A new technique is presented for the treatment of comminuted intraarticular fractures of the base of the thumb metacarpal. A triangular Hoffmann external radius and first- and second-metacarpal fixator is used. The reduction is maintained by prolonged distraction and abduction of the first metacarpal bone. The application of the technique in three cases is reported.

The use of an expandable intramedullary device for internal fixation of metacarpal or phalangeal bones is described. The device is applicable to transverse fractures, short oblique fractures, or transverse osteotomies. The device consists of a cylindrical apparatus made of titanium that allows collapse in the circumferential diameter. A two-year-old female with the Pierre Robin anomaly and bilateral index finger malformations is described. Hypertelorism, full cheeks, posteriorly rotated ears with prominent antihelix, short neck, simian creases, bilateral fifth finger clinodactyly, and short toes with hypoplastic small nails were also present. Solid bony union with full range of painless motion is accomplished by correct early diagnosis, adequate reduction, stable fixation, prevention of edema, and institution of early motion programs. The degree of bony injury and involvement of multiple tissues will dictate the type of fixation and length of immobilization. Restoration of a stable skeleton with a functional range of joint motion must always be accomplished.

Fifty nine hands were studied in 42 patients with central ray deficiency. Thirty one of the hands were treated surgically and this enabled analysis of the soft tissue abnormalities that were not constant throughout the group, requiring a sub-division of the usual classification. Rupture of the extensor tendons is common in longstanding rheumatoid arthritis of the hand. The tendons commonly involved are those of the ring and little fingers and the common site of rupture is over the dorsal ulna and distal edge of the extensor retinaculum. We are reporting two cases of extensor tendon rupture over the metacarpal head of the middle finger due to prominent osteophytes.

The analysis of measurements of phalangeal lengths with the metacarpophalangeal pattern profile plots (MCPPP), has been further extended. I suggest to correlate these measurements to the total length of the phalanges in order to obtain only one mean and standard deviation table for all ages from 3 years to adults. Over a 6-year period a total of 23 greyhounds and 10 dogs of other breeds were presented with metacarpal and metatarsal fractures. While in other breeds trauma was the usual cause, in greyhounds the injuries were stress or fatigue fractures. In the greyhounds the fractures occurred mainly in young dogs and were often comminuted. Fifteen cases involved the fifth metacarpal bone of the left foreleg. The fractures were managed by either external immobilisation (8 cases) or internal fixation, mainly with lag screws (14 cases). A case of brachymesophalangy in which ossicles related to metacarpal heads became detached, causing joint locking, is presented. A distinction has been made by arthrography between this lesion and the loose bodies seen in osteochondritis dissecans, which were previously thought to be the origin of the ossicles and the cause of the locking.

Osteochondroses is a well-known entity and typically affecting immature skeleton with few common locations involved are femoral head epiphysis, tibial tubercle apophysis, calcaneal apophysis, humeral capitellum and anterior vertebral end plates. We report a rare case of osteochondroses showing avascular necrosis involving metacarpal heads known as Dieterich disease, involving the head of the third metacarpal and probably the first case with a history of rock climbing as an etiology.
