case study.

BACKGROUND: Even in cases with complexity, simple techniques can be useful to target a specific symptom. Intrusive mental images are highly disruptive, drive emotion, and contribute to maintaining psychopathology. Cognitive science suggests that we might target intrusive images using competing tasks. AIMS: We describe an imagery competing task technique within cognitive behavioural therapy (CBT) with a patient with bipolar disorder and post-traumatic stress disorder (PTSD) symptoms. The intervention - including Tetris computer game-play - was used (1) to target a specific image within one the

Growing up in the shadow of Chornobyl: adolescents' risk perceptions and mental health.

PURPOSE: Despite long-term research on risk perceptions of adults after ecological disasters, little is known about the legacy for the generation exposed to toxic elements as infants. This study examined Chornobyl-related risk perceptions and their relationship to mental health in adolescents raised in Kyiv in the aftermath of the accident. METHODS: Risk perceptions, 12-month DSM-IV major depression (MDD)/generalized anxiety disorder (GAD), and current symptomatology were examined in 265 evacuee adolescents, 261 classmate controls, and 327 population-based controls 19 years after the accident

History of Minor Consent Laws for Mental Health Treatment in the US.

IMPORTANCE: To facilitate access to mental health treatment for minors who are unwilling to involve parents or guardians in their care, states have enacted statutes granting minors the legal capacity to independently consent (ie, without parental/guardian consent) to mental health services. Understanding changes in these laws over time may facilitate research that assesses whether these laws increase access to care, and support legal and clinical interventions to advance minors' access to mental health care. OBJECTIVE: To analyze the adoption and amendment of minor consent laws for mental hea

The association between paternal PTSD and child mental health: a systematic literature review.

Fathers play a crucial role in their children's psychosocial development, positively influencing emotional, cognitive, and social competencies. Paternal Post-Traumatic Stress Disorder (PTSD) can disrupt this dynamic, potentially compromising the psychological well-being and developmental outcomes of their offspring. This systematic review aims to investigate the relationship between paternal PTSD and the prevalence of psychopathology in children and adolescents, providing a comprehensive understanding of how trauma experienced by fathers may influence their children's mental health. A systemat

In-Theater Mental Health Disorders Among U.S. Soldiers Deployed Between 2008 and 2013.

Deployed service members regularly undergo demanding and stressful experiences that can contribute to mental health difficulties; however, there is a scarcity of studies examining rates of mental health disorders in-theater. The current study examined case rates of mental health disorders among deployed U.S. Army Soldiers using diagnostic encounter data from the Theater Medical Data Store. Case rates were calculated across 12 categories of mental health disorders. While in theater, soldiers' highest rates were for stress reactions and adjustment disorders, depression, anxiety, and sleep disord

Improving Access to Mental Health Services for Racialized Immigrants, Refugees, and Non-Status People Living with HIV/AIDS.

The demographic characteristics of people living with HIV/AIDS (PHAs) in Canada are increasingly diverse. Despite literature suggesting a potentially heightened mental health burden borne by racialized immigrant, refugee, and non-status PHAs (IRN-PHAs), researchers have hitherto paid insufficient attention to whether existing services adequately address this need and how services might be improved. Employing community-based research methodology involving PHAs from five ethnoracial groups in Toronto, Ontario, this study explored IRN-PHAs' mental health service-seeking behaviors, service utiliza
