Otitis media with effusion: benefits and harms of strategies in use for treatment and prevention.

Otitis media with effusion (OME) is a common clinical condition that is associated with hearing loss. It can be diagnosed at least once in approximately 80% of preschool children: 30-40% of them have recurrent episodes, and 5-10% have chronic disease. OME, in recurrent and persistent cases, might significantly delay or impair communication skills, resulting in behavioral and educational difficulties. Several therapeutic approaches have been used to avoid these problems. Most, however, have not been adequately studied, and no definitive conclusions can be drawn. Official guidelines do not recom

Intelligent vision guide for automatic ventilation grommet insertion into the tympanic membrane.

BACKGROUND: Otitis media with effusion is a worldwide ear disease. The current treatment is to surgically insert a ventilation grommet into the tympanic membrane. A robotic device allowing automatic grommet insertion has been designed in a previous study; however, the part of the membrane where the malleus bone is attached to the inner surface is to be avoided during the insertion process. METHODS: This paper proposes a synergy of optical flow technique and a gradient vector flow active contours algorithm to achieve an online tracking of the malleus under endoscopic vision, to guide the worki

Attic aeration in temporal bones from children with recurring otitis media: tympanostomy tubes did not cure disease in Prussak's space.

HYPOTHESIS: Blockage of the aeration pathways to the attic may cause circumscribed or widespread alterations that are difficult to diagnose clinically. The narrow route via the posterior pouch to Prussak's space is especially vulnerable to obstruction in recurring otitis media. BACKGROUND: Recent studies of the epitympanic diaphragm and compartments have clarified the anatomy of the attic aeration and drainage pathways and emphasized the role of their patency in the healing process of middle ear infections. In neonatal otitis media, the amniotic fluid cellular content (AFCC) has proved to be

Late effects on ear disease in otitis-prone children: a long-term follow-up study.

A group of 88 otitis-prone children, 66% boys and 34% girls, born in 1978-81 with more than 11 episodes of acute otitis media, were examined in 1984-85. Seventy-four of the children (with the same distribution of sex) were reexamined in 1990-92. Their medical history of ear, nose and throat diseases was studied and an examination, including otomicroscopy, tympanometry and audiometry, was performed. Fifty-one (69%) continued to have episodes of acute otitis media during the follow-up period and 34 (46%) continued to have episodes of secretory otitis media. Surgery had been performed during this

Tympanic membrane perforation following ventilation tube removal in a pediatric setting: a historical study.

This study is concerned with the rate of tympanic membrane perforation following the surgical removal of ventilation tubes. Consideration for inclusion in the study was given to all patients who had ventilation tubes surgically removed between 1982 and 1991 at The Hospital for Sick Children. All patients included in the study were followed for a minimum period of six months. The patients ranged in age from two to 13 years old, with a mean age of 4.7 years. Our study followed 203 ears which met the entry criteria. It was found that there were 21 (10.3%) documented perforations within this study

[Water precautions after insertion of a tympanostomy tube: necessary or obsolete?].

INTRODUCTION: Since the introduction of tympanostomy tubes by Armstrong in 1952, physicians and patients alike have been concerned about the possible harm associated with water entering the middle ear via tubes. It is the current practice of many physicians to advise patients to avoid water entering the middle ear by using water precautions when swimming. However, the potential harmful effect of water in causing otorrhoea and otalgia still remains controversial. MATERIALS AND METHODS: A prospective study was performed analysing the effect of water exposure in patients with tympanostomy tubes,

Effect of Water Precautions on Otorrhea Incidence after Pediatric Tympanostomy Tube: Randomized Controlled Trial Evidence.

OBJECTIVES: Tympanostomy with ventilation tube insertion is the most common otologic surgery. Many surgeons recommend water precautions, although its utility is questioned. We aimed to investigate if water precautions reduce the rate of otorrhea after transtympanic tube insertion. STUDY DESIGN: Multicenter randomized controlled trial. SUBJECTS AND METHODS: A total of 244 children aged 2 to 10 years undergoing their first set of Shepard tubes for otitis media with effusion and concomitant adenoidectomy were randomized to 2 groups: 1 with ear protection during water exposure (ear plugs and hea

[The role of computerized rheoencephalography in the assessment of normal pressure hydrocephalus. Preliminary report].

UNLABELLED: Background: Anterior epitympanic recess (AER) is a small anatomical space in the epitympanum anterior to level of the head of malleus. This space has received lot of attention because of its role in cholesteatoma. Dysventilation of AER can lead to retraction pockets and cholesteatomas. Due to the advent of endoscopic middle ear surgeries the visualization of the mucosal folds and spaces have been possible for the past 2 decades. Mucosal folds and spaces play an important role in middle ear ventilation and pathologies obstructing these ventilation pathways can lead to dysventilation…

Balloon Eustachian Tuboplasty in children.

Endonasal ballon dilatation of the Eustachian tube (BET) is a promising treatment for Eustachian tube dysfunction with encouraging results over the last years in adults. In addition, in children, single studies demonstrated promising results, but revealed the necessity for broader and additional studies. Our retrospective analysis presents outcomes with BET in children with chronic obstructive dysfunction of the Eustachian tube, showing resistance to the conventional therapy after adenotomy with paracentesis or grommets (ventilation tubes). The data of 52 children, having undergone BET from Ap

Use of the malleus handle as a landmark for localizing the round window membrane.

Localization of the round window membrane is desirable in order to allow a more direct application of medication to the inner ear. A new procedure has been developed to deliver direct, near-continuous inner ear perfusion to the round window with the Silverstein MicroWick. In this office procedure, the wick is inserted through a tympanostomy tube into the round window niche. Accurate localization of the round window is a necessary component of this procedure. In an effort to ascertain the precise location of the round window, we examined 25 cadaveric human temporal bones and measured the distan

Tensor veli palatini electromyography for monitoring Eustachian tube rehabilitation in otitis media.

BACKGROUND: The pathogenesis of otitis media is related to Eustachian tube dysfunction. The tensor veli palatini muscle actively opens the Eustachian tube and promotes middle-ear ventilation. This study describes a technique for paratubal electromyography that uses a surface, non-invasive electrode able to record tensor veli palatini muscle activity during swallowing. METHODS: Twenty otitis media patients and 10 healthy patients underwent tensor veli palatini electromyography. Activity of this muscle before and after Eustachian tube rehabilitation was also assessed. RESULTS: In 78.5 per cent

Comparison between the eardrop method with tympanostomy tube and intratympanic injection in a rat model.

OBJECTIVE: Medications for middle-ear diseases are commonly delivered into the middle-ear cavity either by the eardrop method with a tympanostomy tube followed by tragal pumping or by intratympanic injection. The solution delivery of the eardrop method was compared with that of intratympanic injection. METHODS: Thirty rats were used. For the eardrop method, tympanostomy tube insertion, dye solution eardrop application and tragal pumping were performed in one ear. Intratympanic injection of the same solution was performed in the other ear. The ratio of the area stained by the dye solution to t

[The function of tubomanometry in forcasting the progonosis of acute otitis media with effusion].

OBJECTIVE: To evaluate the function of tubomanometry (TMM) in forcasting the progonosis of acute otitis media with effusion (OME). METHOD: We used the technique of TMM to quantify the degree of eustachian tube (ET) dysfunction in 65 patients with OME. The opening of the ET and the transportation of gas into the middle ear were registered by a pressure sensor in the occluded outer ear after applying the stimulus of a controlled gas bolus into the nasopharynx during swallowing. Three excess pressure values were tested subsequently (30, 40, and 50 mbar). If tube opening was registered, the time
